Post Office Box 2770 La Plata, Maryland (301) Information Sheet and Checklist for Charles County Scholarship Fund Applicants Please make sure you read the criteria for each scholarship and apply for ALL of the scholarships for which you QUALIFY. Only one application needs to be completed. COMPLETED applications must have the following documents in the following order: Cover sheet of the application completed (print or type) and signed. Scholarship boxes checked on pages 3-6 for which you meet the criteria. A copy of your resume that MUST include the following: 1. Work experience, 2. Extra-curricular activities, and 3. Community Service activities during high school. A typewritten personal statement on a separate sheet that MUST include the following: 1. Academic Goals and 2. Career Goals Two letters of recommendation (unless scholarship criteria states otherwise); one from an academic source and one from someone who knows you well Any additional typewritten comments you or your parent wish to make on a separate sheet that explain any financial need or circumstances that you or your parent(s) or guardian(s) feel might help the Scholarship Selection Committee make its decision An official copy of your high school transcript and a copy of your first semester senior year report card You may enclose your SAT/ACT scores if you have them If you apply for any of the scholarships whose criteria include merit and financial need,* you MUST print and enclose a complete copy of the Student Aid Report (SAR) form for the FAFSA (Print out the SAR from Applications must be turned in to your CCA in the Career Center of your high school by the end of the school day Wednesday, March 1, 2017 or mailed with sufficient postage and postmarked by Wednesday, March 1, 2017 to: Charles County Scholarship Fund, Attn: Alicia Jones, P.O. The following pages contain a listing of available scholarships. Be sure to check all of the scholarships for which you meet the eligibility criteria. The scholarships are separated into two lists: those based strictly on merit and those that consider both merit and financial need. The complete copy of the Studen Aid Report (SAR) form for the FAFSA must be submitted for the scholarships based on financial need. * This application must be submitted to your High School College and Career Advisor in the Career Center by the end of the school day Wednesday, March 1, 2017 or mailed with sufficient postage and postmarked by Wednesday, March 1, 2017 to the address above. 2
3 Merit Scholarships Richard J. Abela Scholarship: ($2,000) Graduating senior from North Point HS with a minimum 3.0 GPA who played baseball and/or football at North Point HS all four years and plans to enroll in a four-year college. Must include a recommendation from the high school coach. Michael J. Anderson Memorial Scholarship: ($1,000) Graduating senior from La Plata HS with a minimum 3.5 GPA, Charles County resident. Complete and attach an additional essay (up to 300 words) describing the applicant s commitment to and demonstrated performance in community service. Darren A. Bowie Scholarship: ($1,000) Graduating senior from Lackey HS with a minimum 3.5 GPA who has taken two years of AP English, Charles County resident. Jeremiah and Elijah Borgnis Memorial Scholarship: ($500) Previous graduate or a graduating senior from a Charles County public HS with a minimum 3.0 GPA, Charles County resident, must major in Visual Art. Unnae Pak-Borgnis Memorial Scholarship: ($500) Previous graduate or a graduating senior from a Charles County public HS with a minimum 3.0 GPA, Charles County resident, must major in Music. Charles County Health Department and School Nurses Scholarship: ($250) Graduating senior or first-year nursing student with a minimum 3.0 GPA, Charles County resident who plans to enroll or is enrolled in a registered nursing program. Charles County Teacher Education Assistance Grant: ($1,000 renewable) Charles County resident must major in Teacher Education and intend to apply for a teaching position in Charles County public schools after graduation. John H. Cox Memorial Scholarship: ($1,000) Graduating senior/scholar athlete with four varsity letters and a minimum 3.75 GPA, Charles County resident. Ronald G. Cunningham Scholarship: ($2,500) Graduating senior from each Charles County public HS with a minimum 2.5 GPA, Charles County resident who is planning to enroll full-time in Teacher Education. (One scholarship is given at each Charles County public HS.) Harding Memorial Future Educator Scholarship: ($1,000) Graduating senior from a Charles County HS who is a Charles County resident planning to enroll full-time in the fall in Teacher Education. Preference is given to a member of a high school chapter of Future Educators of America (FEA). William and Vivian King Scholarship: ($1,000) Graduating senior from a Charles County HS with a minimum 3.2 GPA, Charles County resident. (One scholarship is given at each Charles County public HS.) Kiwanis Club of Charles County Ronald G. Cunningham Scholarship: ($500) An active Key Club member in good standing who is graduating from a Charles County public HS with a minimum 2.5 GPA; Charles County resident. Attach both a brief statement outlining your community participation through Key Club and an additional letter of recommendation from your Key Club Leader or Club Liaison. 3
4 Thomas Kurtz Memorial Scholarship: ($1,000) Graduating senior from Lackey HS with a minimum 3.2 GPA, Charles County resident. Complete and attach the separate application available from the Career Center. La Plata Garden Club Scholarship: ($1,000) Graduating senior with a minimum 3.0 GPA, Charles County resident and U. S. citizen who plans to major in Plant Science, Environmental Science, Animal Science, or Horticulture at an accredited college or university. (Two awards) Mary Matula Scholarship: ($500) Graduating senior from La Plata HS showing scholarship, leadership, and service to school and community. Timothy Minor Criminal Justice Memorial Scholarship: ($500) Graduating senior from a Charles County public HS with a minimum 3.0 GPA, Charles County resident, must major in Criminal Justice. Timothy Minor Memorial Scholarship: ($500) Graduating senior from Maurice J. McDonough HS with a minimum 3.0 GPA, Charles County resident. Rachel Myers Trade School Scholarship: ($500) Graduating senior or former graduate from a Charles County public HS who will attend or is attending an accredited vocational/technical school, Charles County resident. Nicole Pitonyak Memorial Scholarship: ($1,000) Graduating senior from North Point HS with a 3.0 GPA who sang in the NPHS Choir, Charles County resident who will enroll in a vocal music or theater arts degree program. Katherine D. Racey Memorial Scholarship: ($1,500) Graduating senior from Lackey or McDonough HS, Charles County resident with a minimum 3.0 GPA, must major in Math Education, Education or a math-related field. Edward Rorer Memorial Scholarship: ($500) Graduating senior from a Charles County public HS who was enrolled in a Career & Technology Center/North Point STI class with a minimum 3.0 GPA. Eric Sawchak Memorial Scholarship: ($1,000) Graduating senior from a Charles County public HS with a minimum 3.0 GPA who is on the high school Golf Team, Charles County resident. Ken Stump Memorial Scholarship: ($500) Graduating senior from a Charles County public HS, Charles County resident with a minimum 3.0 GPA, must major in Math or a math-related field. Michael S. Tayman Memorial Scholarship: ($1,000) Graduating senior from Maurice J. McDonough HS with a minimum 3.0 GPA, Charles County resident. Michael S. Tayman Memorial Music Scholarship: ($1,000) Graduating senior from Maurice J. McDonough HS with a minimum 3.0 GPA, Charles County resident who plans on a major or minor in music. Michael S. Tayman Memorial Nursing Scholarship: ($1,000) Graduating senior from Maurice J. McDonough HS with a minimum 3.0 GPA, Charles County resident who plans to enter a nursing program. 4

Phillips Weaver Scholarship: ($1,000) Graduating senior from a Charles County public HS with a minimum 3.0 GPA who plans to attend The College of William and Mary or the University Of Maryland In College Park. Patricia Sugg Weiers Memorial Scholarship: ($250) Graduating senior from a Charles County public HS, Charles County resident, must major in Fine Arts, Performing Arts, Music, or Creative Writing. Zonta of Charles County Scholarship: ($1,000) Graduating female senior from a Charles County public high school with a 3.2 or lower GPA who demonstrates accomplishments in community service and leadership, has faced a significant roadblock or challenge and who plans to pursue a profession that will benefit women or children; Charles County resident. (Two awards: one for a student planning to attend CSM and one to be used at the college of the recipient s choice). Merit and Financial Need* Scholarships Remember: enclose a complete copy of the Student Aid Report (SAR) for the FAFSA if you apply for any of the following financial need scholarships*. Charles County Chamber of Commerce Scholarship: ($500) Graduating senior with a minimum 2.5 GPA, Charles County resident showing financial need* and planning to be a full-time business major at the College of Southern Maryland. Charles County Retired School Personnel Association Scholarship: ($1,000) Graduating senior from a Charles County public HS with a minimum 2.75 GPA, Charles County resident showing financial need*, must major in Teacher Education. (One scholarship is given at each Charles County public HS.) Ronald G. Cunningham Nursing Scholarship: ($500) Graduating senior from a Charles County public HS with a minimum 3.5 GPA, Charles County resident showing financial need* and planning to enroll in a college program to attain a BS in Nursing. Lisa Michele Duckett Achievement Award: ($1,000) Graduating senior from a Charles County public HS with a minimum 2.5 GPA, Charles County resident showing financial need*. Lizbeth Frazer-Fatig Helping Hand Scholarship: ($4,000) Previous graduate or a graduating senior from North Point HS with a 3.75 or lower GPA, who plans to attend college or an accredited vocational/technical school, Charles County resident showing financial need*, preference given to a student with an Individual Education Plan (IEP) or 504 Plan. John Howie Memorial Scholarship: ($500) Graduating senior from a Charles County public HS with a minimum 3.0 GPA, who played a high school sport all four years, Charles County resident showing financial need*. (Two awards one for a female and one for a male athlete). Stephen E. Mitchell Educational Scholarship: ($500) Graduating senior from a Charles County public HS with a minimum 3.0 GPA showing financial need*, Charles County resident planning to enroll in a math, science or engineering program. 5

Mudd Nurse Scholarship: (Full two years) Graduate of a Charles County public HS showing financial need*, born in and residing in Charles County, planning to practice nursing at Charles Regional Medical Center after graduation, major in nursing at the College of Southern Maryland. Thomas B.R. Mudd Teacher Scholarship: (Full two years) Graduate of a Charles County public HS showing financial need*, born in and residing in Charles County, planning to teach in Charles County after graduation, major in teacher education at the College of Southern Maryland. Kenneth Bernard Proctor Sr. Scholarship: ($500) Graduating senior from a Charles County public HS with a 3.0 or lower GPA showing financial need*, Charles County resident. Mildred Rice O Callaghan Scholarship: ($500) Graduating senior from Lackey HS with a minimum 3.0 GPA, showing financial need*, Charles County resident. (Two awards one scholarship for a student entering a vocational/technical school and one scholarship for a student entering a 2- or 4-year college.) Starkey Memorial Scholarship: ($500) Graduating senior from a Charles County public HS, Charles County resident showing financial need*, preference given to a student planning to attend West Virginia University. Robert Dean Stethem Thomas Stone Scholarship: ($1,000) Graduating senior from Thomas Stone HS showing financial need*, Charles County resident. Robert Dean Stethem National Memorial Scholarship: ($1,000) Graduating senior who is a U.S. citizen with a minimum 2.5 GPA showing financial need*. Tiny Hopes Scholarship: ($1,000) Graduating senior from a Charles County public HS showing financial need*, Charles County resident. (Three awards) Ashley Jayne Younger Memorial Scholarship: ($500) Graduating senior from Westlake High School with a minimum 3.0 weighted GPA, showing financial need* and planning to enroll in a business program.
