3 Day 2 14 December 2017 Case Management Leadership Training: From Theory to Practice Facilitators: Prof N Malangu, National School of Public Health at MEDUNSA Aim and objectives of the workshop: This workshop is structured to consolidate the themes on Day 1 and provide participants with an opportunity to reflect and discuss on how to work collaboratively with the patient, other health care providers (doctors, pharmacists, ), family/significant others, and caregivers; how to encourage clients/patients to self-direct care, self-advocate, and make decisions when appropriate and to the degree possible; and how to deal with relevant and embedded ethical issues. This workshop would be useful for all health care providers including Case managers, nurses, medical doctors, pharmacists, social workers, physical therapists, rehabilitation counsellors, as well as interns. Outline: 1. Towards A Working Definition of Case management: Explore participants 'understanding of case management 2. Towards Collaborative Care in Case Management: Explain the concepts of collaborative care as opposed to the traditional view and its application in case management 3. Patient Centred-Care as the Core in Case Management: Describe the principles of PCC as applied in case management 4. Seven Essential Interventions in Case Management: Describe the seven essential interventions that require case management 5. Roles of Professional Case Manager: In line of the above, discuss the roles of a Case Manager 6. 4 Brief Professional Biographies of Presenters CAROL GARNER Managing Director- Global Case Management Solutions (GCMS) Chairperson: Case Manager Association of SA Carol s extensive healthcare career began in 1975 where she began her nursing career at Greys Hospital in Pietermaritzburg. During 1986 Carol joined CAMAF (Chartered Accountants Medical Aid Fund) where she embarked her journey in the medical schemes industry spanning the past 31 years. Carol is involved in both local and international healthcare markets with a broad client base across all sectors of the community with a specific focus on case management and risk control. In April 2015 Carol left the corporate world and opened her own independent case management company focusing on onsite case management and transition of care, ensuring the most appropriate outcome for patients. Carol is passionate about case management in SA and as the current chairperson of the Case Manager association is involved in the development and training of case managers with strong links to the USA, Australia and the UK. Carol has a strong personal faith and holds true to family values, she has been happily married for 40 years and has 4 married children and 7 grandchildren who keep her on her toes. She takes a keen interest in all forms of sport and is a Tottenham Hotspurs and Western Province fan. She is working hard on improving her golf handicap which is proving more of a challenge than she ever thought possible. Carol firmly believes that more can be achieved by building bridges and relationships and working together as a team and surrounds herself with likeminded people. She loves the slogan Just do it Erna Van Rooyen, Clinical Coding Expert, Spesnet Erna is currently employed at SpesNet (Pty) Ltd and has been since January 2015, where she is an integral part of the Health Code Index Division. She holds the position of Clinical Coding Expert, and is co-responsible for development of different coding products including the crosswalk between ICD10 and CPT4 to DBM-codes, and maintenance of coding, coding rules and guidelines for coding, within Health Code Index. She assist in the development of training material for both ICD-10 and CPT4, as well as Case Management within DUXAH. Prior to joining SpesNet she held the position of Group Case Manager at Netcare Head Office where she was responsible for all Case Management, ICD-10 and CPT4/CCSA training that occurred within the group and worked for Netcare between in various positions. Within Netcare she was actively involved with Medical Schemes on all matters relating to coding, case management, billing, alternative reimbursement systems and short payments. She served on the Netcare Medical Scheme s Board of Trustees for 6 years [ ] Erna originally qualified as a Registered Nurse [General, Community health, Psychiatry and Maternity] through 1 Military Hospital/UNISA in 1984 thereafter she specialised and worked in ICU: Neurosurgery, Cardiothoracic and Burns Units [ ]. Between she moved into Managed healthcare and started a career as a Case Manager at MSO. She completed a Post Graduate Certificate in Case Management at the University of Pretoria in 1999 and did Advance ICD 10 coding through Sue Eve-Jones [PACC-UK, in 2006]. About Your Workshop Facilitator- Prof Gustav Malangu Prof Malangu is a public health specialist with extensive experience in the African health care systems. He is a Professor and Head of Epidemiology and Biostatistics at the University of Limpopo where he teaches post-graduate students in the School of Public Health. He has many years of experience as international consultant and technical advisor. He is currently involved in the strengthening of pharmaceutical services and supply chain management of health commodities, including pharmaceuticals and other health products. He assist with business process re-engineering for public and private institutions involved in the scaling-up of treatments for HIV, TB, Malaria. He has provided technical assistance in the development and implementation of national policies to Ministries of Health of several nations. He works in projects for major international organisations of the United Nations system such as UNDP, Global Fund, UNFPA, as well as USAID under the PEPFAR project. He has worked and consulted in many countries including Angola, Botswana, Cote d Ivoire, Dem. Rep of Congo, Gambia, Guyana, Malawi, Rwanda, South Africa and others. Publications-He is also a co-author of a national reference guide on the management of the side effects of antiretroviral treatment (South Africa) and an author of a book on Antiretroviral treatment in Pretoria: Issues and Outcomes. Awards and recognitions- For his accomplishments in research, Malangu was promoted ad hominem to the rank of Associate Professor of Epidemiology in He is also the recipient of the Senate Award for Beginner Researcher in the Faculty of Medicine in 2002; the Best Upcoming Researcher in the School of Public Health in 2006; and the Overall Best Upcoming Researcher for the University of Limpopo He is a reviewer for several international scientific journals including the WHO Bulletin. In 2005, he was appointed as expert witness for National Prosecuting Authority of South Africa. He is an accredited assessor for the South African Pharmacy Council; and a WHO/TDR accredited trainer on publication of scientific journal articles. Pierre Matthee holds a Master s degree (Health) (Cum Laude) through the University of Pretoria. He has worked at several welfare agencies and physical rehabilitation hospitals as a social worker, and later as a case manager at Clayton House Sub-Acute Rehabilitation Hospital. He also operates a private practice, where his passion is to offer medical social work services to patients and their families, with a special interest in chronic diseases. Pierre is actively involved in initiatives that create awareness regarding chronic diseases and is an active member of the South African Arthritis Society. He also served a three-year term on the National Board for Case Managers in South Africa. Currently, he is preparing to start a research Phd on Transition of Care through the University of the Witwatersrand.
