1 JOB DESCRIPTION FORM TITLE: Eye Health Training and Support Coordinator RESPONSIBLE TO: Associate Professor Angus Turner Relationships Lions Outback Vision Liaise with TEAM MEMBERS - Optometry Ophthalmology Retinal Screening Students Coordination Liaise with Population Health Liaise with Member Services and Remote Clinics Liaise with other Pilbara Aboriginal Community Controlled Health Services Relations outside Lions Outback Vision Liaise with Kimberley Pilbara Medicare Local Liaise with WACHS Liaise with Clinic and Programme Coordinators Liaise with Government and non-government agencies and organisations when required DUTY STATEMENT This position is based in the Pilbara (location to be negotiated with the successful applicant) and employed through the Lions Outback Vision Unit of the Lions Eye Institute in Perth, but extends
2 across ACCHS, WACHS and KPML, liaising closely with the regional optometry program and visiting ophthalmology services. Partnerships, planning and management Collaborate with key stakeholders as above. KEY RESPONSIBILITIES Workforce development/training Assist sites to deliver routine retinal screening for patients with diabetes, to provide effective eye health education for patients and families, including education re: eye surgery procedures and preventative eye treatments such as laser therapy Work with the local primary health care team to identify local staff for training in the use of the retinal camera and coordinate the training and development of primary health care staff across the region in the effective use of the retinal camera When necessary be part of the data collection and analysis team for regional eye health data, in relation to retinal screening rates, outcomes of screening and timeless to appropriate the follow up of abnormal screening results Assist in identifying gaps in the delivery of eye health, assist clinics with the resolving of issues in eye health services and program delivery and coordinate the access of patients to retinal screening to minimise duplication Identify equipment needs, education resource needs and funding bodies to access these resources Maintain networks and contribute to planning forums, steering groups and consultancies in order to assist with local Aboriginal eye health program development and priority setting Work with other health care professionals to raise community awareness of the eye health program and health literacy of the community and other health professionals Enable a streamlined integrated patient journey across all providers by ensuring local pathways for referral and reporting of retinal photos are in place, that the outcomes of the photographs are known and understood by the local retina camera staff and that appropriate patient follow up is enacted Provide appropriate and relevant feedback to key stakeholders Work closely with the Health Worker Education Institutions to support the development of skills in retinal screening and eye health education during regional undergraduate health training. Health Promotion Assist with the planning, development and distribution of health promotion training resources for clients and the local and regional community in eye health.
3 Wherever possible, ensure service delivery is combined with opportunities for transferring information to local community service providers and community members Clinical services (including access to testing, treatment, support and aftercare) Provide ophthalmology clinic assistance when required at sites throughout the Pilbara Be acceptable and engaging with clients, their partners and families by providing non stigmatizing services When it is not possible to have local primary health care staff screening patients, provide the screening service in that area until an appropriate local staff member is identified and trained Support wide spread uptake, implementation and regular revision of the Pilbara Eye Health Protocols, including development of new protocols already identified as having a gap Work with services to apply best practice standards in the provision and use of the retinal camera Ensure care pathways are personalised according to risk, needs and choice Identify risk factors including social and environmental factors that make some clients more likely to experience poorer outcomes Health hardware Monitor the availability and accessibility of eye health hardware such as Retinal Camera Assist local services when required to coordinate any retinal camera repairs Data collection and monitoring Assisting services to identify gaps in eye health services and to source additional funding/resources where required Assisting with collection and collation of local and regional eye health data, for the purpose of local feedback and quality improvement. This includes regional mapping for gap analysis Where necessary, develop and support key staff in the use and application of information management systems to organise eye health, including the monitoring and maintenance of data quality Assist the collection and reporting of relevant data and information pertaining to the program for reports to funding bodies Evaluation Monitor the quality of retinal camera services being delivered to the region so that service related problems are picked up earlier and corrected
4 Provide appropriate and relevant feedback to key stake holders and local sites Assist local sites to develop strategies to improve services and ensure eye health services are accessible and acceptable for the needs of the site and the clientele. SELECTION CRITERIA Essential Allied Health qualifications Current C class WA driver s licence Willing and able to travel frequently bi light aircraft or 4WD to communities in the Pilbara region Good communication skills and able to gain patient s confidence Experience in delivering training to individuals in small groups High level of integrity in maintaining confidentiality Demonstrated ability to work independently, with minimum supervision, to achieve agreed duties Desirable Previous Eye health experience Previous experience operating a Retinal Camera and implementing Retinal Camera Photography clinics Sound understanding of Aboriginal culture and family groups Good computer skills, particularly in word processing and desktop publication
