3 1.0 Introduction 1.1. Central Washington University s Accident Prevention Program is contained on the CWU Environmental Health and Safety (EH&S) website. The chemical hygiene (laboratory) or hazard communication (non-laboratory) sections provide information on how to use chemicals safely. The information on personal protective equipment (PPE) includes conducting a hazard assessment for PPE and details what personal protective equipment to wear for particular tasks. 2.0 Accident Reporting, Investigation and Industrial Insurance / Workers Compensation 2.1 Accident Reporting Faculty, staff, or student employees who are injured or become ill on the job can suffer from physical pain, disruption of normal activities and frequently, partial loss of income. CWU can experience possible loss of productivity of the injured or ill person, interruption of departmental work and process, increased expense for temporary help and higher industrial insurance costs. To minimize these negative impacts, accident prevention is a major goal of the overall environmental health and safety program All accidents and occupational illnesses must be reported immediately to determine if first aid or professional medical assistance is required. Call 911 for emergency medical aid if needed. Supervisory personnel are to be notified immediately or as quickly as possible following an accidental injury or illness. Supervisors/Department Heads must investigate all accidents, injuries and illnesses occurring to employees for whom they are responsible. EH&S may assist with this process. The CWU Employee Accident Report form and reporting procedure are provided on the EH&S website. The form is to be completed by the employee and the supervisor and forwarded to EH&S and the Workers Compensation Coordinator in HR by end of shift. Visitors and students (non-employees) who sustain an injury or illness while on the campus must complete the CWU Student/Visitor Accident Report (found on the EH&S website) and forward it to EH&S as soon as possible. 2.2 Accident Investigation Investigation of work-related accidents or illnesses involving days of modified duty or lost time from the job will be given special attention by the EH&S staff. Work-related fatalities, hospitalizations, amputations or loss of an eye must be reported to the Department of Labor and Industries (L&I) by the EH&S office within 8 hours. The accident scene must be preserved as much as possible pending investigation by L&I Accident statistics are reviewed by the CWU Safety and Health Council, composed of administrators, faculty, staff, and a student representative. 1 P age

4 Departmental safety committees and supervisors may request departmental accident information from EH&S Accident investigations are to be completed in a timely fashion, generally within hours. If recommendations for corrective action cannot be implemented by individual departments or within departmental operating budgets, they should be submitted to the appropriate Vice President or Dean as soon as cost information is available. Implementation of corrective recommendations is dependent upon design requirements and institutional scheduling. 2.3 Industrial Insurance / Workers Compensation The University purchases workers' compensation/industrial insurance from the Washington State Department of Labor and Industries. Coverage is provided for all faculty, staff, and student employees who are injured or become ill while acting within the course and scope of their duties. Coverage begins automatically the first day of employment for all faculty, staff, and students who receive University paychecks. Each employee contributes a monthly amount for the medical aid portion of industrial insurance through payroll deduction. For further information on workers' compensation regulations and benefits, contact the Workers' Compensation Coordinator in Human Resources Volunteers are provided industrial insurance, medical benefits only, as required by Washington Industrial Insurance Code, RCW For additional information on volunteers contact the Payroll office. 3.0 Accidental Injury / Illness Records 3.1 Maintaining accurate injury and illness records is an important part of CWU s accident prevention efforts. Evaluating these records assists in determining where training may be needed and where additional protective equipment or procedures may be valuable. 3.2 The Occupational Safety and Health Administration (OSHA) 300 log for injuries and illnesses is kept current (within six days of an event). The Workers Compensation Coordinator is responsible for maintaining this log. The log for the previous year is posted during February of each year on safety bulletin boards found in most campus buildings. 3.3 The log is presented on an annual basis to the CWU Safety and Health Council and, as with accident reports, is available to departmental safety committees and supervisors for use in improving the safety and health of the campus community. 4.0 Safety Bulletin Boards 4.1 Safety information about employee rights under the Washington Industrial Safety and Health Act is provided on posters located on safety bulletin boards in most academic and administrative buildings. Updating of these posters is the responsibility of EH&S. 2 P age

5 4.2 Any person noticing a damaged or missing poster should notify the EH&S at X2252. Posters are available for departments wishing to provide additional copies by calling the above number. 5.0 Safety and Health Orientation 5.1 All employees of the University, whether temporary or permanent, are to receive a safety and health orientation when they begin work. This includes faculty, staff, student employees, rehires, and transferred individuals. Safety orientation information is provided in the EH&S website under New Employee Information. Additional information is available from the EH&S office. Topics covered include: How and when to report injuries Information about first aid How to report unsafe conditions Use and care of personal protective equipment Proper action to take in emergencies Identification of hazardous materials, instructions on safe use, and emergency action (references Hazard Communication and Chemical Hygiene programs) Description of Central s safety program 5.2 Supervisors/department heads are responsible for ensuring all new employees (faculty, staff, exempt professionals, and student workers) receive a proper safety orientation. The supervisor should review the Safety Orientation Checklist with the employee (form provided in EH&S website or from the Human Resources Office). Faculty in a teaching or supervisory role should perform this orientation for student workers. Supervisory staff may wish to review information specifically for them presented in the EH&S website in section Safety and Health Information for Supervisors. 6.0 First Aid 6.1 Each department must maintain first aid kits so they are accessible to faculty, staff, and students. The kits must be stocked and maintained in a serviceable condition. The kits must be stocked with materials appropriate to the location they serve. First aid kit information is available on the EH&S website. 6.2 Chairs and department heads are encouraged to allow individuals within their units to obtain certification in First Aid/CPR and to ensure their certificates are current. Training may be arranged by contacting the EH&S office. 6.3 Automated External Defibrillators (AEDs) have been placed in a number of campus buildings. These devices are a proven aid in the survival of those suffering from a sudden cardiac event. First Aid/CPR training routinely covers the use of AEDs as well. See the EH&S website for additional information. 3 P age

6 7.0 Self-Audit 7.1 Department heads, chairs, supervisors, or unit heads are encouraged to conduct a safety and health audit on an annual basis using a checklist provided by EH&S. Completed audits may be provided to departmental safety committees, if organized, and to the CWU Safety and Health Council by sending them to the EH&S office. 7.2 Because of the widely varying nature of department activities, the audit is a guideline only. Departments may delete or ignore any section that is not applicable. Assistance in performing audits and correcting any deficiencies identified is available from the EH&S office. 8.0 Safety and Health Evaluation 8.1 All supervisors, department heads, chairs, etc. are encouraged to include occupational safety and health issues during faculty or staff evaluations. Accidents, near misses, safety rule violations, short cuts, attitude toward safety and health, and knowledge of safe work practices should be included as evaluation factors. 9.0 Reporting Unsafe Conditions, Equipment, Practices, or Assignments 9.1 Faculty, staff and student employees are to report unsafe conditions, equipment, practices, or assignments to their supervisor, chair, or director. If this person is unable to resolve the issue, it should be reported to the EH&S office. The Safety Hazard/Incident Report form is available on the EH&S website Back Safety / Lifting 10.1 Back safety is important in accident prevention because back injuries are frequently painful and debilitating. A back safety checklist is provided on the EH&S website. Training in this important topic is available upon request Fire Safety 11.1 All employees are to understand how to respond in the event of a fire, including use/location of fire alarm pull stations and emergency evacuation procedures. Employees are encouraged to be familiar with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting. Employees are not expected to utilize a fire extinguisher unless confident about the use of the extinguisher, can safely attempt to extinguish the fire and have a safe exit way out of the building should the fire become uncontrolled. Contact EH&S for information and/or training Office Safety 12.1 An office safety and health checklist is provided in the EH&S website. Also provided are guidelines on proper ergonomic work station setup Tools, Equipment and Machinery 13.1 All tools, equipment and machinery are to meet basic safety and health requirements. Any item that does not meet minimum requirements is to be taken out of service (red-tagged) until repair or replacement can be effected. Loose or 4 P age

7 torn clothing is not to be worn when working around machinery. Jewelry is to be removed, and long hair confined Outside Contractors, Temporary Employees, Vendors, Volunteers, or Guests 14.1 Outside contractors working at the University are expected to comply with safety and health regulations by contract. Safety concerns that may arise are reported to the project manager in the Capital Planning & Projects Department. Temporary employees, vendors, volunteers and guests are required to be trained before working with chemicals, hazardous equipment or machinery. They are to comply with safety and health regulations that may affect their personal safety and health or the safety and health of others in the campus community Heat Stress 15.1 The Environmental Health and Safety office provides information and training on heat stress and heat-related illness for affected departments. This includes training for recognition of conditions which may cause concerns as well as symptoms of heat-related illnesses and appropriate first aid response Supervisors are responsible for monitoring weather and temperature-related conditions which may be hazardous for their personnel as well as providing measures for mitigating the hazard. 5 P age
