2 Public Health Responsibilities Chisago County Public Health Adequate Infrastructure Prepare & Respond to Emergencies Assuring Health Services Preventing Spread of Communicable Diseases Environmental Hazards Promoting Healthy Behaviors Public Health works independently and in collaboration with community partners to protect the health of entire populations.

4 Community Health Assessment and Planning In 2016 Public Health Team reviewed the Community Health Improvement Plan (CHIP) and determined further development on strategies were needed. Further analysis of 2014 survey was completed to establish baseline data. Community Health Survey Distributed randomly in May Mobilizing for Action Through Planning Partnership (MAPP) committee reconvening this summer to complete assessments, prioritize, and establish strategies to revise and implement the Community Health Plan. A community-driven process leads to collective thinking and a sense of community ownership in initiatives, and, ultimately, may produce more innovative, effective, and sustainable solutions to complex problems.

5 Public Health County Health Rankings 2017 data shows further improvement placing us at 20 out of 87. This is a snapshot in time but we believe our population health is improving for three possible reasons: -More people gained access to healthcare insurance. -Public Health and community partners collaborated to evaluate community needs, analyze gaps and leveraged work. -Community Partners involved in, America s Best Communities, SHIP and other groups led community initiatives to drive policy and system changes in making our communities healthier Place in county health rankings

6 Statewide Health Improvement Partnership (SHIP) SHIP, through community partnerships, is designed to: Improve health by reducing risk factors that contribute to chronic disease Reduce health care costs Reduce obesity through Healthy Eating/Active Living (HEAL) Reduce population tobacco use Chisago County s Community Leadership Team (CLT), helps evaluate and determine community health needs. The CLT Team represents schools, businesses, housing owners/ managers, farmers, community groups, senior organizations, etc. The team met once in 2016 due to county staffing changes.

8 Maternal Child Health Program The program offers two types of home visiting. One is the traditional nurse visit, the other is an accredited, evidenced based model, Healthy Families of America (HFA). Member of Metro Alliance for Healthy Families, under a joint powers agreement. Serve children and families that have identified risks such as single parenthood, low income, childhood history of abuse, substance abuse, mental health issues, and/or domestic violence are more likely to experience developmental delays. Nurse visits focusing on child growth and development, parenting and pre/postnatal care to help reduce the incidence of childhood injury Number of MCH Home Visits Done

9 Child & Teen Checkups Outreach is to assure and promote that children on Medical Assistance (MA) receive comprehensive health care and well child checkups through their primary physician. Every family with a child on MA is eligible to enroll in the program. Program benefits -ongoing follow up regarding: healthcare benefits, appointment assistance, check-up components, county program eligibility and referrals. C&TC serves clients from birth to age Number of Children Receiving Service

10 Follow Along Program Any Chisago County resident with a child between the ages of 2 months and 36 months is eligible for participation. Provides screening of infants and toddlers using Ages and Stages Questionnaires. Screening helps PH nurses and clients identify developmental delays or concerns. Education materials and referrals are given when concerns are identified Number of Children Served

11 Women, Infants and Children (WIC) Provides Nutrition education and support to help promote a healthy lifestyle. Evidenced based program that significantly reduces the cost of medical assistant dollars for nutritional related conditions such as diabetes and low birth weight. 12 Disease Prevention and Control -Central Minnesota Information Connection (CMIC) / Regional Immunization Improvement Project. Chisago County was chosen as one of two Regional Coordinators for the CMIC project. Project Goals: Provide technical assistance to LPH Departments and Clinics for reporting immunizations in database (MIIC). To improve and document immunization rates. To improve immunization rates for the population in the five-county sub-region of CMIC. -Immunization Practice Improvement (IPI) Project The DP&C Coordinator (IPI Coordinator) conducts visits with the six Chisago County clinics to ensure compliance with the Minnesota Vaccines for Children (MnVFC) Program. This program provides free or low cost immunizations to all eligible children in MN. -Public Health Nuisance Project (In development) Statue 145A.04 subd. 1a(iv) Public Health has drafted a framework for Standard Operating Procedures for the handling of nuisance calls.

13 Public Health Preparedness Public Health threats are always present whether caused by natural, accidental or intentional means. The outbreaks of 2016 Zika, 2015 Avian and 2014 Ebola or recent natural disasters like flooding underscore the importance of having a prepared agency and community. Locally we have a active Chisago County Emergency Preparedness Advisory Committee with members from healthcare, law enforcement and other government sectors we completed 8 exercises in collaboration with partners and 13 drills. Currently we are meeting all 8 capabilities we are responsible for in preparing for a medical countermeasure response.

14 Additional Public Health Services 2016 Public Health contracted with Jail Administration to provide a FTE nurse position to provide health services in coordination with other professionals. This arrangement ended 1 st quarter of Car Seat Program Dental Varnishing Lead Screenings Early Childhood Screenings Teen Parenting Early Hearing Detection and Intervention
