This session will be exploring core competencies that all clinicians should possess, regardless of their discipline, to meet the needs of the 21st century health system: provide patient centered care; work in interdisciplinary teams; employ evidence based practice; apply quality improvement, and; utilize informatics. The session will continue with strategies for interdisciplinary collaboration and how they can result in better outcomes of patient centered care. 1. Describe the required standards of (c) and in the new CoPs 2. Review best practice strategies to incorporate into agency care planning 3. Outline a structure and process that operationalizes these standards Target Audience: Home health administrators, clinical managers, and clinicians Webinar 2 An Integrated, Living QAPI Program Tuesday, April 24, 2018 * 2:00 3:30 p.m. ET Webinar Overview: (c), and in the new CoPs can be operationalized within a structure and process that integrates these standards within an agency cost efficiently. This session will present strategies that objectify processes in practice and documentation to address these requirements. Care plan outcomes are targeted through an embedded QAPI program that loops back around with carefully constructed Plans of Action intended to influence targeted outcomes before they occur. This session will also outline the metrics to 1
2 monitor for evidence of successful outcomes and the documentation necessary for evidence of compliance with the new CoPs. 1. Describe the required standards of Quality Assessment and Performance Improvement (QAPI) 2. Outline a structure and process that operationalizes these standards in an integrated method 3. List metrics quantifying improvement in outcomes and lowered costs with this approach Target Audience: Home health administrators, clinical managers, and clinicians Webinar 3 Changes in Therapy Approach to Home Health Tuesday, May 1, 2018 * 2:00 3:30 p.m. ET Webinar Overview: CMS plans changes to the PPS case mix system to rely on patient characteristics to set payment for therapy and non therapy services and should no longer use the number of therapy visits as a payment factor. The practice patterns and therapy compensation creep associated with the current PPS methodology will fit poorly with a case mix system not relying on a therapy visit number. However, practice pattern changes do not occur as abruptly as payment changes. This session will review the proposed new Home Health Groupings Model and the impact on therapy practice. We will then apply reasonable and necessary plans of care to typical home health conditions and link them to the outcomes agencies strive to achieve. Participants will learn to prepare changes in practice and to demonstrate the value therapy services can bring to the home health agency. 1. Describe the changes in the Proposed Groupings Model for HH PPS 2. Review the impact on therapy from eliminating thresholds in the PPS case mix 3. Explain how reasonable and necessary therapy plans of care should directly relate to outcomes home health agencies need Target Audience: Clinical managers, Clinicians Webinar 4 The Value of Remote Patient Monitoring to Outcomes Tuesday, May 8, 2018 * 2:00 3:30 p.m. ET 2
3 Webinar Overview: CMS recently pledged more support for telehealth in upcoming payment rules. Longstanding advocates have complained about the lack of coverage for telehealth and the impact it has on the home health industry. The focus will be on how to leverage technology to improve access to care. The 2018 physician fee schedule included new billing codes for telehealth, including chronic care management and data analysis collected by remote monitoring. Regardless of specific coverage for remote patient monitoring, there is great value in the service for improved outcomes and reduced cost. This webinar provides insights into one remote patient monitoring call center. We will review the relationship of the call center with the agency, its nurses, the patients, and the value it brings to improved patient outcomes. Karen Vance will be joined by Karen Thomas from Advanced Telehealth for this webinar. 1. Explain the history of telehealth s role in home health to date and recent CMS announcements for coverage 2. Describe the difference between telehealth and remote patient monitoring 3. Define the relationships between the call center and the agency 4. Vance, OTR, Supervising Consultant BKD, LLP, Springfield, MO Karen is an occupational therapist with 30 years of home care experience as a provider and clinical manager. She specializes in clinical and general operations consulting services to home care and hospice agencies and helps providers establish and maintain quality management practices. Specifically, she provides care management training and implementation, OASIS accuracy and compliance assessments, operations and productivity assessments and other similar services. Registration Fee: HCAF Members: $150 per webinar or $540 for series; Non Members: $300 per webinar or $1080 for series. Registration fees are based on one phone/webinar connection per webinar offering; multiple site participation for your agency will require a separate registration fee for each connection. Sharing of registration or fees with other agencies and/or individuals is prohibited. Recording: If you are unable to participate in one or more of the live webinars of this series, recordings will be made available online through May 31, Registration Deadline: Those registering for the live presentation must do so at least three days prior. Registrations to access the recordings (see above) will be accepted through May 30. 3

This confirmation will include a link to access the presentation handouts, the sign in sheet, and evaluation, as well as a link enabling you to access the webinar. CEUs: 1.5 CEU's for SN, OT & ST per session. To apply for nursing contact hours, within one week following webinar participation, a completed sign in sheet and evaluations from each individual must be returned to HCAF. Certificates will then be issued by e mail. Cancellations: Fees will be refunded only if written cancellation is received one week prior to the workshop and no refunds after the dial in number is sent to your agency.
