1 Position Title: Social Worker Grade 1 Division: Community Services Reports To: Operations Grade 2 Social Worker: Administration Allied Health Manager Primary Objective: Direct Reports: Nil To provide the highest standard Social Work services to patients, clients, families and staff within as a member of the allied health interdisciplinary team. The Social Worker undertakes best practice interventions that ensure optimal treatment and care and efficient discharge planning. Position Dimension & Decision Making Authority: Key Communication Contacts: Without referral to manager Patient care. 2 Key Accountabilities Key Result Area Major Activities Performance Measures: 1. Quality Care Undertake psychosocial assessments; formulate and carry out Social Work interventions Provide recommendations and risk assessments to the treating teams within the expertise and domain of Social Work practice Client treatment and care goals achieved. Appropriate notifications and applications to authorised agencies are made. Treatment and care plans reflect the SW risk assessment. File audits identify adherence to legislative requirements and the undertaking of proper action where risk is identified Discharge Planning; Counselling that addresses the emotional and psychological well being of clients, in particular illness adjustment; Care coordination; Family Work; Liaison and referral; Advocacy; Guardianship and Administration and Advance Care Planning for patient and clients in the inpatient, rehabilitation and community settings. Facilitate case conferences and clinical discussions, ensuring that psychosocial factors are considered in treatment and care, rehabilitation and discharge planning. Social Work supervision Attendance and contribution to treatment and care planning meetings Feedback from clients Evaluation results documented. PJB audits reflect appropriate time utilization. Adherence to best practice guidelines Effectively participate and contribute to the function of multidisciplinary teams (acute, Rehab, and Community) All interventions will be purposeful and responsive, meeting the needs of clients and the wider organisation Provide resources and information to Colac Approved By: Director of Community Services Date of Issue: September 2010 Number: PD # AHSW1 Page 2 of 6
3 2. Responsibility and Leadership. Area Health staff To participate in the orientation of new Colac Area Health staff. Comply with all operational data entry and reporting; and departmental administrative duties Demonstrate flexibility and responsiveness within the scope of professional practice in order to meet the organisation s goals and priorities. Responsible for clinical area as agreed with discipline senior Task supervision for social work students To participate in the education of students from other disciplines. Identifies issues, considers solutions and presents these to manager. Participates in problem solving and decision making with more senior staff. Participates in specific and delineated projects and activities, which may include leadership in a particular project. 3. Information Management Client documentation will adhere to Medical Services documentation standards guidelines and comply with AH documentation standards. Statistics and data are recorded as per guidelines and within required timeframes. Documented guidelines adhered to. Works to the student supervisor Demonstration of problem solving approach. Specific projects identified and documented. Retrospective file audits reflect compliance with documented protocols. PJB audits reflect requirements are met. Organisational Quality Improvement Participate in organisational quality accreditation processes as required. Participation in continuous improvement activity and projects 6. Partnership and Community Liaison Relationships and communication with internal and external stakeholders is clear, purposeful and relevant 7. Professional Development Participate in professional development activities relevant to clinical area. 8. Infection Control Ensure all practice is conducted in accordance with infection control policies, procedures and standards 9. Code of Conduct and Equal Opportunity Develop and maintain sound knowledge of and commitment to CAH policies and procedures Ensure that you engage in behaviour that is at Demonstrated participation in accreditation processes Evaluate outcomes for clients in allocated caseload. Outcome measure evaluation results documented, and indicated changes implemented. Adherence to documented clinical guidelines which indicate clinical best practice. Person Specification: Qualifications, Experience and Personal Attributes Bachelor of Social Work or eligibility for membership of the Australian Association of Social Workers Previous experience in a health care setting Demonstrated Social Work knowledge and theoretical frameworks that underpin assessment and practice. Ability to work independently Desire to work in a rural/regional setting Excellent written and verbal communication skills Proficient in the core competences expected of a Grade 1 Social Worker Excellent IT skills in word processing and data entry Knowledge of relevant community resources and systems.
