Description of the Service we provide and Client groups who may access it: Shackleton is a Dementia Assessment and challenging behaviour unit. We look after people who have an organic illness (mainly a Dementia) with associated challenging behaviour. Mostly patients will be over 60 years of age Occasionally patients suffering from an alternative organic brain injury may require admission to shackleton. These can include those suffering: Brain Injury due to a stroke/rta etc; Huntingtons Chorea People who have MS. Occasionally people under 60 will be admitted for assessment and/or treatment if Shackleton is felt appropriate and their illness decrees it necessary. Shackleton is a consultant led unit, but with the introduction of the Advice Line and Outreach Service referrals for input can often come from Gp s, families, homes etc. K:\Learning Environment Team\Intranet documents\practice Placement profiles\community Health\SHACKLETON\part d - student audit july 2011.doc Page 3 of 9

4 Patients do have to be seen by a consultant prior to admission, or their agreement to admission sought by the referring party. Via the Advice line and Outreach services we are able to offer verbal input and, when possible, to offer visits and on site assessments/management strategies in order to keep the person involved in more familiar surroundings. There are Inreach work stream meetings set up as part of the Dementia Strategy which include heads of nursing and residential homes and in-patient working streams which involves collaborative working with staff from wards at St Mary s and the Dementia Liaison Nurses. An Alzeimers café for relatives and others to attend serves to raise awareness, offer friendship, support and education for those wishing to attend. Other areas of interest and related experience available Key Learning Experiences: 5 Mentors (6 th mentor, experienced nurse doing the e-learning mentorship course and is a buddy ). Various forms of dementia Shackleton hold Teaching sessions in laymans terms. Communication skills, using therapeutic environment, learn a little bit about yourself. There can be verbal abuse and physical resistance. (Learn how not to take things personally, how to withdraw from a situation safely and effectively) Learn how to work with and treat someone who has Dementia as an individual. Using your environment effectively in order to provide optimum patient care. Meet physical care needs, to do daily obs (blood pressure, diabetes, hypertension). Wounds assessing wounds, using aseptic techniques, wounds dressing. Take an active part in development of our service (If you see something you don t like or if you know a different way of doing something that you think will work then let us know, you will be supported and we will look at change/ improvement.) Medications used in the elderly including side effects, alternate uses. In-house training available to students. Learn about the MHA, MCA, DOLs etc; Care planning the whole process. A shift with the nurse prescriber. Earlies :45, lates and nights (nights are optional, but encouraged). First shift is likely to be middle for induction, introduction and orientation. University of Southampton Mental Health Student uniform (purple polo shirts and trousers), proper foot wear, minimal jewellery, hair tied back (in uniform policy) 1 staff room, female cloakroom, male cloakroom. Staff toilet on the ward. Food to be brought to the site, there are facilities in the main kitchen, microwave, oven, hob, fridge. Staff stay on site throughout their shift, there is a park nearby where students are able to take their lunch. Swift and PAS are used. The mentor will have a computer. Encouraged to go to the library (Oliveira). There is a welcome folder, covering all relevant information for the placement. Students are welcome to any training sessions that are happening.
