1 Clinical Evaluation Criteria Clinical Nursing II NUR 1242L Student Name Semester Year Clinical Instructor(s) Rate the student on a 1-4 scale for each of the following items. 1 - Inadequate. does not meet expected standards 2 - Fair. meets expected standards inconsistently 3 - Competent......meets expected standards consistently 4 - Exemplary...meets & exceeds expected standards consistently Students receive a mid-term formative and final summative evaluation score. An overall final summative evaluation of competent (3) in ALL criteria is necessary for advancement in the nursing program. Criteria identified with an ** are critical safety issues and student must achieve at least a score of 3 in those criteria to progress in the program. Course Outcome 1 Perform nursing assessments and deliver safe, appropriate, and competent care for assigned clients in the acute care and community setting. Program Outcome 2 Coordinate and collaborate with the interdisciplinary healthcare team to assess patients and provide and direct nursing care that promotes optimal wellness of clients throughout the lifespan. Program Outcome 3 Demonstrate competent and safe implementation of clinical nursing skills in meeting clients psychosocial and physiological needs. 1.1 Gathers accurate objective and subjective assessment data. 1.2 ** Seeks and informs instructor of abnormal findings. 1.3 Develops an individualized plan of care for each client's unique response to alterations in health. 1.4 ** Implements a plan of care within the parameters of knowledge and experience. 1.5 Evaluates client's response to plan of care. 1.6 ** Recognizes safe and unsafe patient care conditions and reports safety hazards. 1.7 ** Demonstrates safe use of medical equipment and supplies. 1.8 ** Demonstrates the procedure for properly identifying patients. 1.9 ** Demonstrates knowledge of medical & surgical asepsis and procedures such as hand-washing and infection control techniques ** Implements appropriate Joint Commission Patient Safety Goals. 2 Course Outcome 2 Demonstrate clinical judgment, based on best practices, for making appropriate clinical decisions for 1 or 2 adult clients in the acute care setting. Program Outcome 5 Demonstrate effective clinical judgment and decision making utilizing critical thinking supported by scientific knowledge and evidence based practice. 2.1 Explores options before coming to conclusions. 2.2 Sets goals and priorities with instructors guidance to solve problems and prevent complications. 2.3 States appropriate nursing interventions with scientific rationales for each. 2.4 Determines priorities in giving nursing care. 2.5 Organizes patient care assignments utilizing time management skills with guidance from instructor. 2.6 Demonstrates knowledge of disease process of assigned patients. 2.7 Identifies patient's alterations in health status. 2.8 Develops a list of patient problems / nursing diagnoses by assessing client s response to actual and potential health problems. 2.9 Prioritizes patient needs/problems/care Individualizes patient care based on identified needs Determines patient care outcomes and evaluates care based on those goals Modify client care as indicated by the evaluation of outcomes Uses evidence-based information to support clinical decision making. Course Outcome 3 Implement a health promotion teaching plan for patient/family or appropriate community based group.
