3 Summary This report and grades represent our assessment of the quality of the areas of performance which were examined during this inspection. Grades for this care service may change after this inspection following other regulatory activity. Service users continue to tell us how much they enjoy attending the day care and how important it is to them. Staff presented as being committed to achieving positive outcomes for service users. The service has very good methods of consultation with service users and carers. The environment is maintained to a very good standard, offering service users a pleasant and comfortable lounge and attractive landscaped garden. What the service could do better The provider should develop assessment and personal planning information around peoples' social needs. page 3 of 30

4 The daily activity programme should be developed to offer an improved choice of daily activities and take account of the physical and cognitive abilities of individuals. The service should develop and improve the ways in which service users and carers' views are sought. What the service has done since the last inspection The provider has maintained the previous level of performance. Conclusion Creggan Bahn Day Care provides a care service offering a very good range of activities and stimulation for service users. Service users and their relatives continue to comment positively about the quality of the service provided. The manager, through discussion, demonstrated a commitment to developing participation methods with service users and aspects of care planning and activity scheduling. page 4 of 30

5 1 About the service we inspected Inspection report The Care Inspectorate regulates care service in Scotland. Prior to 1 April 2011, this function was carried out by the Care Commission. Information in relation to all care services in available on our web site at This service was previously registered with the Care Commission and transferred its registration to the Care Inspectorate on 1 April The service is registered to provide a support service for up to 20 older people per day. The service operates from a designated area of the providers care home, Creggan Bahn Court. Creggan Bahn Court is situated a short walk from Ayr promenade and less than a mile from the town centre. The stated aim of the service is: "to support our clients and help them remain as independent as possible in their own homes. We provide the opportunity to socialise and engage in a wide range of interesting and stimulating activities". "Creggan Bahn is striving to be an organisation where service users are at the heart of our work. This vision permeates throughout our work and in the actions of all our staff". Recommendations A recommendation is a statement that sets out actions that a care service provider should take to improve or develop the quality of the service, but where failure to do so would not directly result in enforcement. Recommendations are based on the National Care Standards, SSSC codes of practice and recognised good practice. These must also be outcomes-based and if the provider meets the recommendation this would improve outcomes for people receiving the service. Requirements page 5 of 30

6 A requirement is a statement which sets out what a care service must do to improve outcomes for people who use services and must be linked to a breach in the Public Services Reform (Scotland) Act 2010 (the "Act"), its regulations, or orders made under the Act, or a condition of registration. Requirements are enforceable in law. We make requirements where (a) there is evidence of poor outcomes for people using the service or (b) there is the potential for poor outcomes which would affect people's health, safety or welfare. 7 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection This inspection was carried out by one inspector on 28 October Feedback was provided to the service manager on 30 October During the inspection we gathered evidence from various sources, including the relevant sections of policies, procedures, records and other documents including; - evidence from the services' most recent self-assessment - care plans of people who use the service - service user reviews - minutes of service user and staff meetings - staff training and supervision records - staff recruitment records - accident, incident records - complaints procedure - the service's own quality surveys discussions with various people including; - the service manager, accounts manager, staff and people who use the service. Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. page 7 of 30

8 Details of what we found are in Section 3: The inspection Inspection Focus Areas (IFAs) In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. Where we have examined an inspection focus area we will clearly identify it under the relevant quality statement. Fire safety issues We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may consider what action to take. 9 The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We check to make sure this assessment is accurate. We received a fully completed self-assessment document from the service provider. We were satisfied with the way the service provider had completed this and with the relevant information they had given us for each of the headings that we grade them under. The service provider identified what they thought they did well, some areas for development and any changes they planned. Taking the views of people using the care service into account Service users spoke positively about the kind of activities and events they had enjoyed. They told us they were able to participate in quizzes, crosswords, sing songs, exercises and arts and crafts. They also told us about visiting entertainers and how much they enjoyed them. The comments made about staff, transport, quality of meals and activities continued to be highly complementary. We received 12 Care Standard questionnaires from service users. Nine of whom strongly agreed and three agreed that overall, they were happy with the quality of the service. The following additional comments were made. page 9 of 30

10 "Lovely day care, always feel welcome". "I am very happy with the service and staff here". "the minute staff collect me and return me home everything is perfect". "I am very happy here & the girls are very good and helpful & very nice meals and I also enjoy getting my hair done". "All my dietary needs are met due to my. All my meals are lovely and I can manage well. The staff are all very helpful and cater for all my needs. The grounds and the whole place are well kept. My family never worry when I am at Creggan Bahn". "I can only say regarding Creggan Bahn that we the clients who frequent this place each week, is that we always enjoy a very pleasant day. The staff could not be kinder or more caring and are all very helpful, caring and pleasant. It is a day to look forward to, long may it continue". "Able to travel by bus. The staff are all so kind and helpful. The meals are so good & the staff so helpful. If I was to stay anywhere I would choose Creggan Bahn". A staff member supported a service user to complete this questionnaire and commented: "[named individual] is very pleased on the way she is looked after at day care and she knows if she needed to speak with someone regarding a problem, she only needs to ask, and she is confident that the staff will always help. The meals are well presented and always finds them lovely and happy that the staff are always happy to cut up my food. The bus driver and carer are very good always take the time to help you on and out the bus, and are always nice". Taking carers' views into account There was no opportunity to speak with relatives/friends during the inspection. We received eight Care Standard questionnaires from relatives of service users. page 10 of 30

11 Six of whom strongly agreed and two agreed that overall, they were happy with the quality of the service. Additional Comments made were as follows: "Being able to attend day care has given my neighbour a new lease of life. She looks forward to getting ready to meet up with her friends and it is now the highlight of her week. The staff are so nice and the bus driver is a lovely man who never rushes her when she needs to catch her breath. She says the food is like a 5* hotel and the baking is as good as she used to make. Praise indeed!". "wonderful service, wonderful staff. Lovely building. Excellent!!". Inspection report "My wife loves going to day care. Don't know what I would do without it". "My mother has been attending Day Care for over 2 years. It is a great comfort to the family who do not live locally to know that she is no longer housebound. Her day at CB keeps her interested and up to date with daily news and in turn gives us plenty to discuss when we phone. We are always made most welcome by staff when we are through to visit and enjoy sitting in the garden for a cup of tea and delicious cakes". "Without this service it is likely that full time care in a nursing home would be required". page 11 of 30

12 3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 5 - Very Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service Strengths We found that the service was performing at a very good level. We concluded this from discussions with the manager, staff, and service users. Other sampled evidence included; - Personal plans - Menu consultation - Service user reviews - Completed quality surveys - Newsletter - Minutes of service user's meetings. Creggan Bahn had a participation strategy stating a commitment to seeking the views of service users on the quality of the services they provide and how service users will have a meaningful role in the continued development and improvement. We saw that service users were encouraged to express their views about the quality of the service during informal day-to-day discussions. More formal methods of consultation continued to be used. This included six monthly service page 12 of 30

13 user meetings. The minutes showed that service users discussed planned events, outings, activities, meals and developments to the service. An annual satisfaction survey was distributed to service users or where appropriate, their representative. This took account of specific National Care Standards. The manager planned to use this information to inform the selfassessment we ask them to complete. The service received 49 responses of which 33 rated the service as excellent, 14 as very good, 1 as good and 1 as adequate. The manager produced an action plan in response. The provider had conducted a menu survey. This was to be repeated in November The chef also attended meetings to gather feedback about the menu and quality of meals and snacks. This showed an ongoing commitment to proving meals and snacks in accordance with the preferences of the service users. The service had a number of scrap books showing photographs of service users enjoying activities, outings, parties, speakers and entertainers. There were separate evaluations of some of the activities, stating, if it was successful and enjoyable. This showed an ongoing commitment to proving activities in accordance with the interests' and preferences of the service users. We noted day care feedback forms in care files. They took account of the transport/driver, activities, meals, personal plan and invited an overall rating. This showed that the service valued the views of service users. The provider also conducted six monthly relative surveys to ensure ongoing satisfaction with the service provided. The service recognised the importance of communicating effectively with service users who had dementia or other difficulty that impacted on their ability to communicate. The provider had purchased the "talking mats" communication aid and this was being piloted. Information about how to make a complaint was available to service users in information leaflets and displayed on the notice board. Information was also available on how to access advocacy services. page 13 of 30

14 The service continued to publish a seasonal newsletter which provided information about events, activities and entertainment in both the day care and care home service. Information included changes to the environment, changes to the staff group, staff training and achievements. The service ensured that care reviews took place at a minimum of six monthly intervals. Service users and where appropriate, their relative/representative were invited to reviews by letter. Areas for improvement The provider should continue to build upon the current good practice in place. To do this, consideration should be given to independent facilitation of meetings and support to assist service users in completing surveys and evaluations. Service users and relatives should be more directly involved in completing the self-assessment we ask services to complete. The provider should consider a 'You said - We did' display to evidence to service users and relatives how they have responded to comments they have made. The provider should consider involving service users or relatives in staff development, training and recruitment. The service should also consider the role of service users or relatives in formulating satisfaction surveys, newsletter, meeting agenda etc. Grade 5 - Very Good Number of requirements - 0 Number of recommendations - 0 Inspection report page 14 of 30

15 Statement 3 We ensure that service users' health and wellbeing needs are met. Service Strengths We found that the service was performing to a very good level. We concluded this from discussions with the manager, staff and service user. Other sampled evidence included; - Personal plans - Service user reviews - Individual risk assessments - Activity records. Staff demonstrated, through discussion, that they knew service users very well. They knew their needs and preferences and had established close links with some families. There was a keyworker system in place. The key worker prepared personal plans and arranged formal reviews. Individual risk assessments had been carried out for each service user. This included falls risk assessment and moving and handling risk assessments. The service continued to have links with local health care services. Care records showed communication with services, such as, dietician and speech and language therapy. Care plans included information about dietary requirement and food preferences. Service users continued to tell us they were happy with the quality of the food. Dietary needs and comments about meals were shared with the chef and used to inform changes to the menu. On the day of the inspection service users were observed enjoying morning coffee and freshly made scones, a three course lunch and afternoon tea and a selection of home baking. page 15 of 30

16 The menu was displayed on the table and if service users wished an alternative staff arranged this with the chef. None of the service users attending the service at the time of this inspection required their medication to be administered and managed by staff. However, one individuals medication was held securely for them to self-administer. We could see from various records, photographs and in discussion with service users that the activity programme consisted of a range of activities. We saw that service users had influenced the programme of activities on offer. Some outings had taken place to a local garden centre, local restaurant, Girvan for a fish tea and Maidens for an ice cream. The manager had completed risk assessments for outings. The provider had arranged for an individual to visit the service on three days per week to facilitate an exercise class. We heard that this was very popular. On the remaining days a member of staff conducted this activity. This helped to promote peoples health and wellbeing. A record of each individuals' visit to the service was maintained. These contained good information about each persons' presentation and wellbeing and the activities they participated in. All of the service users we spoke with continued to tell us how much they enjoyed attending the service. They told us that they particularly enjoyed the opportunity to meet people and take part in different activities. It was evident that the service continued to achieve very good outcomes for service users. We observed very positive and warm interactions between service users and between service users and staff. Areas for improvement It is the service stated aim to provide the opportunity for service users to socialise and engage in a wide range of interesting and stimulating activities. Service users spoke positively of their time at day care and of the activities and outings they took part in. However, we thought that the service should offer a choice of activity and adopt a more person-centred approach. page 16 of 30

17 On the day of the inspection the activities were gentle exercises, quiz and crosswords led by one member of staff. All service users participated in the planned activities. No alternative activity was offered. We noted that attendees at day care varied in their ability to take part in some activities. For example, word finding can be difficult for some people with a diagnosis of dementia or a hearing impairment would limit an individuals' ability to participate in this type of activity. The service should develop the activity programme to make it more person-centred and to take account of peoples' varying levels of physical and cognitive ability on a daily basis. This may include group work in smaller groups or one-to-one time. Personal plans were based around peoples' health related problems. The service should consider enhancing existing paperwork with an individualised "pen picture". This would give information about each persons' interests, past hobbies, employment and the things and people who were important in their life. This information could be used to develop a person-centred activity plan which may include, identified goals agreed with the service user. These could be evaluated at an agreed time. We noted that there were some service users with memory impairment. The service should explore whether an individualised communication diary for day care staff, home care staff and relatives to use would facilitate communication of how the individual presented, their dietary intake and how they spent their time at day care. We spoke with staff who discussed some individuals who were nutritionally compromised. We noted that their care needs were recorded in their care plan. This aspect of care could be further developed by introducing, where required, a specific risk assessment tool designed for the purpose of monitoring an individual's nutritional status. 18 Quality Theme 2: Quality of Environment Grade awarded for this theme: 5 - Very Good Statement 3 The environment allows service users to have as positive a quality of life as possible. Service Strengths We found that the service was performing to a very good standard. Service users we spoke with and respondents who returned our Care Standard questionnaires spoke very highly of the standard of the accommodation. The day care centre is located within Creggan Bahn Care Home. The designated day care area was split into a lounge and dining space. The accommodation was very well presented with a range of comfortable seating, nicely set dining tables and toilet facilities nearby. Service users could access the well maintained and pleasant garden areas. The provider had supplied attractive garden furniture and we heard that service users enjoyed afternoon tea in the garden when the weather allowed. Service users told us that the vehicles used to transport them to and from the day care service were clean, warm and comfortable. 19 Statement 4 The accommodation we provide ensures that the privacy of service users is respected. Service Strengths The service was performing to a very good standard. We observed staff to be discreet and sensitive in their approach to care. The designated day care area consisted of one large room, however, staff informed us that that access to more private lounges/rooms throughout the care home building. They would not hesitate to use these if required. Named care records were securely stored to promote privacy. We found that toilet facilities were lockable to promote privacy and dignity. 20 Quality Theme 3: Quality of Staffing Grade awarded for this theme: 5 - Very Good Statement 2 We are confident that our staff have been recruited, and inducted, in a safe and robust manner to protect service users and staff. Service Strengths We found that very good recruitment practices were employed when recruiting new staff. Creggan Bahn Day Care is staffed by a small team of staff. There is a very low turnover of staff. We examined the recruitment records of the two members of staff recently recruited. They had completed application forms and attended for interview. The provider had obtained a PVG certificate and two references. We sampled one induction record. The provider had an induction checklist signed by the inductor. The inductee shadowed a member of experienced staff for a few days as part of the induction process. The probationary period also involved one-to-one time with the service manager. Areas for improvement We noted on one occasion that the references obtained did not include the previous employer. We were satisfied with the explanation offered. We heard that the provider had unsuccessfully tried to obtain a reference from this source. We discussed the need to retain evidence of this in the future. 22 Statement 3 We have a professional, trained and motivated workforce which operates to National Care Standards, legislation and best practice. Service Strengths The performance in this statement was very good. We concluded this from discussions with the manager, staff and service users Other sampled evidence included; - Training records - Supervision records - Staff meetings - Observation of practice. We found the staff team very motivated to securing positive outcomes for the service users. We observed kind and patient interactions. There was a very good rapport between staff and service users and the staff clearly knew the individuals in their care. Staff communicated very well with each other and demonstrated good team work when supporting service users. Staff had access to the company policies and procedures which included those related to training and development. Staff had accessed training in areas such as; moving and handling, fire safety, adult support and protection, food hygiene, infection control and dementia awareness. The staff team were qualified to at least SVQ level two in accordance with the requirements of the SSSC. Staff met each morning to plan the day ahead. Within the staff group individuals had specific areas of responsibility such as escorts, paperwork, facilitating activities. The manager was aware of the registration requirements of the SSSC. page 22 of 30

23 Staff team meetings took place. The minutes of these meetings contained relevant content and an action plan to progress matters discussed. Staff continued to receive regular supervision with their line manager. Records showed the content included, discussions in training and development and specific care issues and practice. We received five Care Standard questionnaires from staff. They all confirmed that they had induction training and that they did not have any outstanding training needs. They confirmed that they had a copy of the SSSC codes of conduct and considered that the service provided good care and support. Additional comments were made: Inspection report "Day Care is all about understanding the requirements of the service users in my care. Regular meetings are held to review and assess service users needs, having already attended a course on dementia I have gained the knowledge and support to help with the condition. A further course on dementia will be attended by myself in the near future". "Regular meetings are held to review procedures and discuss any matters relating to those under our care. Staff are always in the "know" as to what is expected of them and the gentle patience in dealing with all situations. Clients are continually mentioning their thanks for the care given to them". "I have always felt very welcome and supported in my role by the management and staff. I have attended all training relevant to my job role and is still ongoing" "All staff have made me feel welcome and as part of the team. All the clients have welcomed me and I have formed good positive professional relationship with my clients. I have had my induction and was made aware of policies and procedures and where they are kept". Areas for improvement The training plan should continue to take account of the aims and objectives of the service, needs of the current service users group and the training and page 23 of 30

24 development needs of individual staff. The promoting excellence dementia training programme should be completed by all staff. The provider should encourage all staff to complete reflective learning accounts of how any training has influenced their day-to-day work practice. 25 Quality Theme 4: Quality of Management and Leadership Grade awarded for this theme: 5 - Very Good Statement 3 To encourage good quality care, we promote leadership values throughout the workforce. Service Strengths We found that the service was performing to a very good level. There was a clear management structure. This included, a service manager, senior day care officer and day care workers. Each individual had specific areas of responsibility. We know that the service provider and manager were approachable. We heard that they were responsive to comments and suggestions. The manager was involved in a leadership programme called "My Home Life". The manager spoke positively about this. The manager also attended management events hosted by the local authority. Staff had the opportunity to meet on a one to one basis with their line manager. Areas for improvement The provider should continue to build upon the existing good standards in place. page 25 of 30

27 Statement 4 We use quality assurance systems and processes which involve service users, carers, staff and stakeholders to assess the quality of service we provide Service Strengths We concluded that the service was performing to a very good level. The strengths discussed in statement 1.1 also apply to this quality statement. There was evidence that the management monitored the standards within the care service. There were audits of care plans, accident and incidents. The local environmental health department had audited the kitchen area. The fire service also audit fire safety measures within the service. The provider distributed quality surveys to stakeholders who had an interest the care service. The provider/manager had a system to inform us and other bodies about notifiable events, such as, serious accidents or incidents that occurred in the service.
