1 Practice Placement Learning Opportunities/Experiences (NURSING) DEPARTMENT OF NURSING AND MIDWIFERY Placement Code Location Code (Office Use only): Practice Placement Area: Renal Dialysis Unit (Where appropriate, include Ward/Dept, Hospital, Specialty, Locality, Care Unit) Please provide a brief description of your area: Nurse led 6 stationed satellite haemodialysis unit treating low acuity patients requiring renal replacement therapy. Suggested student preparation prior to practice: Able to name and discuss functions of kidney. Able to name and discuss causes of renal function. In order to register with the Nursing and Midwifery Council (NMC), students are required to achieve learning outcomes in the Common Foundation Programme (CFP) and Standards of Proficiency in the Branch Programmes (BP) in relation to the following four domains: 1 Professional & Ethical Practice 3 Care Management 2 Care Delivery 4 Personal & Professional Development (Standards of Proficiency for Pre-Registration Nursing Education NMC 2004). Professional and Ethical Practice Explain what opportunities, experiences and specific skills the student may encounter in relation to: Confidentiality: Information related to patient verbal, paper and electronic Medical notes Nursing notes Ethics: Information of options available to enable patient to make informed choice. Palliative options. Withdrawal of treatment. Transplant issues. End of life. Legal Issues: 2. Care Delivery Explain what opportunities and experiences the student may encounter in relation to: Communication: Between renal team/consultants/multidisciplinary team/ staff and patient / carers. Conveying information in a form that is appropriate to individual s level of understanding and situation. Written /oral / electronic. Maintain accurate records in line with professional standards of record keeping. Health Promotion/Education: Advising patient of dietary and fluid restrictions and dangers to health if compliance poor. Advising patients care of access ie fistula/graft/ permcath Discussing with patient results of monthly monitoring of biochemistry, haematology and changes to medication. Process of haemodialysis/ haemodiafiltration. Dialysis adequacy and tools used to monitor ie KT/V URR. Hepatitis Vaccination programme. 3 Specific Nursing Skills: Vital signs recording ie BP, pulse, temperature, respiratory rate, blood sugars. Patient assessment /Calculation of volume fluid to be removed. Fluid overload/isolate (UF) Ultrafiltration. Managing hypotensive episode. Care of vascular access. Intra dialysis monitoring machine recordings: BVS/ KT/V / Profiling. Lining and set up of dialysis machines. Obtaining blood samples from dialysis machines. Anaemia management. Care Management Explain what opportunities, experiences and specific skills the student may encounter in relation to: Risk Management: Infection control - cleanliness of unit environment. - cleaning of dialysis equipment. - isolation of patients with blood borne viruses/ MRSA. - care of patient vascular access. Patient acuity. Water treatment testing. Supply of consumables. Heparinisation. Inter-professional Working: Nursing staff local and NHS Highland Laboratory staff local and NHS Highland Dieticians Medical staff local and NHS Highland Infection control team Housekeeping Pharmacy Organisation of Care: Preparing dialysis equipment : lining and priming machine/ preparing dressing packs Patient assessment pre dialysis / vital signs/ weight/ blood glucose Monitoring vital signs /machine recordings during treatment Administration of oral / IV drugs during and post treatment : Anaemia management Preparing for termination of treatment Patient assessment post dialysis. Assessment of access before patient leaves unit.
