1 People Melissa s Story: Gastric Sleeve Success By: Stacy Tintocalis, Freelance Writer For Melissa Loftis, her mother s heart procedure was a wake-up call. It opened my eyes, Melissa said. Diabetes and heart disease run in my family, Melissa said. And I m a single mom. My son is autistic. I wanted to be there for him, to play with him and watch him grow up. But the problem wasn t Melissa s heart. It was her weight. OMCNews The right care, right here. Melissa was 35 years old, over 400 pounds, and had a body mass index (BMI) of 64. She was hurting. She couldn t breathe. If she was out of the house, she had to stop walking every few steps. I knew something had to change, she said. Melissa had already gone through the weight loss roller coaster. She d done over-the-counter weight loss supplements, exercise, and dieting. I d done it all and nothing worked. I d lose a few pounds, and it would come right back. The answer was gastric sleeve surgery, a procedure performed laparoscopically through small incisions in the abdomen. The surgeon removes part of the stomach and reduces the patient s stomach size by approximately 20%. The result is permanent stomach-size reduction, though some patients may see stomach dilation over time. Dr. Magdy Giurgius, a bariatric surgeon at Ozarks Medical Center (OMC) Surgical Specialists Clinic, performed gastric sleeve surgery on Melissa Loftis on July 19, Since surgery, she has lost 55 pounds. Combined with her preoperative medical weight management, she has lost a total of 137 pounds so far. Melissa s gastric sleeve surgery only took two-and-a-half hours. Before and after the surgery, Dr. Giurgius talked to Melissa and her mother to answer their questions. You don t see the same care and compassion in Springfield, where I grew up, Melissa said. Dr. Giurgius took the extra time, so we could understand. Dr. Giurgius has patients take a class before their gastric sleeve procedure. Melissa talked to a dietician, physical therapist, and counselors. They all prepare you. So I knew there was going to be pain. I knew there d be a struggle. When I went into the hospital, I knew what to expect. After the procedure, there was pain, but Dr. Giurgius s staff helped. If I needed something, they were there, Melissa said. 4 October 20, People Making a Difference Every Day at OMC: Gena Brucker s Story Gena Brucker wasn t yet an Ozarks Medical Center (OMC) employee when she came in for a routine doctor s visit at OMC Women s Healthcare. She was pregnant, and everything about her pregnancy had been normal until Dr. Brian Israel said he couldn t find the baby s heartbeat. After a quick ultrasound, Dr. Israel confirmed that she d lost the baby. It was a total shock. Dr. Israel let us cry, Gena said. Then he gave us educational materials and gave us options. We decided that night that we d get induced. To get induced, Gena had to return to OMC s labor and delivery department, which meant walking down a hall past the nursery. Walking past that nursery, she said, it seemed like the longest hall in the world. Shannon, the nurse who would assist with the induction, was waiting at end of hall for Gena. Just as Gena reached her, a baby started crying. When Shannon heard the cry, she instinctively knew what to do and whisked me away. Even though Gena was there to get induced, she was still in denial. I told Shannon, I need a new ultrasound. I thought something was wrong with Dr. Israel s machine. That s when Carol Powell in radiology took Gena to get another ultrasound. Again, the ultrasound showed that Gena had lost her baby. Carol touched my leg and said, I am so sorry that we had to meet under these circumstances. That touch meant a lot, Gena said. Carol took the time to acknowledge that this experience sucked. After that, Gena began the induction with Shannon. When Shannon mentioned that she d be giving Gena pain medicine, Gena asked, Is this going to hurt the baby? Hearing her own words, Gena started crying. Shannon replied by grabbing my hand until the crying subsided. She had great instincts. She knew the reality of my words had hurt me greatly: My baby was gone, and I didn t have to worry about pain medicine affecting him. Shannon continued to hold my hand as we began the induction. Eight hours later Gena delivered her son Rayden, spent some time with him, then had to give him back. It was my final goodbye, Gena said. After she gave Rayden back, Gena had to go into a surgery room for an emergency procedure. I remember this anesthesiologist with big brown eyes, Gena said. And he gets kind of close to my face and whispered, Just breathe. I needed to breathe to go to sleep, but he was actually telling me, Just breathe. When I heard him say, Just breathe, I knew he meant, We got this. You breathe, and we ll take care of you. The saying Just breathe has been my motto since Rayden s death. In moments of deep sorrow the only thing you can do is breathe. Now, nine years later, I still remember the color of that anesthesiologist s eyes. I can still see what he looks like. At that point, four OMC employees had made compassionate gestures towards Gena. Each gesture, though small, was part of her healing journey. Even as she was leaving OMC, one more person made a difference. OMC gives you a box when you ve had a loss, and everything pertaining to the birth is in that box. The person who was wheeling me out of the hospital, a volunteer, took a moment and touched my shoulder. She knew what that box represented. That day was August 1, That day, I became OMC, Gena said. I couldn t wait to become part of an organization that treats its patients so well. Six weeks later, Gena started working for OMC, first in medical records, then in registration. Gena is currently manager of patient access. No matter what interaction we have during our workday, Gena now says, those interactions make a difference and create a story of compassion and love and understanding. We get to impact patients lives. We get to decide how people will remember their experience at OMC and what story they ll tell others about us. When I think of why I m at OMC, I think about how every minute I m here, I can make a difference.

She was the mail clerk and has worked for OMC for over 6 years Rule for Closing Tasks In order to help our IT team concentrate on active requests we will be implementing a rule for closing tasks. A task will be closed if there is no response from the requestor after 3 business days, 2 s, and 1 phone call/voice mail. If we get a response at any time within the 3 day window the window will reset. This rule is being applied to all task categories in our service desk system including help desk, business intelligence/report request, network, security and applications. When a task is closed this does not mean you have lost your chance to receive support for any of our teams, we are simply trying to keep our task queues clean and relevant. ENVIRONMENTALhealth& safety The OMC EH&S Program has created new Chemical Spill Kits and will be placing them throughout the organization in the next couple of weeks. Along with these Spill Kits, Tom Morris (OMC s Environmental Consultant) will work to schedule training appointments, for those departments who work with Hazardous Chemicals, on the actions and SOPs to be employed for OMC s Spill Response protocols. Melissa was in the hospital for two days after the surgery to make sure her pain was under control and also to ensure there were no complications. Once home, she could do basic needs without someone in the house with her. There were no blind spots throughout this whole thing, Melissa said. Dr. Giurgius sends patients home with a notebook that contains guidelines for before and after the surgery, plus exercises and nutrition options. I knew every step of the way what to expect. Dr. Giurgius and his team are amazing, Melissa said. They want you to have success. I don t think I could have done it without them. Gastric sleeve surgery is one of many weight loss (bariatric) surgeries provided by Dr. Giurgius at OMC. He also performs balloon weight loss surgery, a procedure where the surgeon endoscopically places a gastric balloon into the stomach and fills it with saline. Six months later, the balloon is removed. To be a candidate for gastric sleeve weight loss surgery, the patient must have a body mass index (BMI) below 60. If the patient s BMI is above 60, a supervised medical weight loss program can help the patient meet the BMI requirements. The BMI for obesity starts at 30. Doctors can help patients lose the initial extra weight. For the Gastric Balloon, a BMI of is recommended. In Melissa s case, Dr. Christopher Cochran of OMC Internal Medicine Clinic helped her get her weight down enough to safely do the procedure. She went from a BMI of 64 to 60 before the surgery. Three months after surgery, Melissa has a BMI of 49. You have to follow the diet and do clear liquids before surgery and then clear liquids after surgery, Melissa said. [Dr. Giurgius s staff] gives you a booklet that walks you through the process on what to expect and how to go about it. You have to have 65 grams of protein a day. Now Melissa keeps a nutrition log of what she takes in every day, including carbs protein, water, and also an activity log, which keeps her motivated. This surgery isn t a solution. It s a tool that will help you, Melissa said. If you don t follow the diet guidelines, you can stretch your stomach out and go back to square one. According to the OMC Community Health Needs Assessment, roughly one third of the population in Howell and Oregon counties (32.3% in Howell and 33.4% in Oregon) over the age of 20 has a BMI over 30. The national average is 27.1%. Missouri s state average is 30.3%. Insurance often will cover the cost of bariatric procedures. The staff at OMC Surgical Specialists Clinic can contact a patient s insurance company to see if they will pay for all or part of the surgery. Medicare also will pay for some forms of weight loss surgery. The benefits of bariatric surgery may outweigh the risks of remaining obese. Obesity is linked to over a dozen serious health conditions including Type 2 diabetes, heart disease, stroke, and multiple cancers. Patients who have bariatric surgery see many of these conditions improve. In some patients, weight loss after surgery can reverse eye damage and improve eyesight. Getting this surgery shows you how much strength and willpower you have for yourself, Melissa said. Nothing happens overnight. I didn t put on the weight overnight, so it s not going to come off over night. The sleeve, as a tool, really helps. Even though it s only been three months since her surgery, Melissa has a new lease on life. She can now eat small meals that stay within the post-surgery guidelines. 11 October 20, Quality Ozarks Medical Center Adds Ninth Clinic with Chronic Care Management By: Stacy Tintocalis, Freelance Writer Recently, Ozarks Medical Center (OMC) rolled out Chronic Care Management (CCM) in OMC Urgent Care Clinic. OMC currently has CCM in eight other clinics: OMC Alton, OMC Gainesville, OMC Mammoth Springs, OMC Mountain Grove, OMC Mountain View, OMC Thayer, OMC Winona, and OMC Internal Medicine in West Plains. OMC Winona recently reached 200 patients enrolled in their Chronic Care Chronic Care Management is a form of telephonebased case management for patients who have two or Hilt, Clinic Director; Ashley VanWinkle, LPNII; Back Row: Heather Hill, Management Program. CCM provides education to Clinics Program Coordinator; Barbara Honeycutt, LPN II; Dr. Diane help patients be better self-managers and establishes Koenig; Windy Horton, LPNII-Chronic Care Management Nurse; Angie a comprehensive care plan that is monitored, revised, Smith, EVS; and Susan Lindsey, Clinic Supervisor. and implemented in a three-way partnership between the physician (or nurse practitioner), nurse, and patient. Once in the CCM program, the patient s doctor devises a care plan for one year, and the CCM nurse coordinator contacts the patient at a minimum of monthly, but sometimes weekly, depending on need. They also see the patient at doctor s visits about every 8 to 12 weeks. CCM nurses get to know their patients and use phone conferences to focus on the preventative services that CCM nurses offer such as relaxation techniques; label reading; better sleep habits; exercises for flexibility, strength and balance; and diabetes plate method. In January, OMC s CCM nurses will also train to be diabetes self-management educators. According to Heather Hill, OMC Clinic Program Coordinator, CCM nurses are responsible for scheduling annual wellness visits and other preventative tests covered by Medicare. They can arrange transportation, perform thorough medication checks, and enroll patients in programs, such as, Meals on Wheels. This is all in addition to monitoring the patients A1C, weight, or blood pressure, or other measures depending on the condition, Hill said. The program is allowing us to provide extra support to our patients, for example, when a patient is too ill to prepare food, the CCM nurse coordinator can make sure meals are delivered to the patient. CCM nurses are easily accessible to the patients and are the liaisons to the health care providers regarding any medical related condition the patients may experience. Patients with two or more chronic conditions may be eligible for CCM. According to Medicare, chronic means the patient will live with the condition for greater than 12 months. Individuals with the following chronic conditions may be eligible for CCM: Alzheimer s Disease and related dementia Arthritis (osteoarthritis and rheumatoid) Asthma Autism Spectrum Disorders Cancer Chronic Obstructive Pulmonary Disease (COPD) Congestive Heart Failure Depression Diabetes Heart Failure Hypertension Ischemic Heart Disease Obesity Osteoporosis CCM is not limited to these conditions. A patient s physician will decide what is or is not chronic. Each CCM nurse can manage up to 200 patients on the program. Currently, OMC Gainesville, OMC Internal Medicine, OMC Mammoth Springs, and OMC Winona have successfully enrolled 200 patients each. Spaces are available in other OMC clinics. Medicare and Medicare Advantage cover the majority of the cost of the program so the patients have little or no out-of-pocket expense.
