Three groups of community stakeholders, including the CHNA group convened by TMH and the Leon County Health Department, collaborating with Florida State University School f Medicine s Center for Integrated Healthcare to analyze four data sets regarding behavioral health in this community and region. The results of that analysis will establish a baseline for current self-reported need for behavioral health services, access to those services, and experience of behavioral health issues including anxiety, depression, and psychosis. 2. These results establish a baseline for community functioning which may be measured at regular intervals to establish whether progress has been made through implementation of the strategic plan. 11
12 Describe facility s priorities in meeting these needs: The results of the neighborhood survey indicates an alarming prevalence of behavioral health issues. Our priority is to work with our community partners to determine the cause so that we can help lessen causal affects.
