2 2018 MAHC Conference Schedule About This Conference The Home Care industry has faced many formidable challenges this year including increased focus on complying with existing and new state and federal mandates and audits all on top of reduced reimbursement. Home care has managed to continue to provide critical home care services even in the most difficult circumstances, but what does the future hold? This year s theme speaks to the home care industry in terms of how we collaborate, share our vision, make a difference in the lives of those we serve and collectively transform the home care experience. Assessing Your Agency s Regulatory Compliance Through Survey Readiness The home health industry is complex, highly regulated and changes constantly. This session will provide a summary of the regulations in home health and ways that you can ensure your agency is meeting them. It will also provide an overview of the survey process and how you can perform a Mock Survey within your agency to minimize risk, alleviate deficiencies and achieve compliance before the surveyor arrives. Care Enough to Understand: Navigating the Hospice and Home Health Medicare Benefit Across the Continuum of Care In order to properly care for patients, all staff need to have a strong understanding of the Home Health and Hospice Medicare benefit, and how to best serve the patient across the continuum of care. Often the knowledge of the eligibility requirements and the pros and cons of each is left to a select few members of the team, which can leave patients and families without covered services and may place the agency in jeopardy of a complaint. All interdisciplinary team members should have a solid grasp of the Medicare benefits, from the covered and non-covered services to revocation, transfer and discharge requirements. Participants will leave this presentation with a strong understanding of how the two benefits work as well as the regulatory requirements needed to ensure compliance with the Medicare Conditions of Participation and Medicare billing requirements. Optimizing Operations Through Data - It Takes a Community of Data Now more than ever the health care landscape is focused on lowering the cost of care across the continuum and to do so is dependent on the data that providers are using to make decisions. Home Health and Hospice services are cost-effective care options that provide high quality care at a lower cost. Through claims data Home Health and Hospice organizations can validate their market position and gain insights into their contribution to patient Medicare spending. These data elements will define opportunities to collaborate with other providers in advancing higher quality care while leading the industry in lowering the overall cost in Medicare spending. Once hired, we will also review an effective onboarding (orientation) process and discuss the need for a good orientation to assist in retaining staff. Some potential interview questions will be provided as well as information on items needing completion prior to the start date for new employees. On-boarding will provide a sample orientation schedule that can be recreated and adjusted to meet each agency s need. There will be lecture, discussion and small group activities. Care Planning and Coordination - A Common Goal Care planning and coordination of care are two new Conditions of Participation that can work together to improve care delivery, quality outcomes and satisfaction for both patients and staff. This session will briefly review the CoP requirements, then focus on how agency staff can use care planning to identify patient goals, develop interventions to address patient needs and concerns, and establish a cooperative relationship between the agency and clients. Care coordination takes the plan from the 485 to the clinical field staff, patients and families. Take home resources to smooth the process and foster an agency approach that will benefit everyone. HHGM: Assessing the Potential Impact to the Caring Community The Home Health Groupings Model (HHGM) is a game-changer for the Medicare home health program that completely changes the basis of the prospective payment system and sets the tone for a new era in home health. This session will provide the latest information regarding possible HHGM implementation and strategies for successful operations under HHGM or similar payment model. M Aaron Little, CPA; Karen Vance, BKD, LLP, Springfield, MO Hospice Documentation Necessities This session will allow hospice staff members to obtain information in order to evaluate the documentation required for hospice charts to pass medical review under the Targeted Probe & Educate standards of review. From the standards for medical necessity documentation to the intricate details of the interdisciplinary group meetings, certificate of terminal illness, documenting continued qualification of patient condition and face to face documentation requirements will be addressed. Emergency Preparedness Table-Top to Go The Emergency Preparedness CoP s require that home health agencies conduct at least one table-top exercise annually. Missouri is at risk for tornadoes, so this makes a good example of a table-top for agencies. In this session, we will walk through a table-top exercise for a tornado with a scenario slide presentation, discussion handout and key points to focus on as you lead your agency staff through this exercise.
