In 2015, the CCHD achieved its goal and became the nation s 68th accredited health department. With our vision ever evolving, we have set our sights on a new goal: to be a leader in community partnerships to improve health outcomes. We will continue to not only Our Mission: deliver quality programs and services within the Christian County community, but take the reigns, when necessary, to ensure the community is making progress toward improved health. 4 Director s Note The fiscal year (FY2016) was one of the most successful years in the history of the Christian County Health Department (CCHD). After receiving National Accreditation status through the Public Health Accreditation Board (PHAB) in May 2015, the CCHD set its sights on a new vision: to be a Mark Pyle, MHA leader in community partnerships to Director improve health outcomes. This new vision encompasses the ten essential public health services and a commitment to the public health system....new vision encompasses the ten essential public health services and a commitment to the public health system.. The CCHD led the way in the establishment of the Christian County Health and Wellness Coalition, a group made up of 36 community leaders from 25 different sectors within the public health system. Thus far, the coalition has organized the development of the 2016 Community Health Assessment (CHA) and is working to finalize the Community Health Improvement Plan (CHIP). Upon completion, this coalition will meet on a continual basis to evaluate the progress of health improvement initiatives created within the plan. CCHD did see its share of changes during FY2016. Clinical services were down 21% seeing 1,035 fewer unduplicated patients than in FY2015. This decrease in patient volume is in direct correlation to the implementation of patients being assigned a primary care provider through KyNECT, Kentucky s Health Insurance Exchange, and Expanded Medicaid. Traditional sliding scale fee health department patients were assigned a primary care provider, and the CCHD s Christian County Connection Asset Mapping Tool was used to link patients to their assigned primary care provider within the public health system. In lieu of the decrease in services, CCHD clinical staff still provided 83,626 clinical services to 10,814 unduplicated patients. Committed to delivering core public health services, the CCHD has one of the most successful HANDS programs in Kentucky, and served 128 families in Christian County during FY2016. The CCHD health education department totally revamped the development and delivery system used for health education, and our health education staff reached over 21,000 participants through the use of evidence based programs designed for specific populations. The CCHD environmental department conducted 2,084 service activities including 786 restaurant inspections for 217 eating establishments in Christian County. Quality Improvement continues to be a primary focus for CCHD. Our quality improvement efforts have enhanced off-site flu clinic processes and have helped develop and expand programs such as Quick WIC. The CCHD has invested in a full-time Quality Improvement Coordinator position entrusting the oversight of quality improvement efforts, continuation of the accreditation process, performance management, and grant writing. The CCHD facilitated numerous events in the community this year. The 100 Mile Challenge saw more than 1000 participants walk their way to a healthier lifestyle, and the CCHD s first annual Colon Cancer 5K had over 100 participants. Our Low Cost Rabies Clinic administered almost 200 doses of the rabies vaccination providing one free shot to those with a valid Medicaid card courtesy of the CCHD. Our clinical staff conducted over 500 Humana Vitality biometric screenings encouraging individuals to take an interest and make choices to facilitate a healthier lifestyle. As always, CCHD practiced fiscal responsibility in the management of federal, state, and local tax dollars by achieving a balanced budget during FY2016. With the success of FY2016 combined with a commitment to the 10 Essential Public Health Services, Performance Management, continuous quality improvement processes, and a new found vision, the Christian County Health Department is looking forward to a bright FY2017.

8 Quality Improvement & Success Stories Quality Improvement Policy: Devin Brumfield Quality Improvement Coordinator Accreditation Coordinator Grant Writing CCHD has an inherent interest in improving the programs and services it provides to its stakeholders. CCHD evaluates its key programs, processes and services toward improving their effectiveness, efficiency, outputs, outcomes and customer satisfaction. CCHD desires to establish a culture of quality within the department; this culture is achieved through continuous quality improvement at all CCHD locations. What are we trying to accomplish? How will we know that a change is an improvement? What changes can we make that will result in improvement? Overview of Quality in the Agency: CCHD has established a Quality Improvement Committee to identify and monitor quality improvement projects. CCHD has also developed a training curriculum to train staff in basic quality improvement methods and tools. CCHD Continuous Quality Improvement (CQI) Teams are created to address specific areas of improvement within the agency and Christian County such as asset mapping, mass vaccinations, medical records destruction, needle exchange/syringe access, and many more.
9 The CCHD identified an opportunity to improve how mass vaccinations/strategic National Stockpile (SNS)/ Point of Dispensing (POD) exercises/flu clinics were conducted. A continuous quality improvement team was commissioned to review existing plans, processes and procedures and conducted a flu clinic, which also served as a mass vaccination/sns/pod exercise on October 15, 2015 with all CCHD staff members. The team, or a newly assigned commissioned team, will review/evaluate SNS and POD plans on an annual basis, identifying new opportunities for improvement and making revisions as needed. In July 2015, the CCHD WIC department set a goal of obtaining 15 weekly participants in the Quick WIC sessions, performed by the WIC Coordinator or a health professional in WIC. The goal of having Quick WIC sessions was to help the caseload of WIC in the clinic. During the first quarter of the 2016 fiscal year, Quick WIC participants had already surpassed the original goal by 125%. Due to steady participation increase in the Quick WIC sessions, the Quality Improvement Committee commissioned a new Continuous Quality Improvement team to plan and design a new WIC education classroom and relocate and design a new employee break room to the basement classroom. 12 CCHD in the Community Mile Challenge In September, the Christian County Health Department, in conjunction with Challenge House and Hopkinsville YMCA, sponsored the first annual 100 Mile Challenge in our community. For $10.00, each participant received a free 100 Mile Challenge wristband, pedometer, and t-shirt. Throughout the challenge, participants shared their successes on Facebook, encouraged each other to meet their goals, and together spurred the progression toward increased physical activity in our community. A finale event was held in November and participants gathered to celebrate their successes! Colon Cancer 5K In March, the CCHD hosted its first annual Colon Cancer 5K to promote the importance of early detection through colon cancer screenings. Over 100 participants joined us on race day for the 5K run.
13 Christian County Community Health Assessment and Improvement Plan The Christian County Health Department and the Christian County Health and Wellness Coalition have worked together to collect current health data and will soon publish the 2016 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). Ideas generated from the 2012 CHA and implemented in the CHIP have come to fruition and are now a working part of our community. Our community has successfully implemented a smoke-free ordinance, contributing to the improvement of smoking rates from 36% of the population to 27%. The Hopkinsville Transit System, established in 2014, has now added a third route improving accessibility to health care providers and other facilities in the area. The city has completed phase one of the Hopkinsville Greenway System providing a safe, cost-effective opportunity for increased physical activity and are avidly working toward the completion of phase two. The Christian County Health Department is committed to our vision as we continue to work closely with community members sharing the same passion in a combined effort to improve the quality of life among our citizens. It is our hope that the 2016 CHA and CHIP will continue to springboard community efforts and stimulate change in a healthy direction. Humana Vitality During FY , the CCHD clinic conducted over 500 Humana Vitality screenings among state employees. Individual BMI and blood glucose levels were measured and referrals were made when individuals depicted levels putting them at risk for diabetes, hypertension, etc Rabies Clinic According to state law, all cats and dogs must be vaccinated against rabies. In September of 2015, the CCHD partnered with local veterinarians to host its annual Low Cost Rabies Clinic and vaccinated over 196 dogs and cats. Shots were given to animals 4 months or older at a minimal cost of $5.00. One free shot was administered to those who had a valid Medicaid card, courtesy of the Christian County Health Department.
