1 Our theme for National Health Center Week 2018 (NHCW) is Celebrating Health Centers: Home of America s Health Care Heroes. Health centers serve 27 million patients a number that continues to grow along with the demand for affordable primary care. In addition to their long history as health care homes to millions, health centers produce innovative solutions to the most pressing health care issues in their communities and reach beyond the walls of conventional medicine to address the social determinants of health affecting special patient populations. Each year we celebrate the work and services health centers provide to special populations within their community on designated days during the week. In keeping with the theme of America s Health Care Heroes, we will be acknowledging certain heroes of CHC throughout the week! August 14, 2018 We have arranged a number of activities throughout the day to get

3 staff moving and active. Activities include various fitness sessions in Mashpee. Staff members are also encouraged to get up and take a walk at some point during the day. August 15, 2018, 7:45-9:30am Legislative Breakfast. Politicians from across Cape Cod and the Commonwealth are invited to learn about the current state of CHCs. August 15, 2018, 1-3pm CHC Open House. CHC will open its doors to the community to learn about our services. If you know of anyone who wants to learn more about CHC, please encourage them to attend. August 16, 2018, 8-9am - Staff Service Awards and Dedication of the Outreach and Enrollment Center. Join us as we acknowledge staff members who have been with the Health Center for 3, 5, 10 and for the first time, 15 years! We will also present some special STAR Quality Awards. After the awards, stay as we dedicate the Outreach and Enrollment Center to Mary Ann Waygan and the Founders of the Falmouth Free Clinic!
