4 VBEMS Operations OSCAR Online Scheduling System The OSCAR Online Scheduling System Duty Requests are due by the 10 th of the preceding month. Submit your Duty Requests on By Christi Budy Captain Are you documenting your procedures? Make sure that you are listing all of the procedures that you perform for your patient in the EMR. Choices include wound care, splinting, BVM, oral/nasal airways, LBB, suction, KED, time of extrication, CPR, intubation, IV and many, many more items. Remember that skills and procedures completed by engine crews should also be included here. The section where you can list things prior to arrival is for bystanders and police. A good rule of thumb is if the person is listed in the EMR (fire and EMS), the skill they perform should be listed in the detailed procedure section with the time, size, location, etc. SPA Pharmacy: ED Exit/Entry When heading to the pharmacy to swap out drug boxes, you must have a staff member swipe their badge to exit the ED. The good news is that entry back to the ED can be gained without key access. Simply wave your hand in front of the panel. By Christi Budy Captain Page 4 of 42

5 VBEMS Administration By Bill Kiley Deputy Chief Below is a training bulletin sent out to all Virginia Beach Police Officers. Please read and understand the limitation the Police have over their ability to detain individuals for treatment. TRAINING BULLETIN #12-05 April 23, 2012 Medical Treatment Authority Purpose This training bulletin is designed to provide guidance and attempt to eliminate confusion as to whether EMS can compel a person to seek treatment and what role, if any, an officer may have in that decision. This is a result of a few isolated incidents where EMS has requested a transport for a subject where the Police Department has no authority to do so. The following information should help to clarify those instances where it is legal for us to do so. In the event that you are on the scene of a case where EMS believes a person needs to be treated and/or transported to the hospital, and the person refuses, the police department has no authority to force the person to accept treatment or to be transported to the hospital. No officer may take a person into custody for purposes of medical treatment when there are no underlying criminal charges to justify a seizure of the person, or unless they are acting under the authority of an Emergency Custody Order issued by the court or a magistrate (see Code of Virginia below.) The process to obtain an Emergency Custody Order under such circumstances is outlined below under Code of Virginia Code of Virginia Citizens right to refuse rescue Provisions of this article are cumulative with existing law and shall not be construed to modify an individual s right to consent or refuse to consent to medical treatment if he/she is capable of making an informed decision, or to alter or limit authority that otherwise exists under the common law, statutes or regulations of the Commonwealth of a health care provider to provide health care;; or of a person s agent, guardian or other legally authorized representative to make decisions on behalf of the person incapable of making an informed decision. The provisions of this article shall not impair any existing rights and responsibilities which a health care provider, a patient, including a minor or incapacitated patients, or a Page 5 of 42

6 patient s family may have in regard to the providing, continuing, withholding or withdrawal of life-prolonging medical procedures under the common law or statutes of the Commonwealth. The below information is for Emergency Custody Orders only: : Emergency custody orders for adult persons who are incapable of making an informed decision as a result of physical injury or illness. A. Based upon the opinion of a licensed physician that an adult person is incapable of making an informed decision as a result of a physical injury or illness and that the medical standard of care indicates that testing, observation, and treatment are necessary to prevent imminent and irreversible harm, a magistrate may issue, for good cause shown, an emergency custody order for the adult person to be taken into custody and transported to a hospital emergency room for testing, observation, or treatment. B. Prior to issuance of an emergency custody order pursuant to this section, the magistrate shall ascertain that there is no legally authorized person available to give consent to necessary treatment for the adult person and that the adult person (i) is incapable of making an informed decision regarding obtaining necessary treatment, (ii) has refused transport to obtain such necessary treatment, (iii) has indicated an intention to resist such transport, and (iv) is unlikely to become capable of making an informed decision regarding obtaining necessary treatment within the time required for such decision. C. An opinion by the licensed physician that an adult person is incapable of making an informed decision as a result of physical injury or illness shall only be rendered after the licensed physician has communicated electronically or personally with the emergency medical services personnel on the scene and has attempted to communicate electronically or personally with the adult person to obtain information and medical data concerning the cause of the adult person's incapacity, has attempted to obtain consent from the adult person, and has failed to obtain consent. D. If there is a change in the person's condition, the emergency medical services personnel shall contact the licensed physician. If at any time the licensed physician determines that a person subject to an order under this subsection has become capable of making and communicating an informed decision, the physician shall rely on the person's decision on whether to consent to further observation, testing, or treatment. 7 Members, By Kevin Lipscomb Paramedic We often cite the huge capacity of our volunteer-based system to respond to demand surges. Consider, when the F-18 crashed into the apartment complex, we didn't have to turf a single call to mutual aid. Fortunately, it's pretty easy to muster back-up units when every local media outlet is reporting a major incident and there is a plume of smoke rising hundreds of feet into the air. It's more of a challenge when the need for backup creeps up insidiously. Years ago, members responded when the civil defense siren, or later their Motorola Minitor pager, went off. But it's clear that nowadays the way to reach you is through your cellphone. Enter OscarTRACK's new "OSCALERT" module. It's an opt-in/opt-out, individually adjustable subsystem integrated into OscarTRACK. It can send a text alert to your cellphone within *minutes* of certain conditions developing in the field. You can choose the general demand conditions that you want to be alerted to... Page 7 of 42

8 You can choose the ALS demand conditions you want to know about... And you can choose which special situations you are interested in... And it gets better! An OSCALERT looks something like this: OSCALERT: Trap or equiv, ROSEMONT RD S/DAM NECK RD. The text of the alert contains a web link ( leading to a mobile-oriented page where you can monitor the VB EMS Internet Scanner Feed to hear what's going on (if your mobile supports streaming audio in this way). Caveat: I've noticed that when you press the Play button on the audio page, it can take seconds before the audio starts playing. That's out of my hands, and I'll be looking for a better solution.) To get started, you must tell OscarTRACK two things: Your cellphone number and your cellular provider. You can do that by logging into OscarTRACK and clicking the "profile" link near the upper left corner: Page 8 of 42

9 Then scroll down on your profile page until you see this section: Complete that section as indicated and you're all set! I think I've worked out any serious bugs, but if something crops up, be assured I will fix it as quickly as possible. Please bear with me. 10 VBEMS Training ALS Student/Intern Note By Raj Islam Captain VBEMS, since its inception, has been a service that has continually evolved. This includes enhancing education and updated state regulations. With our history in mind we have continuously moved forward as a recognized premier entity in the EMS Community. As the end of the Spring Semester comes to a close, we will have a different setup for the future on how we will be giving Scholarships and Sponsorships through TCC. As we all have experienced in the last few years, a member could get released as an AIC/Driver and then immediately move forward to the next ALS Level. Also, a member could do strictly no ambulance duties for a period of 90 days while focusing strictly on field time as an intern. This past year, we received clarifications from TCC and VA OEMS regarding how EMS student s and interns can operate in the field. To ensure that every student and intern s needs are being met, ALL students and interns will have to do an application for each term (spring, summer, and fall). This will significantly increase how we will be able to allow expanded service ability to our students and interns. This is information that our ALS Coordinator PM Brenda Kim George has been advertising for the past 6 months. To clarify, a student is someone that is currently in a course to be certified. A student can ride with a non-als FTO, Intermediate or Paramedic, up to their level of student status for class until their student badge expires at the end of each semester. An intern is someone that is already certified and riding to become released. An intern can ride with an ALS FTO, Intermediate or Paramedic, up to their level of intern status for release. As with the students, the interns will have an expiration dates to their badges. Something new that will be coming up over the Summer is an ALS FTO update for all ALS FTO s that operate in the City of Virginia Beach. This will then become a yearly update to ensure that all ALS FTO s have the latest and greatest information that our Emergency Response System has to offer in a more consistent manner. Look for more information on this in the June Newsletter. I want to make a note of one last important thing. Remember our system is and always has been a teaching and learning system that depends on each and every member, experienced or brand new, to keep the evolution of education in VBEMS at its greatest potential. I sincerely thank you all for your contributions to our student and intern achievements! Any concerns or suggestions for improvement should be addressed through your chain of command and we will work together to obtain the best resolution as appropriate. Note: The current Fire Recruit Academy will begin their enhanced certification class with TCC during the month of June. Their classroom will be accelerated so they will be out for their student field clinical time approximately 2 after the start of their course. Their clinical field competencies are estimated to be complete by the beginning of August. After their testing is done with the state in the middle of August they will begin their intern time. It can be taken at any point during the internship interns are encouraged to take it early because you are not allowed to use equipment such as CPAP until you have taken the class.
