1 California Nursing Students Association California State University, Sacramento Chapter 7667 Folsom Boulevard, Sacramento, CA EQUIPMENT INFORMATION AND ORDER FORM Congratulations, and welcome to the School of Nursing! Sacramento State Nursing Students are required to have the following necessary uniform and equipment for clinical: Required equipment includes: stethoscope pair of bandage scissors hemostat (kelly forceps) blood pressure cuff penlight Required uniform is made up of: green scrub pants green scrub top white lab coat closed-toe and heel white or black shoes name badge EQUIPMENT ORDERING Attached is the order form to purchase name badges and other required equipment from CNSA. THE NAME BADGE MUST BE ORDERED FROM CNSA, purchase of other required items from CNSA is optional. Please fill out the order form and include a check (payable to CNSA) in the envelope with your order. Orders must be postmarked no later than Tuesday, July 19, Alternatively, order forms may be dropped off in person at the School of Nursing in Folsom Hall no later than Monday, July 25, 2016 by 12 pm (noon) (please deposit in lobby mail slot). Please make a copy of your order form for your records. Orders will be ready for pick up on the day of the mandatory New Student Orientation. CNSA is happy to provide a one-stop shop for your required clinical items. All proceeds from CNSA equipment and uniform sales go directly to support our school s nursing student activities. Please help us fund the events and opportunities for you in this upcoming semester through your support. CNSA members may participate in a special price offer on their order; please see the form for discounts. To receive a discount on your order, you will first need to join NSNA (membership in the national organization automatically provides membership in the state and local CNSA chapter). To join, visit the NSNA website ( and follow the application process. A one-year membership is only $40. Enclose proof of your membership with your order so that we may apply the appropriate discount (Photocopy of receipt for membership or photocopy of CNSA membership card). CNSA membership looks great on a nursing resume and will open many doors for you within our program! Other Sac State Nursing items (t-shirts, sweatshirts, etc.) will be available for purchase at orientation.
2 CNSA EQUIPMENT INVENTORY INFORMATION Required Equipment, must purchase through CNSA Name Tag Badge and Holder: Four different colored badge holders are included, color coded for each semester (Red for 1 st, Yellow for 2 nd, Green for 3 rd, Blue for 4 th ). Purchase also includes your name and photo on a Sac State School of Nursing ID Badge to use in clinical experiences. Photos will be taken at orientation. Must be purchased through CNSA. Required Equipment, offered to purchase through CNSA 3M Littmann Classic II S.E. Stethoscope: Has a dual-frequency, "tunable" diaphragm for listening to both low and high frequency sounds by alternating pressure on the chest piece. Firm pressure allows you to listen to high frequency sounds. Ease off to light "skin contact" and the diaphragm instantly switches to let you hear low frequency sounds (previously heard using the traditional bell). The bell side can be used for low frequency sounds and special procedures such as bruits, intercostal spaces and auscultation of smaller and bony patients. (Includes extra set of eartips and a name tag.) 3M Littmann Classic III Stethoscope (newest model): This is the newest version of the Littmann Classic II Stethoscope. Has a dual-frequency, "tunable" diaphragm for listening to both low and high frequency sounds by alternating pressure on the chest piece. It also has a second turnable diaphragm that is smaller and can be used for pediatric patients, smaller patients, or to pinpoint a particular sound of interest. This second turnable diaphragm includes a non-chill sleeve that allows it to be converted to a traditional open bell. The new tubing is more resistant to the constant cleaning in a clinical environment. (Includes extra set of eartips and a name tag.) 3M Littmann Master Classic II Stethoscope: Traditional bell and diaphragm modes are incorporated into the single-sided "Master" chest piece. To hear low frequency sounds typically heard with a traditional bell side of a chest piece, simply use light contact on the skin. To hear high frequency sounds typically heard with a diaphragm side, apply firm pressure. No longer will you need to remove the chest piece from the patient and mechanically switch from a bell side to diaphragm side. You can switch from low to high frequency response instantly without interruption. (Includes extra set of eartips and a name tag.) Classic II SE: Classic III: Master Classic II: NOTE: Stethoscope does not have to be purchased through CNSA but doing so will allow us to
3 fund Sac State Nursing events and opportunities for you to learn and participate in the community in order to grow and boost that resume! ONLY ONE STETHOSCOPE IS REQUIRED. Blood Pressure Cuff: Black enamel 300mmHg no-pin stop manometer Navy blue nylon cuff with range markings to facilitate selection of correct cuff size; artery label, and gauge holder Latex inflation bladder and bulb Chrome-plated deflation valve Complete with leatherette zippered carrying case and operating instructions. Bandage Scissors: 5.5 inch angled metal scissors with a blunt bottom tip for use in cutting bandages during clinical. Hemostat (Kelly Forceps): A metal clamp resembling scissors for use in clinical; 5.5 inch straight tip with locking clamp. Disposable Pen Light w/ pupil gauge: White plastic; to examine pupils, etc. during clinical.
4 Optional Equipment, offered to purchase through CNSA Nursing ProPack: (NOT mandatory equipment, but a nursing student favorite!) This is not your average fanny pack! It s very useful to put all your required clinical equipment in, (plus ample room for non-required items, such as a PDA) rather than in your pockets. It also comes with a strap to hang an always-handy roll of tape on. They are machine washable. Accessories not included, but most are available through CNSA. Nurse s Clipboard w/ built-in calculator: *New Item* (NOT mandatory equipment, but a nursing student favorite!) Folding Clipboard with helpful information back, including: Conversion charts, lab normal ranges, ruler, acid-base balance, IV information, sepsis/sirs criteria, Pain scale, cardiac monitoring, injection sites, etc. Holds up to 30 pieces of paper, and folds horizontally to fit in your pocket.

6 CPR Keychain Mouthpiece: (NOT mandatory equipment, but a nursing student favorite!) Inside contains a plastic barrier (as seen on the right) that allows you to perform mouth-to-mouth in the field, without swapping saliva! (actual size) Tumblers: (NOT required, but nursing student favorite!) reusable mug with a screw-on lid and straw, so you can show your Sac State Nursing pride while staying hydrated! CSUS Nursing School Patch: (NOT required, but a fun favorite!) Sew-on Sac State patch!
7 CSUS Nursing T-Shirt: (NOT required, but a fun favorite!) Limited stock in Adult unisex S- 3XL sizes. 65% Polyester and 35% Cotton, does not shrink! This shirt is a dark gray color, NOT black despite what the picture looks like. If you pay for a t-shirt after we run out of stock, a full cash refund will be granted during First Semester Orientation. Note: There will be an opportunity during the semester to order CSUS School of Nursing Sweatshirts and T-shirts. Stay tuned! Items will also be sold at orientation cash or check only.

Q: What am I required to purchase through CNSA? A: You are required to purchase an ID card and Badge Holder from CNSA. Q: When will photos for the ID card be taken? A: Photos will be taken at First Semester Orientation. Q: How will I receive the items I ordered? A: Orders will be ready for pick up at First Semester Orientation. Q: Who should I make the check out to? A: Please make checks payable to CNSA. Q: How do I join CNSA?
