Should you need further assistance at any time, please contact me directly at (615) ARRIVAL: Check-in at the Arizona Biltmore The Arizona Biltmore is located at 2400 East Missouri Avenue. Room check-in is conducted in the lobby of the hotel and starts at 4:00 p.m. Should you need to cancel your hotel reservations, you must do so 48 hours prior to your day of arrival. Late cancellation or failure to arrive on your scheduled arrival date will result in a charge of one night s room rate and tax. Check-in at Embassy Suites Biltmore The Embassy Suites Biltmore is located at 2630 East Camelback Road. Room check-in is conducted in the lobby of the hotel and starts at 3:00 p.m. Should you need to cancel your hotel reservations, you must do so 48 hours prior to your day of arrival. Conference Registration Registration will take place in the Frank Lloyd Wright Ballroom Foyer on Sunday, January 31 at 7:00 a.m. Upon onsite registration, you will receive your conference guide and an attendee list. Presentation materials for this conference will be provided in an electronic format. To view them during the conference, you will need a laptop, tablet or any device that has Internet capability. Be sure to bring your charger. There will be a charging station available at the Conference Registration Desk. Welcoming Reception On Saturday, January 30 from 5:30 p.m. to 7:00 p.m., you and your guest are invited to attend the welcoming cocktail reception, located on the Frank Lloyd Wright Lawn at the Arizona Biltmore. There will be a shuttle running every 30 minutes from the Embassy Suites to the Arizona Biltmore starting at 4:00 p.m. with the last pick up at 7:45 p.m. Golf Tournament The golf tournament, sponsored by Jefferies LLC and PricewaterhouseCoopers LLP, begins at 1:30 p.m. on Tuesday, February 2 at the Arizona Biltmore Golf Club. The tournament will be held on the Links Course with a shotgun start. Boxed lunches will be available after the closing general session to take with you to the golf course. You must be pre-registered with Bank Director to play in the golf tournament. If you have not registered for the golf tournament and would like to play, please contact Nicole Smith at nsmith@bankdirector.com to reserve your spot. Be sure to indicate your handicap and whether or not you need rental clubs and/or shoes. Final golf pairings will be posted onsite at the Conference Registration Desk on Monday afternoon, February 1. Golf clubs and shoes can be reserved for an additional fee; golf club rentals are $70 and shoe rentals are $15.

2 If you are registered for the golf tournament and know that you will not be able to play, please let us know at your earliest convenience so that we can fill your spot with another participant. Golf Bags Regardless of whether you are staying at the Arizona Biltmore or the Embassy Suites, the Arizona Biltmore bell stand will arrange for storage of your golf clubs throughout your stay. If you are pre-registered to play in the Golf Tournament, please indicate this to the attendants, and they will transport your bag to the course for you. DEPARTURE Agenda The conference sessions conclude at 11:50 a.m. on Tuesday, February 2. Boxed lunches will be available in the Frank Lloyd Wright Ballroom Foyer following the closing general session. There will be a closing cocktail reception in the Aztec Room & Patio from 6:00 p.m. to 7:00 p.m., after the conclusion of the golf tournament. All conference attendees are invited to attend. Rates remain the same regardless of the company, number of passengers and number of bags. The first mile is $5, and each additional mile is $2.30. Each hour of a traffic delay is $23. The minimum fare is $15. Each per trip airport surcharge is $1. SuperShuttle SuperShuttle is the company contracted to pick up passengers at the Sky Harbor International Airport. Vans operate 24 hours a day on a scheduled basis, with vans departing every 15 minutes from 9:00 a.m. to 9:00 p.m. Rates are zone based and range from $12 to $46 for the first passenger. Each additional passenger per reservation is $8 regardless of zone. Reservations are highly encouraged. If you wish to use this service, please contact SuperShuttle directly at (602) Rental Cars The Rental Car Center at the Sky Harbor International Airport accommodates all airport rental car companies, their rental counters and vehicles. If you are planning to rent a car, be sure to make a reservation. Vehicles are limited for walk-up customers. The Rental Car Center is located within its own building at 1805 E. Sky Harbor Circle South (between 16th Street and 24th Street, south of Buckeye Road). Below are a few of the car rental companies that operate at Sky Harbor International Airport. 4 CONFERENCE INFORMATION: Attire We encourage casual resort attire or business casual for all meetings and activities. The temperatures in January average in the mid-70s, but evenings can be cool, so please dress accordingly. Audio Video Recording Audio and/or video recordings of the presentations made at this conference are expressly forbidden unless advance written consent has been obtained from Bank Director and the presenter(s) to be recorded. Bank Director may conduct audio and/or video recordings during a session. Bank Director owns the rights to all such recordings. Business Center o Arizona Biltmore - The 24-hour Business Center is available for use to all hotel guests. Certificate of Attendance - To receive a certificate of attendance (COA), please fill out the appropriate form at the Conference Registration Desk before you leave the event. There will be forms provided at the Conference Registration Desk that you will need to fill out in order to obtain the appropriate credits. State CLE credits are not guaranteed and may vary by state. Many states have minimum deadlines for applications or credits may be denied, so requests must be submitted to us prior to the event. Sign-in forms must be turned in to the Conference Registration Desk at the end of the event. Requests received at or following the event for states not previously submitted for credit may not be processed. In that case, Bank Director can provide attendees with the necessary documentation to apply for credits on their own. 6 Guest Program Only registered guests may participate in the guest program. The $295 standard guest fee includes admittance to the lunch on Sunday and all four evening cocktail receptions at the Arizona Biltmore. All guests must have a name badge to be admitted to these events. The guest program does not include participation in Tuesday s golf tournament. Guests interested in playing with attendees in the golf tournament can sign up based on space availability and will be charged an additional $205. This fee includes Tuesday s boxed lunch, but it does not include rental clubs and other expenses. 7 Payment Payment for the conference must be made to Bank Director prior to the conference. Visa, MasterCard, Discover, American Express and checks are all accepted. Those who have not paid upon arrival to the conference will not be admitted to any activities without submitting payment at the Conference Registration Desk.
