2 MENTORS(N) AND EDUCATORS(OT) This profile is a comprehensive document, detailing all the learning opportunities available in a specific clinical area. It is intended that students, mentors and educators select the appropriate opportunities to help the students to meet the specified competencies for particular placements. Students should not expect to access all of these opportunities during one placement. 2

4 AREA PROFILE CLINICAL AREA LOCATION TELEPHONE NUMBER STAFF LIST Keswick House St Nicholas Hospital External Internal The nursing team consists of: Ward manager Charge Nurse Staff Nurses Support workers. The extended multidisciplinary team includes: Consultant Psychiatrist Specialty Doctor Social Worker Pharmacist Technical Instructor Domestic Staff Clinical Nurse specialist Nurse Consultant Occupational Therapist WORKING HOURS PHILOSOPHY OF CARE Keswick House staff work a four on four off rota which slides over an eight week period. A rehabilitation ward providing a safe supportive environment, in which people can develop individual potential via a programme of rehabilitation at a pace appropriate to each individual using interventions which promote recovery. We aim to provide a service that works with people which is welcoming, accessible, responsible, safe and willing to listen and learn. We are committed to having staff with a range of skills that are 4
5 MODEL OF CARE approachable, empathic, courteous and able to develop positive relationships with service users founded on hope and optimism. We encourage a sociable and interactive environment where ordinary everyday communication and activities between service users and staff are encouraged. We acknowledge peoples strengths and seek to offer a variety of approaches to suit the whole person. We believe that people should be given choices that support them to achieve their goals. We value our links with the local community and our work in partnership with service users, carers and voluntary organisations. We use a bio-psychosocial approach to care. By using a range of assessment tools including Recovery Star that are evidence based. This allows all patients not just to have personalised care pertinent to their needs but also to have individual care pathways that allows their goals to be achieved, whilst also acknowledging that the patient is the expert in their illness. It ensures that the patient s needs are fully identified and allows intervention to be planned and implemented at its most effective point. By using this approach it ensures full collaboration of care this enables their aspirations, needs, wants and desires to be realistically achieved. The use of recovery based care focuses on the essential role of hope and optimism, promoting well being, social inclusion, accessing housing, employment, social contact and leisure activities which all contribute to an individual s quality of life and having a sense of control. Personalisation encourages choice and control. It is recognised that all service users are individuals with their own values, knowledge, experience, needs and 5
6 problems. It is our primary aim to provide holistic assessment, care, support and treatment in an environment conducive to meeting the needs of the service users. Working with people and establishing non-judgmental, non-discriminatory relationships encourages empowerment and mutual respect. ORGANISATION OF CARE Keswick House is part of stepped care services. It is one of the many services provided by NTW Foundation NHS Trust. The unit works alongside services provided by other Stepped Care wards within the Newcastle and North Tyneside locality such as the Grange, Willow View and Church View as well as community services such as Mental Health Concern, Mental Health Matters, etc Admission to Keswick House is via a referral system and referrals are accepted from a variety of sources We use the care-coordination system of care planning and delivery, which brings together health and social services, specialist mental health care. The Care Coordination policy (s) identifies how mental health services work in close collaboration so as to ensure that people of working age with mental health problems, their families and carers receive the right range of services in the least intrusive and simplest way possible at the right time. 6
7 PROFILE OF LEARNING OPPORTUNITIES Students have the opportunity to: Observe and participate in the assessment of patients and in the planning, implementation and evaluation of care from referral stage through to discharge. Liaise and work with other members of the multi-disciplinary team, e.g., Consultant Psychiatrist, Nurses, Occupational Therapist, General Practitioner, Behavioural Nurse Specialist, Lymphoedema Clinic, Physiotherapy, SALT, Dentist, Social Worker, practice Nurse, Haemophilia Clinic, Specialist Training Department, Epilepsy Specialist Nurse, Dietician, Accident and Emergency, etc Access statutory and voluntary organisations, e.g., Social Services Department s and Help the Aged. Observe, plan, participate in and facilitate group therapies (under supervision). Assist with all aspects of care planning and risk assessment. Perform as the nurse in charge / OT( under supervision) in regard to; the management of human resources, auditing and ordering various physical resources from outside agencies, liaise with outside agencies, carry out financial procedures, Access journal articles and text books relevant to their placement. 7
