2 Executive Summary The Lane County Safety Net Clinic Directory is an inventory of free and low-cost community health clinics in Lane County, Oregon. Safety Net Clinics are defined not by any specific legal terms, business model or scope of practice, but by their common mission: to provide health care services to uninsured, underinsured and vulnerable people in need. Examples include: Federally Qualified Health Centers or Community Health Centers, School-Based Health Centers, and community based nonprofits providing health services. The Safety Net Clinics offer a variety of services including: medical, dental, mental and behavioral health, health screenings, and social services. The Directory has two main purposes for the community: 1. To serve as a resource guide for community members seeking free and low-cost healthcare services; and 2. As a tool for community providers (clinical staff, case managers, social workers, health providers, etc.) to refer patients in need of safety net clinic services in Lane County. This document provides information on Safety Net Clinics in Lane County. Each clinic profile includes information on the clinic s location, hours, services and screenings provided, patient eligibility, payment practices, public transportation options, and languages services are provided in. The Lane County Safety Net Clinic Directory was developed in response to the Lane County Regional Community Health Improvement Plan (CHIP). This directory meets the goal of the CHIP Action Plan to map and promote the availability of healthcare screenings, services, and referrals in Lane County. The directory was developed in response to the following CHIP strategy: Support the implementation of evidence-based preventive screening and referral policies and services by physical, behavioral, and oral healthcare and social service providers. The information in the directory was collected through 211, websites, and confirmed via phone by each safety net clinic. Note: The information provided in this document is up-to-date as of March Clinic changes after this date are not included in the directory.
