By learning to navigate the CAA process effectively, you will awaken critical thinking skills that lead to more resident-focused care plans. Following this session, you will have a clear understanding of regulations, including why the CATs trigger, and how to develop and analyze an effective CAA that can be used to assist your residents in achieving their goals, while meeting regulatory care planning requirements. unlocking the Mystery of the CMS Quality Measures Kathleen Pellatt, RN, RAC-CT; Linda Spokane Why do some residents flag for a certain measure while others do not? What MDS items are used in the calculations? Which measures are risk adjusted and how do I improve them? During this session, you will acquire the keys to unlock and understand these and other QM mysteries in your facility. You will define and interpret the terms and conditions that make up QM logic in order to help your own team code more accurately and identify residents who will likely flag on a QM. You will leave able to identify the difference between long-stay and short-stay residents, recognize MDS items used in QM calculations, and explain why some residents are excluded if they have specific conditions coded on the MDS. using ehr to its full Potential: 21st Century leadership Dan Billings, RN The facility of tomorrow is almost here. Paper-filled filling cabinets and folders will soon give way to sleek systems staffed by tech-savvy nurses. Discover how an optimized and effectively used EHR (Electronic Health Record) system can transform your facility s operations and outcomes and discover where you fit in. Walk away with the tools and resources needed to promote better IDT communication, better use of staff time, and better facility reimbursement. Learn how to configure an EHR system to watch for residents change in condition, incorporate INTERACT into an EHR, and build in the benefits of EHR into your everyday practices. rehospitalizations: Solutions through root-cause analysis Lisa Hohlbein, RN, RAC-MT Not surprisingly, hospitalization is common for long and short-stay residents for varied reasons, such as de-conditioning, illnesses, or other complications. Hospitalizations are costly, and many are potentially avoidable. Healthcare reform promises continued scrutiny on this issue, which will challenge you to develop stronger clinical and operational systems. In this session, you will increase your understanding of how to analyze patterns of re-hospitalizations through a process of root cause analysis and management of risk factors. Learn additional strategies to prevent avoidable hospital readmissions, including utilizing the INTERACT process to keep your residents in the home and out of the hospital. With this much great content, the most painful part might be choosing which session to attend. The opportunity to hear speakers that are so knowledgeable, and trusting that what they present is the latest and most accurate information, is invaluable. Sharon Siemonsma, rn Madison, SD Pain assessment and treatment: documenting and tracking results Karen Feldt, PhD, ARNP, GNP-BC Among residents in SNF, 71% to 83% report pain that affects their activities of daily living. As caretakers, we want to minimize both the acute and chronic pain that our residents feel. During this session, you will review pain assessment and treatment for residents in long-term care facilities with special attention to the MDS 3.0 assessment criteria. Obtain suggestions for tracking assessment and demonstrating treatment strategies to reduce pain with care activities. overcoming Challenges in diabetes Care for the nursing home resident Dana A. Saffel, PharmD, CPh, CGP, FASCP Type 2 Diabetes has reached epidemic proportions in nursing home residents. The vast majority of these residents have co-morbidities that complicate treatment and put them at risk for decline and negative outcomes. Nursing staff are working in nursing homes with the challenges of assessing, treating, and managing co-morbidities of diabetic residents. These nurses are facing survey deficiencies and require skills in responding to resident choice and voice on treatment regimens. Learn how to support enhanced assessment and improve patient outcomes in residents, using evidence-based treatment of diabetes. Supported by an unrestricted educational grant from Novo Nordisk. improving Quality of life for elders through assessment Jacqueline Close, PhD, RN, GCNS-BC, FNGNA; Diana Sturdevant, MSN, APRN This session, collaboratively provided with NGNA, will help you learn to design processes using evidence-based resources that improve outcomes while meeting the needs of older adults with comorbid conditions requiring complex care. At the end of this session, you will understand the importance of helping residents catch some ZZZs.
