3 Training Center Policies and Guidelines The following policies and guidelines are to ensure quality and consistency in the programs offered by Fire Service Training, LLC (DBA CPR Pensacola) (referred to as the Training Center/Site). All instructors are required to comply with these guidelines and policies, and any requests for deviation from these must be in writing prior to the event. Certification Card/Completion Certificate Security Policy Training Centers are expected to take precautions to preserve the value of American Safety and Health Institute (ASHI), American Heart Association (AHA), and Florida State Fire College (FSFC) certification, and to ensure that all programs continue to be widely accepted. In order for a course participant to be certified by an authorized Instructor, the participant must have met the required knowledge and skill objectives of the course. Certification for many programs is indicated by issuance of a certification card. Certification cards must not be issued, or in any other way made available, to course participants except by an authorized Instructor as a result of successful completion of a verified course. In order to prevent unauthorized possession of a certification card or certificate of completion, the Training Center/Site has implemented the following procedures. Certification Card Security Procedure To ensure proper security and handling of certification cards and certificates of completion, the following procedures will be implemented by all Training Center/Site staff. 1. All physical certification cards and certificates of completion will be stored inside the Training Site office in a locked file until issued to Instructors to take to class. 2. The Training Center/Site will maintain an updated inventory of all physical and electronic (ecard) cards and certificates in its possession, along with records of issuance or destruction of cards no longer in its possession. 3. Certification cards and certificates of completion issued to participants must be completed in ink by the Instructor prior to being issued. No student is to be given access to a certification card or certificate of completion with either the holder or the date fields blank. At no time shall a certification card or certificate of completion with blank fields leave the sight of the Instructor. 3

In circumstances where cards or certificates of completion are not issued in class, such as when insufficient cards or certificates of completion have been ordered by the Training Center/Site, or where issuance of cards or certificates of completion is contingent upon a contractual agreement between the Training Center/Site and its customer, Recognition of Participation documents may be issued to the students. 5. On completion of a course, Instructors will return all un-issued certification cards or certificates of completion to the Training Center/Site, along with rosters for the class within twenty-four (24) hours. Instructors should also retain copies of rosters for their own records. Examination Security Policy Training Centers/Sites are expected to take precautions to preserve the value of ASHI, AHA, or FSFC course certification, and to ensure that they continue to be widely accepted. In order for a course participant to be certified the participant must have met the required knowledge and skill objectives of the course. For some ASHI, AHA, and FSFC courses, knowledge is measured and reinforced by the student s successfully completing written examination(s). In order for these examinations to be valid instruments of measurement, students must not have access to them outside of the classroom environment. When written examinations are used, all staff will follow these procedures. Examination Security Procedure To ensure the integrity of course examinations, the following procedures will be implemented by all staff. 1. Hard copies of course examinations will be stored in a secure, locked location in the Training Center/Site. 2. Electronic copies of examinations shall be kept in an electronic location requiring a log in access to access the course examinations, and not stored in a readily accessible networked (intranet) location with general access. 3. Examinations will be duplicated only in the number needed for testing and will be numbered before being distributed in class. Instructors will check out each numbered hard copy and will check them back in after testing as applicable. 4

Except during testing, Instructors will secure hard copies of course examinations on their persons for the duration of the time that the examinations are not secured in the Training Center/Site. 5. Training Center/Site staff will destroy copies of any course examinations which are no longer to be used in class. 6. If a student refuses to relinquish a copy of the examination, that student is not to be issued a certificate of completion or certification card, and the incident must be reported to the Training Center/Site immediately. The Training Center/Site will follow up with the customer, and will report the loss of the examination to Health and Safety Institute (ASHI), AHA, or FSFC. 7. Any use or distribution of an ASHI, AHA, or FSFC course examination other than indicated above is grounds for dismissal and for dissolution of the Instructor affiliation with the Training Center/Site as well as reporting to the appropriate agencies. Quality Assurance and Improvement Policy It is the Training Center/Site s responsibility to maintain the goodwill associated with ASHI, AHA, and FSFC and their programs. The Training Center/Site s policy is continually to strive for high standards of quality in the administration and delivery of ASHI, AHA, and FSFC programs. Quality Assurance and Improvement Procedure As part of the Training Center/Site s efforts continually to improve the quality of its courses, the following procedures will be implemented by all Training Center/Site staff. 1. All courses will be conducted according to ASHI, AHA, and FSFC published standards. Normal class parameters with respect to class length, student-to-instructor ratio, student-to equipment ratio, classroom characteristics, audio-visual materials, etc., are defined in the ASHI, AHA, and FSFC Program Standards. When one of these variables exceeds the limits as defined by ASHI, AHA, or FSFC, the Training Center/Site policy is to adjust one or more of the other variables to account for this (for instance, if there are additional students and the Instructor number cannot be increased, the course length must be increased). In these instances, Instructors will confirm with the Training Center/Site the acceptable accommodations to be made for students. 5
6 2. Instructors will review class materials and check equipment no later than 24 hours before classes start to ensure that nothing is missing, inoperable, or out of date. If any piece of equipment needs to be replaced or repaired, Instructors will communicate with the Training Center/Site to ensure their readiness for the class. 3. Instructors will confirm contract classes with customers no later than 3 days before classes start to confirm course, class location, start time, minimum and maximum size, and other variables including if there are any students who require specific accommodations per the Americans with Disabilities Act. If the confirmation call identifies any issue, the Instructor will verify that the customer has the correct contact information for the Training Center/Site, and will request that the customer communicate with the Training Center/Site. The Instructor will also personally inform the Training Center/Site of the issue. The Training Center/Site will resolve the issue with the customer before the class date. 4. Course evaluations will be made available to every student at the end of every class. The Instructor will return the course evaluations to the Training Center/Site with the class rosters. Instructors are not to have students complete the evaluations prior to the end of the class. 5. The Training Center/Site will make a follow-up contact to the customer after each on-site class has been completed to ascertain that the class was conducted according to Training Center/Site standards. Among the questions asked will be whether the Instructor arrived at the time scheduled, whether the class length was within the normal parameters as set forth by HSI, AHA, or FSFC, whether the Instructor was appropriately dressed, whether the Instructor s hygiene was appropriate, whether the classroom was left in reasonable condition, and any other questions specific to the customer. 6. The Training Center/Site Director or Office Manager will review all student evaluations and results of follow-up contact, and after review, will make them available to the Instructor. If these documents indicate that an Instructor is in need of remediation or disciplinary action, the Training Center/Site will address this need, giving the Instructor the opportunity to make appropriate corrections or modifications, and escalating to termination of the Instructor-Training Center/Site relationship when these measures are unsuccessful. 7. Periodically, the Training Center/Site may arrange with a customer for a Training Center/Site Faculty member to evaluate a class. After the class is completed, the monitoring person will submit a teaching evaluation to the Training Center/Site Director for review with the Instructor who taught the class (each employed instructor will be monitored a minimum of annually) 6
7 8. The Training Center/Site will monitor its Instructors to ensure that they comply with all Training Center/Site, HSI, AHA, or FSFC requirements. When an Instructor fails to comply with a requirement, the Training Center/Site will institute appropriate corrective measures. If an Instructor consistently fails to comply with requirements, the Training Center/Site reserves the right to dissolve its affiliation with the Instructor. 9. The Training Center/Site will inform HSI, AHA, or FSFC of any instance of its terminating its relationship with an affiliated Instructor, and of its reasons for doing so. Equipment Maintenance and Decontamination Policy All equipment used to train Training Center/Site customers must be in good working condition and decontaminated appropriately after each use. Equipment Maintenance and Decontamination Procedure To ensure the proper working condition of teaching equipment, the following procedures will be implemented by all Training Center/Site staff. 1. Instructors will check their equipment no later than 24 hours prior to the start of class to ensure fully charged batteries, all equipment is working appropriately, and all equipment has been cleaned and stored appropriately. 2. If equipment needs maintenance or cleaning, Instructors will perform these tasks, or will swap out equipment with the Training Center/Site so as to be prepared for class. 3. After class, Instructors will decontaminate all equipment according to the manufacturer s specifications. 4. For equipment that is stored at the Training Center/Site, Training Center/Site staff will receive equipment from Instructors after class, and will check to ensure that all parts are present and functional. Prior to sending out equipment to the next class, the Training Center/Site will correct any deficiencies in equipment. 7
8 Professional Conduct Policy As representatives of ASHI, AHA, FSFC and of the Training Center/Site, all staff members are expected to maintain the highest levels of professionalism in their interaction with each other, with customers and with the public. Standards of conduct, dress, and hygiene must be maintained. Personal biases must not be identifiable from the actions or words of any employee. Behavior indicative of prejudice or in any way inappropriate for the workplace will not be tolerated. Professional Conduct Procedure All Training Center/Site staff members are expected to adhere to the following: 1. Staff must behave in a professional and polite manner at all times. Staff must be respectful of each other, all customers, and the public. 2. Problems arising with customers and the public must be addressed outside of the classroom and out of earshot of the students. 3. Telephone and communication is held to the same standard as face-to-face and written communication. 4. Disagreements between staff members shall be resolved out of the public eye. 5. Staff members interacting with the public are to be dressed in business-casual attire, with the exception of Instructors teaching in facilities where the customer has indicated that a different level of attire will be more suitable. Business-casual means: a) No jeans unless otherwise authorized (such as casual Monday for office staff.) Instructors are not authorized to wear jeans while conducting AHA course instruction. b) No athletic attire, including shoes, T-shirts, or ball caps. c) No visible underclothing. d) No bare midriffs. e) No flip-flops. f) No scrubs. g) No uniforms from other places of business or identifying other agencies. 8
9 h) No clothing advertising other businesses or products, especially bars, alcohol, musical bands, or cigarettes. i) Makeup and jewelry should be subtle and understated. j) Women s skirts or dresses must fall below the knee. k) In most cases, shorts are not appropriate. If shorts are worn, they must be neatly pressed, and fall below the knee. l) Polo shirts must be neatly pressed and tucked into trousers or skirts. m) Clothing should fit neither too loose nor too tight. n) Clothing should not be frayed or threadbare. 6. Clothing must be clean, free of stains, and neat. 7. During work hours, all employees will conform to common standards of personal hygiene (odor-free, hair neatly combed, teeth brushed, clean, appropriately shaved, etc.) Training Center/Site Communication Policy The Training Center/Site is committed to communicating effectively with its staff and its customers. Training Center/Site customers must be able to contact the Training Center/Site whenever they wish to schedule a class, ask a question, or provide feedback about an experience with the Training Center/Site or its staff. In addition, all customers receiving ASHI programs must be given contact information for HSI. Training Center/Site Communication Procedure To ensure appropriate communication, the following procedures will be implemented by all Training Center/Site staff. 1. Instructors will inform students in each ASHI class of ASHI s Rate Your Program tool, and how to access it on the HSI website. 2. Instructors will maintain a means of communication with the Training Center/Site. The Training Center/Site must be able to contact Instructors within one business day of any issue being brought to its attention. This can be via telephone, , or other appropriate communication medium. 3. The Training Center/Site will establish a regular method of communication with Instructor staff. Instructors must check the Training Center/Site s communication methods on a regular and frequent basis (i.e. website, newsletter, s, etc.). 9
10 4. Instructors will provide all students with the Training Center/Site s telephone number and the Training Center/Site Director s address. 5. All communications with the public, customers, and fellow employees will be kept confidential under most circumstances. Customer information such as name, address, telephone number, and electronic communication is considered secure and confidential, and will not be made available outside the Training Center/Site nor used for any purposes other than official communication regarding classes. Employee Payroll and Benefits Policy The Training Center/Site is dedicated to providing a payroll and benefits package that serves to recruit and retain high-quality employees. Currently the primary benefit provided is salary and required employment benefits (worker s compensation insurance, unemployment insurance.) Employees in an instructional capacity are also covered under an employer-provided professional liability policy to cover material presented during course instruction. Benefits Procedure 1. Payroll will be on a biweekly schedule. 2. For the purposes of defining a work week, this is the period beginning on Sunday and ending on the following Saturday. Hours worked during this period in excess of forty (40) for hourly employees will be paid at a rate of 1.5 times the established hourly rate for the given courses of instruction provided by the individual instructor. Instructors working under a per-class basis shall not be scheduled for more than 30 hours per week of scheduled class time. 3. Depending on specific duties performed and agreements with individual instructors, pay amounts can be either hourly or on a perclass basis. 4. The Training Center/Site may cover any Instructor Certification costs for active instructors employed by the Training Center/Site. For the purposes of this policy, an active instructor is defined as: a. Serving as the primary instructor for a given course at least six times each calendar year b. Serving as an adjunct/assistant instructor for twenty (20) or more courses in each calendar year c. Participation on at least a quarterly basis in the development or revision of course materials utilized by the Training Center/Site 10
11 d. Or other situations upon review of the Training Center/Site Manager/Owner and the Lead Instructor staff 5. Employees are provided with a waiver for course fees to attend courses offered by Fire Service Training, LLC during their term of employment. Independent Instructor Policy 1. Independent instructors are independent contractors who conduct the majority of their instruction independent of the Training Center/Site and occasionally assist as additional instructors for inhouse classes at the Training Center/Site. These instructors are not employees for Fair Labor Standards Act or other purposes. 2. Independent instructors assisting with in-house courses will be paid on a per-class basis by check within fourteen (14) days of the completion of the course. 3. Independent instructors will keep a current W9 on file, and will receive a Federal 1099 form at the conclusion of the fiscal year. 4. Independent instructor alignment can be terminated at any time by either party with any or no cause. Instructor and/or Student Materials Provided The Training Center/Site is dedicated to ensuring instructors have the most current instructor and/or student materials to fill positions in a variety of instructor capacities (BLS Instructor, ALS Instructor, Fire Service Instructor, etc.) In order to appropriately allocate resources and ensure instructional materials as needed, the following procedure is established. Instructor and/or Student Materials Procedures 1. Prior to attending an instructor course, the instructor candidate will notify the Training Center in writing (preferably ) of the need for instructor materials specific to the course they will be attending. Instructor candidates will be provided a copy of the instructor manual and student manual for the respective instructor course to utilize in their preparations and attendance at the instructor course. 2. Prior to instructing any course, instructors will be provided a copy of the instructor and student materials for the course to be delivered with adequate time to allow preparation and review before the class date. 11
12 3. All instructor and student materials provided to instructors will remain the property of the Training Center and must be returned upon completion of a course or termination of employment. Employee Scheduling Policy The Training Center is dedicated to providing a fair and equitable means of scheduling qualified employees to fill positions in a variety of capacities (BLS Instructor, ALS Instructor, Fire Service Instructor, etc.) In order to appropriately allocate resources and ensure instructional experience for all employees, the following procedure is established. Scheduling Procedures 1. Employee scheduling is done using the Homebase scheduling app, and will be provided to scheduled employees as far in advance as possible. It is the responsibility of each instructor to ensure they have updated availability and shift coverage. 2. Many classes are scheduled with short notice; therefore, all instructors are requested to maintain a current schedule of availability with the Homebase app for scheduling purposes. Instructors should check the app regularly for open instructor slots. 3. Scheduled classes are staffed with a minimum number of instructors based on current enrollment levels. Additional instructors needed will be given as much advance notice as possible; however, many classes have a large number of students register within a few days of the course date. It is the responsibility of the instructors to verify their upcoming shifts with the app on a regular basis. While the app does sync with your personal calendar, updates do not always post through. Verify within the app itself on a regular basis and within 24 hours of scheduled classes to ensure instructors scheduled for the class. 4. BLS-only level instructors will receive priority for BLS/Heartsaver classes to ensure they are meeting their minimum class numbers. ACLS/PALS level instructors will be scheduled based on their availability and number of required courses met. 12

15 Independent Instructor Independent Instructor positions entail creativity and the ability to follow standard BLS/ACLS/PALS curriculum. The instructor shall meet all American Heart Association (AHA) and/or American Safety and Health Institute (ASHI) requirements as an instructor and maintain current certification. Status: Independent contractor, fees based on a per-class basis. Programs Instructed: CPR/AED: Adult, Pediatric & Infant (Workplace/Community) Basic Life Support (BLS) Provider Basic First Aid: Adult, Pediatric & Infant Advanced Cardiac Life Support (ACLS) Pediatric Advanced Life Support (PALS) Qualifications/Requirements Obtain and maintain current BLS/ACLS/PALS instructor certification as required for courses instructed Strong verbal communication/teaching skills Responsibilities Instruction of BLS, Heartsaver, ACLS, and/or PALS programs to the community and healthcare professionals in an independent setting Answers student questions in a competent and effective manner Performs and rates student practical skills evaluations Administers and grades written examinations Ensures the reporting of grades (pass/fail) to the training site Ensures all course paperwork and instructor evaluations are returned to the training site Ensure all equipment is returned in the condition received Ability to lift and carry manikins up to 50 pounds. The instructor should have practical experience in performing the skills taught and the ability to communicate the skills to students in a clear and precise manner. Preferably, instructors should have at least 1 year of experience in teaching or training others, although this is not required. Status: Non-exempt; pay rate depending on qualifications and experience and is either a per-class or hourly rate. Position is part-time, as needed. Programs Instructed: CPR/AED: Adult, Pediatric & Infant (Workplace/Community) Basic Life Support (BLS) Provider Basic First Aid: Adult, Pediatric & Infant Wilderness First Aid Bloodborne Pathogens Child and Babysitter Safety Qualifications/Requirements Must be at least 18 years of Age Should have a professional certification (i.e. Firefighter I or II, First Responder, etc...) Should have actually performed CPR in an emergency setting Should be experienced in providing First Aid At least 1 year of demonstrated successful teaching experience, preferably in a health or safety related field. Obtain and maintain current BLS instructor certification Strong verbal communication/teaching skills Ability to work under a variety of conditions while working flexible hours Responsibilities Instruction of BLS, Heartsaver, and basic level ASHI programs to the community and healthcare professionals Answers student questions in a competent and effective manner Performs and rates student practical skills evaluations Administers and grades written examinations Ensures the reporting of grades (pass/fail) to the training site Ensures all course paperwork and instructor evaluations are returned to the training site Makes and/or implements applicable suggestions on how to improve the service 16

18 ACLS/PALS Instructor The role of an Advanced Cardiac Life Support/Pediatric Advanced Life Support (ACLS/PALS) Instructor is critical to both the community and health care providers. ACLS and/or PALS instruction allows students to undertake training that is directly transferable to the clinical setting. We ensure that our instructors know how to give effective presentations of ACLS and/or PALS information and materials; Candidates are expected to know how to function as a leader and participant in all components of an ACLS and/or PALS Provider course including (but not limited to): performing in the initial Megacode demonstration; group control skills; and knowledge and familiarity with all ACLS and/or PALS clinical and medical devices and equipment; setting up and using educational presentation tools; creating, presenting, and leading case-based scenarios, case discussion stations, skills instruction and skills evaluation stations, practice stations, remediation stations, and written examinations; and the educational philosophy of the ACLS and/or PALS Program. Status: Non-exempt; pay range depending on qualifications and experience and is either a per-class or hourly rate. Position is part-time, as needed. Programs Instructed: BLS Programs (as listed under BLS Instructor Position Description) Advanced First Aid Advanced Cardiac Life Support (ACLS) Pediatric Advanced Life Support (PALS) Qualifications/Requirements Must be at least 18 years of Age Must have a medical professional certification (i.e. EMT, Paramedic, M.A., LPN, RN, etc...) Minimum of 1-year experience providing or assisting with Advanced Life Support in an emergency setting At least 1 year of demonstrated successful teaching experience, preferably in a health or safety related field. Obtain and maintain current ACLS and/or PALS instructor certification Strong verbal communication/teaching skills Ability to work under a variety of conditions while working flexible hours Responsibilities Instruction of BLS, ACLS, and/or PALS programs to the community and healthcare professionals Answers student questions in a competent and effective manner Performs and rates student practical skills evaluations 18

20 Fire Service Instructor The role of the Fire Service Instructor is to follow assigned curriculum and instructional materials, facilitate recruitment and enrollment of qualified students, foster effective learning in both classroom and lab settings, and promote good public relations via contacts and regular communications with business, industry, and the public sector. Status: Non-exempt; pay rate depending on course, qualifications and experience and is either a per-class or hourly rate. Position is part-time, as needed. Programs Instructed: Various FFP Courses ICS Courses (if qualified) Hazardous Materials Training Courses (if qualified) Qualifications/Requirements Must be at least 18 years of Age Must have a current fire service professional certification (Firefighter I or II) At least 1 year of demonstrated successful teaching experience, preferably in fire service courses. Current Florida Fire Service Instructor I certification (Instructor II or III preferred) Current Florida Fire Service Professional certifications (Fire Officer I or II, Inspector I or II, Investigator I or II, etc.) Strong verbal communication/teaching skills Ability to work under a variety of conditions while working flexible hours Ensure maintenance of appropriate professional certifications while serving in this classification Additional Qualifications Instructors for Fire Officer I courses must hold current Florida Fire Officer I and Florida Fire Service Instructor II certification Instructors for Fire Officer II courses must hold current Florida Fire Officer II and Florida Fire Service Instructor II certification Instructors for Inspector courses must hold current Florida Fire Inspector I certification Instructors for Investigator courses must hold current Florida Fire Investigator certification Instructors for Hazardous Materials courses must hold current Florida Hazardous Materials Technician certification 20

22 Lead Instructor The role of the Lead Instructor position is to serve as the head instructor for courses of instruction in which multiple instructors may be used. This position provides oversight and guidance to instructors operating as adjuncts for a given course. Documented experience as the lead instructor for coursework requiring coordination of multiple instructors Strong verbal communication/teaching skills Basic business management experience Familiarity with various programs used (QuickBooks, Word, Excel, Scheduling Software, etc.) Ability to work under a variety of conditions while working flexible hours Additional Qualifications Must meet qualifications as a BLS Instructor (ACLS/PALS Instructor preferred but not required.) Responsibilities Answers student and adjunct instructor questions in a competent and effective manner Performs and rates student practical skills evaluations Ensures Instructor evaluations are completed and reviewed. Administers and grades written examinations Ensures the reporting of grades (pass/fail) to the training center Ensures all course paperwork and instructor evaluations are returned to the training center Complies grade reports for submission to ASHI, AHA, or FSFC as needed Makes and or implements applicable suggestions on how to improve course materials 22
23 Plan and coordinate active learning activities Ensure classroom is in order and all equipment is returned to its proper location Ability to lift and carry equipment up to 50 pounds without assistance. Be physically able to kneel, perform physically demanding tasks, stand for long periods, and possibly work long hours Ensure all courses are conducted to appropriate safety standards, using proper PPE for training conducted. Attend and/or lead instructor meetings and meet with members of staff as requested or needed Ensure course payments are documented, reviewed, and deposited in a timely fashion Assist with scheduling of instructors and other instructional staff as needed. 23
