Training sessions will include: What HR related data will your organization need and how to provide that to your leadership; Recruiting and Retention of all staff; Developing your Workforce Strategy; and Question and Answer Session. 9:00am 3:00pm Boot Camp 2: Dental Boot Camp Dr. Samuel Parish The opioid epidemic is a growing problem across the U.S. affecting individuals, families, businesses, and government at the local, state, and national level. Substance Use Disorders (SUD) present in all fields of healthcare although Primary Care and Oral Health providers are uniquely situated to screen and intervene in patients affected by SUD. In this workshop we will review the origins and status of drug use and addiction. The clinical manifestations of SUD as it relates to Oral Health will be reviewed. Tools for screening will be discussed to provide a standardized approach to identify patients at risk for SUD. Pain management issues related to oral health conditions will be reviewed. The value of collaboration between oral health, primary care, behavioral health and addiction services will be demonstrated throughout the workshop. 1. Review the history and significance of drug use in the U.S. and TN with a focus on the opioid epidemic. 2. Understand the Biopsychosocial Model in addiction. 3. Recognize Oral Health manifestations of Substance Use Disorders. 4. Review Screening Tools for Substance Use Disorders in an Integrated Primary Care/Dental practice. 5. Review pain management issues in dental patients with and without history of Substance Use Disorder. 6. Discuss MedicationAssisted Treatment in an Integrated Model. Finance Workshop Transformation Management and Data Metrics for Preparation and Participation: The New Age of Healthcare Reimbursement Part 1: Ron Culberson You have made excellent progress in your transformation journey, and now it is time to take your next step towards highfunctioning practice status. We will combine lessons learned from high performing practices with what you need to do to meet TennCare PCMH Provider Operating Manual requirements. Based on Ron Culberson's book by the same name, this presentation shows primary care staff and leaders how to create a less stressful work environment where people want to work. It also helps them understand that excellence combined with fun and humor can improve productivity, create better working relationships, enhance creativity, change the workplace culture, and lead to the delivery of better patient services Bobbi Ryder This session is designed to address multilevel succession planning, with a focus on the Executive Team, including the CEO. Content will address various types of succession plans, including some that have led to success and others that have not. The role of the Board of Directors in relation to CEO level succession planning will be addressed. Resources include a digital version of the NCFH developed tools titled, "Succession Planning Policy and Procedures" (as sample template for CHC use) and the "Personal Career and Succession Planning Guide," both of which are copyrighted by NCFH and provided to participants for their use and local adaptation. The presenter will assess the interests of the audience and will emphasize areas of greatest interest of those present. This will be a highly interactive session with discussion among participants, exchange of experiences and ideas will be encouraged. Curt Degenfelder Nationally, healthcare payors are looking to align their payments with the Triple Aim: reducing total cost of care, improving clinical quality, and improving the patient experience. Current health center reimbursement systems are not in alignment with these goals, and are likely to change. This session will cover two key topics of payment transformation: payforperformance and FQHC Alternative Payment Methodologies (APMs). The session will help CHC staff understand both topics, and identify preparation steps necessary to prepare for changing reimbursement systems. They are happier, more productive and far more likely to stay for the longterm. In this highly interactive session, we will discuss practical and realistic ways to strengthen engagement by developing goals with meat; implementing structured oneonones; speedcoaching; constructive, direct and tactful communication; timely, individualized and meaningful recognition; and ongoing accountability. Participants should attend ready to gain ideas, strategies and skills to immediately increase employee engagement at their health center. 10:30a Clinical Workshop Patient Engagement Chris Espersen 1. Most of us came to work in healthcare to care for people, and the landscape is changing to get us back to what matters the most: our patients. This session will cover the basics patient engagement: business case, social determinants of health, methods of engagement, measurement, and meeting patients where they are at. Leadership Workshop FTLF This session provides an update on the current requirements for health centers regarding compliance and quality programs and shared best practices for implementing these programs that improve the health of patients and the organization. The topics of Teamlets or Robust Patient Care Teams, Patient Care Team Huddles and leveraging those for a new world of quality outcomes and lessons learned on how to ideally scheduling patients and the Patient Care Team. Diane Marriott In a fastpaced world of change in both care delivery transformation and payment reform, many challenges face communitybased primary care organizations and clinics. This session aims at identifying those most relevant to primary care and designing strategies and tactics that provide for longterm sustainability. 1. 6 2:45pm 4:00pm Finance Workshop Uniform Guidance: Lessons learned postimplementation and continuing developments Gil Bernhard 2. Learn about how other states have approached sustainability challenges. 3. Discern techniques and strategies that can be applied by participants to better prepare their organization for longterm success. The Uniform Administrative Requirements, Cost Principles, and Audit Requirements (Uniform Guidance) has presented unique challenges for community health centers (CHCs) in implementing the various administrative requirements as well as opportunities for enhancing policies and procedures, internal controls and risk assessment. The goal of this presentation is to provide an overview of Uniform Guidance, and insights into the lessons learned postimplementation, including best practices to address common findings. The session will also discuss recent developments including the 2017 OMB Compliance Supplement and it's affect on the Federal Single Audit. 2:45pm 4:00pm COO Workshop Making Customer Service a Reality: Creating and Sustaining a Culture of Service Excellence Lisa Mouscher 1. Understanding the importance of establishing and maintaining effective internal control that provides reasonable assurance that federal awards are managed in compliance with statutes, regulations and terms and conditions of the federal awards. 2. Awareness of the challenges, common findings and best practices in complying with the requirements of the Uniform Guidance. 3. Gain an understanding of the recent developments including extension of the procurement requirements and the 2017 Compliance Supplement for the Federal Single Audit. The quality of customer service in community health care influences the health of both patients and the health center itself, yet in today s fastpaced and often challenging environment, CHCs often find it difficult to create and maintain a culture of service excellence. This training for CHC leaders and managers sets the stage for successfully implementing and/or solidifying a culture of service excellence where customer service is an expectation with every interaction in your department or throughout your organization. In this session we will further explore best practices around care team communication events, care team feedback and staff engagement in the Team Based Care Model. We will also explore the evolving need for support staff protocols and will discuss the role of the Tactical Nurse in supporting the provider and the entire Patient Panel. The latest information on the FY 2018 funding outlook for health centers, including the extension of health center mandatory funding and workforce programs, including the Teaching Health Center Program and National Health Service Corp, and reforms to Medicaid and the Marketplace will be discussed. A review of the current political landscape in Congress, including opportunities and challenges related to the Tennessee Primary Care Association 710 Spence Lane, Nashville, TN

8 10:00am Break Fenway Talk upcoming 2018 Congressional elections, will be highlighted as well as specific action steps that health center staff, board members and clinicians can take to support the national and state policy agenda to assist health centers meet growing unmet need in Tennessee. LGBT people face stigma and related health disparities in health care. In spite of the advancements in LGBT acceptance and policy, many LGBT people remain largely invisible to their primary care providers. In order to address these disparities, new requirements in the UDS require health centers to collect and report sexual orientation and gender identity. This session will prepare health centers on how best to capture this data and begin utilizing it to improve clinical outcomes for LGBT patients. Collecting SO/GI data of new and returning primary care patients is critical for health centers to provide a welcoming, inclusive environment. Routine SO/GI data collection in the EHR can be used to improve care and measure and track health outcomes at the individual and population level. Systems that support collection of SO/GI data support improved quality of care, timely clinical information, and improved communication among patients and care teams. Health centers will share their experiences implementing changes within the EHR, revising workflows, and training clinical and nonclinical staff to successfully implement SO/GI data collection and creating an LGBT inclusive environment in primary care. By the end of this session, learners will be able to: 1. Describe LGBT concepts, terms, and health disparities. 2. Summarize how to incorporate SO/GI data collection into your EHR to improve communication, quality care, and data and quality management activities. 3. Describe the impact SO/GI data collection and development of related decision support will have on key stakeholders (patients, staff, and management). 4. Much of what determines health at the community level depends on factors beyond healthcare. The WHO defines Social Determinants of Health (SDH) as "the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life." Initiatives to improve health at the local, national, and international level are rarely successful without incorporating social factors into the plan. Health Centers can leverage the power of the EHR by using the PRAPARE tool to identify and intervene in the major factors that drive poor outcomes.
