1 University of Wisconsin-Madison, School of Nursing N419 Clinical Site Information - Fall 2014 Students are available all day to begin agency orientation and clinical hours on either Tuesday, September 2 or Thursday, September 4. See time and room assignment in the Agency Orientation column. ATTENTION: Students are expected to complete 120 hours of clinical time. In collaboration with preceptors, students will need to make arrangements to complete 120 hours by working longer days or by attending clinical sessions on alternate days if necessary. Public Health Madison and Dane County (PHMDC) provides public health services throughout Dane County, serving over 500,000 residents in urban, suburban, small town and rural settings. Our basic mission is to join with partners in the community to promote wellness, prevent disease and help ensure a healthy environment. PHMDC programs include: community health assessment, chronic disease prevention, health equity, injury prevention, communicable disease prevention and control, including immunizations, TB and STI-related services, perinatal services, and environmental health promotion and protection. Multiple opportunities include: immunization clinics, WIC, safety education, in-home prenatal care coordination, postpartum and parenting support visits. Students will learn how to have in-depth discussions with residents and family members to create patient-centered care plans according to the family and resident s wishes. Additionally, students will each have a case load of 2 families and learn about case management, perform patient and staff teaching/education, possibly do a quality improvement project and work with individuals experiencing memory deficits through new interventions, such as music therapy, to reduce the effects of sun downing. Opportunities will be available to learn about end of life care in a home setting, medications and equipment for terminal illness, expert symptom management, emotional and spiritual support, and bereavement assistance.

MMSD serves over 25,000 children, and nursing students may be assigned to elementary, middle, or high school sites. Opportunities include caring for children and youth with chronic and acute physical and mental illness, health promotion, individual and group education and counseling, and developing inter-professional skills. Students who choose this lab must have a car and be able to practice independently under a preceptor s guidance. Students must feel confident with their physical assessment skills and their knowledge of growth and development, and be open learning from students and families from different cultural and socioeconomic groups. Prior experience with children is desirable. UW student completing clinical in the fall semester will be involved with medication coordination and administration, obtaining medication orders from physicians, checking immunization records, vision and hearing screenings, diabetic management, allergy precautions and emergency treatment, health teaching for teachers and students, dental health sealant program and oral hygiene teaching, and working with K-4 and K-5 students for dental care. UW student will have the opportunity to attend Lakeland School, which is a school for children with special needs and Wisconsin School for the Deaf to observe.

Students will use their skills in first aid, illness management, health promotion, disease prevention, vision/hearing/lice screenings, and immunizations along with attending 504/Individual Education Planning meetings, and policy development. Bird St. Sun Prairie, WI The Sun Prairie High School will be the primary school for this lab. The district encompasses approximately 84 square miles of land. The district has one high school, two middle schools, and seven elementary schools, with an approximate enrollment of The District Nurse coordinates the health services of the district. NOTE: This site, with complete attendance, includes 20 additional hours of clinical; the 20 hours, when combined with the Community Assessment Project hours, fulfills the self-directed hours requirement (40 hours). The student will have the option to complete 20 additional hours at Sun Prairie School, or instead, choose to complete the 20 self-directed hours at an alternative site. 337 Ann Juzwik Verona Middle and Elementary Schools 8:00 am 3:30 pm 704 North Main Street Verona, WI This school district has over 4000 students in 1 high school, 2 middle schools, and 3 elementary schools. The student who chooses this lab will have experiences in the one middle school and two elementary schools and must be able to articulate and write clear, concise goals for his/her learning experiences. Walworth Ave, Delavan, WI The Wisconsin School for the Deaf is a residential school for approximately 105 students ages 3-21 from throughout WI. Nursing students will have the opportunity to learn about a variety of medical, behavioral, and social issues unique to the school-aged and/or deaf population; administer medication, perform treatments, teach school/dorm staff medication administration, emergency health plans, and about a variety of medical issues. Students will also teach the students about their medications, personal medical needs, how to prepare for their future needs, and nursing students will learn about deaf culture and American Sign Language (ASL). 342 Paul Jones Paul.jones@journeymhc.org Community Treatment Alternative- Mental Health Center of Dane County 8:00 am-4:00 pm 706 Williamson St, Suite 103 Madison, WI Community supported program for persons with severe and persistent mental illness. Client admissions are from Mendota Mental Health and the Dane County Jail. Client age seen in homes, apartments, group homes, and hospitals. These clients have been diverted from the criminal justice system. The program is based on the model for assertive community treatment. Students who are selfdirected and interested in mental health and/or forensic nursing will be most successful in the setting. Student must have driver s license and will need a second criminal background check paid for by the agency. Call Paul as soon as you are assigned and make arrangements for this with him. Students with an OWI or speeding violations of more than 17 miles over the speed limit within the past 3 years are prohibited from working at this agency. NOTE: The student will need to have a short interview with staff to ensure a good agency fit prior to beginning clinical. Brookline Drive 6:30am and finishing Madison, WI by 4:30pm depending on the preceptor s schedule Coventry Village of Wisconsin is a community based long term care option for elderly in Dane county. It includes both an assisted care residence, Sebring; and independent apartments, Brookline.
