2 About the service Thomson Court Care Home is located on the outskirts of Rothesay on the Isle of Bute. It is owned and managed by Argyll and Bute Council. The service provides care for up to eight older people including one respite place. Until recently, it also provided care to a younger, named adult with specific support needs. This arrangement has now ended. Each resident has their own room with en suite facilities. Residents have access to a spacious dining room, comfortable lounge and other shared spaces. The service's aim is; 'to provide residents/clients with a relaxed, amiable environment where staff are highly motivated and constantly endeavouring to ensure the highest possible quality of care while working to meet Argyll and Bute's policies and procedures and the National Care Standards.' It has been registered with the Care Inspectorate since 2011, having previously been registered with the Care Commission. What people told us We met with residents and their relatives who told us about their experiences of the service. These are some of the comments they made: 'XX has extremely complex needs and has been cared for through difficult times to a very hight standard over a period of years by a very caring and stable staff.' 'I am pleased that I have my own room with en suite bathroom and shower. My room is big enough to fit my furniture and my TV.' 'I don't leave the home worrying because I know XX is well cared for and the staff will deal with any problems. So flexible.' 'I am more relaxed because I know my relative is getting cared for. Personal care is to a very good standard.' Self assessment The Care Inspectorate has not asked services to provide self-assessments in the current year. 3 What the service does well The residents of Thomson Court Care Home are well looked after. They are encouraged to choose what clothes they want to wear on a daily basis, decide when they want to get up in the morning and what they might want to do throughout the day. This reflected the service's approach to supporting the residents as individuals with rights to make choices in their day to day life. Residents with personal care needs were supported by staff discreetly and with respect to their dignity. People looked forward to mealtimes when they were given choices of nutritious meals. These gave opportunities to socialise and ensured residents were eating healthily and with regard to any dietary or weight concerns. Relatives have confidence in the care given by staff and appreciate how they are kept informed by the service. This included in being involved in on-going discussions about the care provided, including reviews as well as being made aware of any concerns staff might have. We were told us that was invaluable in reducing some of the pressure they felt as carers and allowing them time to do other things. The staff know the residents well and are committed to providing care that meets their individual needs. The support plans we looked at assisted staff in this approach by providing good information about the individuals who stay in Thomson Court. This included the kind of support they required and their personal preferences regarding food and activities. Thomson Court provides support to some individuals who need palliative care. Relatives told us this was provided sensitively and in keeping with the wishes of the individual. Support was provided in conjunction with other services including family doctors and specialist nurses. The specialist nursing service had made very positive comments on the role staff from Thomson Court played in palliative care. Individuals were fully supported with their medication needs. This ensured they received the appropriate medication at the correct times. There were clear processes in place for this and senior care staff were confident and competent about their responsibilities in this area. Staff told us they feel well supported by management, receive supervision on a regular basis and have access to appropriate training. Nursery children visit on a regular basis and join in activities with residents. We saw this working very well with residents and children enjoying their time together. What the service could do better Monitoring of residents' health is essential to ensure their continued wellbeing. When we looked at the processes used to monitor residents' health and wellbeing, we thought these were not clear. This could lead to confusion on important matters regarding the individual. See Recommendation 1. We observed handovers between senior care workers at the end/beginning of shifts being subjected to constant interruption. These handovers play an important part in ensuring staff are aware of any developments or concerns residents that might affect the care they provide. Important information could be overlooked or misunderstood if they are not done well. The management should consider how best to protect this important time. page 3 of 7

4 With the exception of the visit from the nursery school children, we saw very little stimulation of some residents between mealtimes. Management should consider how best to provide regular activities for residents based on their interests and abilities. Monitoring charts should only be used when appropriate for the individual resident.
