District nurses treat and manage wounds, continence, stoma and leg ulcers and support patients through oncology and palliative events. They also monitor and administer IV medication to patients discharged early. 8 EASTERN BAY PRIMARY HEALTH CARE ALLIANCE Eastern Bay Primary Health Alliance (EBPHA) is a Primary Health Organisation (PHO) that is funded by Bay of Plenty District Health Board to support the provision of essential primary health care services through general practices to those people who are enrolled with EBPHA. EBPHA provides services either directly or through its provider members. These services should improve and maintain the health of the entire enrolled PHO population, as well as providing services in the community to restore people s health when they are unwell. The aim is to ensure GP services are better linked with other primary health services to ensure a seamless continuum of care, in particular to better manage long term conditions. 11 KAWERAU COMMUNITY PATROL GROUP The group patrols the town every night, acting as the eyes and ears for the police. Patrols check right round the town, including the CBD, clubrooms, parks and notified homes of people who are away. Patrols are not responsible for engaging with anyone who may be acting suspiciously, but to notify the police only. People who are going away can access nightly patrols of their home by notifying the Police. The group is affiliated to Community Patrols of New Zealand. Meetings are held at 7.30 p.m. on the 2 nd Wednesday of the month. All welcome. 16 RURAL EDUCATION ACTIVITES PROGRAMME (REAP) REAPs exist to provide lifelong learning support to their communities through multiple work streams, including early childhood, working with schools and adult and community education. REAPs work varies in each of the regions, and includes a very diverse range of activities, including working with all or some of the following government agencies... Ministry of Education, Tertiary Education Commission, Land Transport, Te Puni Kokiri, Accident Compensation Commission, Ministry of Social Development (including Strategies with Kids, Information for Parents - SKIP, Parents as First Teachers - PAFT, Family Start)... REAPs also work closely with local government, local trusts and community groups in their area. Much of the work in REAPs is of a developmental nature. REAPs are specialists in promoting and nurturing community change.
