7 Describe the implementation of a nurse managed clinic on a community college campus Discuss the opportunities and challenges with the model Explain the transition to an interprofessional clinic

8 Nurse Managed Center (NMC) (established November 2006) Nurse Managed Center Mobile Health Unit (MHU) (opened November 2008)

9 There are 250 Nurse-Managed Centers located throughout the US Most in inner cities Lewis & Clark is the first community college to have a Nurse Managed Center Began in November 2006 and initially funded by Health Resources and Services Administration (HRSA) Nurse Education Practice and Retention Program: Purpose is to establish and expand nursing practice arrangements in non-institutional settings to demonstrate methods to improve access to primary health care in medically underserved communities.

10 Additional Funding Fall Submitted continuance grant to HRSA July Received three year award totaling $882,000 from HRSA February 2010 Received Kresge Planning Grant to expand services in collaboration with a Federally Qualified Health Center

11 MetLife Innovation Award 2010 $50,000 Reapplied to HRSA under NEPQR: Nurse, Education, Practice, Quality, and Retention Program - Interprofessional Collborative Practice 2015 Received a three year grant for $1,420,000 from HRSA

12 To expand existing healthcare services to medically underserved clients by: Providing high quality health care services Educating people to take charge of their own health To provide a diverse, culturally rich clinical experience for nursing students

14 Primary Care Preventions Health Education

15 Primary care services: acute, episodic, and chronic condition Initially collaborated with existing health clinics to expand services in underserved areas beginning April 2007 Established the Lewis and Clark Campus Clinic, January 2008 Initially staffed 12 hours a week Cared for faculty, staff and students Expanded to 40 hours a week in August 2008 Opened to the Community September 2009
