2 About the service Buchanan Lodge Care Home is registered to provide care to 38 older people, of whom 19 require nursing care. This registered service is provided by Brighterkind Care. The care home is set within its own grounds in a quiet residential area in Bearsden, Glasgow. The bedrooms are contained over two floors, on ground floor level and first floor level. There is a lift to the first floor. The philosophy of care, as stated within the care home information included: 'At Buchanan Lodge, everyone is treated with respect - residents and staff alike.' What people told us The views of people using the service and relatives have been included in this report. Prior to the inspection we issued 15 Care Inspectorate questionnaires to people using the service of which one was returned completed. The person agreed that they were happy with the quality of care they received. We issued 15 Care Inspectorate questionnaires to relatives and received two completed questionnaires. Both agreed or strongly agreed that they were happy with the quality of care their relative or friend was receiving. During the inspection we spoke to a number of people living in the home. Some of their comments included: "Happy with the quality of care during the day". "Not enough staff at night". "All very good". "My... is cared for in an exemplary fashion by an incredibly hard working and kind staff". "Sometimes it seems that there are not enough staff to attend to the social needs of people". "The place is clean". "The staff are great but pushed at times". "The manager is very approachable". "There is plenty food and a choice of snacks". "I have made friends in here". "It is the next best thing to living in my own home". 3 What the service does well The home had a welcoming atmosphere with public areas recently refurbished. Carpets had been replaced in the foyer and sitting areas to refresh the environment. The conservatory was being decorated and new furniture was awaited for the downstairs lounge. People had personalised bedrooms and told us they were content and well looked after. Newspapers and magazines were available to suit individual preferences. Staff were kind and respectful when working with residents. We noted that staff knew people well. For example, we saw people were supported to have a good fluid and nutrition intake throughout the day. We also saw staff respond sensitively to people who were unsettled. We observed staff following safe infection control procedures and moving and handling practice. These things help to create a safe environment for people to live in. The service was developing memory boxes and a programme called 'Wishing Well'. These things told us that the staff had worked to put their training into practice to provide personalised care and support to people with memory impairment. We saw good community involvement and a variety of social activities. Some community groups came into the home regularly with opportunities for people to participate in activities outwith the home. During the inspection, some people were being supported out to a local coffee shop. We reviewed a range of documentation and were assured that the service met legal requirements in relation to maintaining records. Care plans had been regularly updated with the involvement of families. Risk assessments including falls, moving and handling and restraint were in place in accordance with best practice. We saw that staff responded to changes in peoples health and well being by contacting appropriate professionals such as the GP, district nurse or dietician. Working in partnership with other professionals promotes good quality care. Staff appeared well motivated and told us they valued regular training opportunities. The recent change to more face to face training was welcomed by staff. There was a new system of staff supervision and support in place and we will review this at the next inspection. What the service could do better Greater attention to detail with completion of documentation could have improved the overall standard of record keeping as required by the professional bodies staff are registered with. The detail contained within some assessment documents such as for people living with 'stress and distress' could have been more specific and detailed. This would have provided clearer information for staff to follow. We recognised that a range of new documentation had been put in place and that staff needed time to become accustomed to these new documents. We will review this at the next inspection. The service had a development plan to assist in prioritising issues for the future. We discussed the need for ongoing refurbishment to refresh bedrooms and public areas to maintain a safe and fresh place for people to live in. While we thought the entrance area was pleasant with freshly planted tubs of flowers and raised flower beds, the area is also used for people who smoke. That resulted in a less pleasant entrance with cigarette ends visible and an environment that was not entirely smoke free. We will review this at the next inspection. page 3 of 7

4 Some relatives felt that the time taken for staff to answer buzzers was sometimes too long. During the inspection we did not observe people waiting for a long time for attention, however we brought this to the attention of the manager. We were confident that staffing levels were in accordance with the assessed need of people. The mealtime experience would benefit from review to ensure the experience remains calm and pleasant for everyone. We discussed with the manager how review of staff deployment at mealtimes could assist with this. To help people remain orientated and find their way around the home, directional signage could be clearer within parts of the home. This would be helpful for people who have memory loss or sensory impairment.
