This post is for a Consultant Obstetrician & Gynaecologist with an interest in Urogynaecology and Minimally invasive Gynaecologic surgery. This will in include working in partnership with the current consultants leading the peer review ensuring trust compliance with data collection. Provide gynaecology outpatient services and participate in the multidisciplinary team (MDT) meeting. Provide Urogynaecological services within the Trust working in partnership with the Urogynaecology lead clinician. Ensure good clinical governance of obstetric services and general gynaecology. Provide an on call service for gynaecology and or obstetrics during allocated weekdays and out of hours when required. Provide antenatal and emergency gynaecology outpatient services. Provide obstetrics and gynaecology ward rounds. Provide day assessment unit and elective caesarean list cover. There will be an opportunity to develop any particular areas of interest not discussed in the job description.

5 3.1 Duties of the Post The duties outlined below are not definitive and may be changed in accordance with the needs of the service. The allocation of programmed activities (PAs) will be based on 8.5 for direct clinical care (including predictable and unpredictable emergency on call work) and 1.5 PAs for supporting professional activities. The exact allocation of programmed activities in terms of the timetable and commitments of the post will be agreed with the Clinical Director, and will be reviewed 3 months after appointment. The timetable is indicative as the job plans of all consultants are being reviewed, and trust wide theatre re-organisation is currently also taking place. Extra sessions are required for some specialties including gynaecology, which is showing a marked increase in activity. The job plans of all consultants are reviewed on a regular basis and will need to change over time to accommodate the needs of the service. The department is going through transformation of services and there may be opportunity for further specialisation by existing consultants if this fits with the needs of the service. The MDT will take place at Luton and Dunstable Hospital. 1. Multidisciplinary team work is envisaged as a key component of the working of this Trust and will involve Multi Disciplinary Team (MDT) discussion with other colleagues. He/she will participate in out of hours consultant cover on a rota. On-call responsibilities will involve multidisciplinary team working for all acute admissions referred to the unit. The appointee will be on call for obstetrics and gynaecology. This is a non resident on call commitment. 2. Continuing responsibility for the care of patients in his/her charge, including all administrative duties associated with patient care. 3. Development of clinical interests that fit in with Directorate needs and the strategic direction of the Trust. This may involve an outpatient hysteroscopy clinic once fortnightly. 4. Responsibilities for carrying out teaching, examination and accreditation duties are required, and for contributing to undergraduate and post-graduate and continuing medical education activity. 5. Provision of cover for Consultant colleagues periods of leave in accordance with arrangements agreed within Trust policy. 6. Participation in clinical audit and in continuing medical education. 7. Undertaking ward rounds and Gynaecology day cover as appropriate. 8. Providing evidence-based care for patients in a multidisciplinary setting. Conducting suitable duties in cases of emergencies and unforeseen circumstances The on call would be for obstetrics and Gynaecology, with rota frequency of approximately 1:10-1:11. The on call rota is subject to change in the future, and acute service provision will remain the most important priority for the directorate. The proposed timetable will include:

He/She will share in the teaching, educational activities, training and supervision of junior medical staff. Job Plan The pattern of work outlined below may change in the future to meet local changing needs. There will then be annual review meetings, which will take place in line with the new Consultant Contract, Appraisal and Job Planning arrangements which have been established by the Trust. As outlined above this is based on 42 weeks period to allow for A/L and S/L. The Job Plan will be a prospective agreement that sets out a consultant s duties, responsibilities and objectives for the coming year. The Job plan will cover all aspects of a consultant s professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will provide

7 personal objectives, including details of links to wider service objectives, and details of the support required by the consultant to fulfill the job plan and the objectives. They will be asked to help produce, maintain and update departmental clinical management guidelines in line with evidence based best practice. The successful candidates will have dedicated desk space, within shared office accommodation, with an internet enabled computer and shared secretarial support.

8 Medicolegal To investigate complaints and risk on behalf of the Chief Executive Officer. Liaise with HM Coroner and other legal advisers over relevant matters. Identify and appropriately refer vulnerable infants according to Trust safeguarding policy. Consultants are responsible for the strict control of release of confidential information to third parties and investigation of complaints of malpractice or medical negligence. 10 All Consultant staff are based at the Luton and Dunstable Hospital. Junior Medical Staff 10 SpR Deanery funded posts 4 Specialty Doctor (middle grade) 2 ST1/2 Deanery funded posts 2 GPVTS doctors 2 FY2 doctors 1 FY1 doctor 1 Clinical fellow (junior grade) The department runs a rota with two middle grade doctors on call 24 hours a day, one of whom is more senior. We have recently had trainees complete advanced training skills modules (ATSMs) in fetal medicine, maternal medicine, advanced labour ward practice, labour ward lead, acute Gynaecology and early pregnancy and also offer other ATSMs such as colposcopy, Urogynaecology, medical education and subfertility and reproductive medicine. Management Arrangements A Divisional structure is now well established within the Trust, the four divisions are; Women and Children s, Medicine, Surgery and Clinical Support Services. Miss Neela Mukhopadhaya and Mr Bright Gyampoh are the Clinical Directors for Gynaecology and Obstetrics, respectively, both supported by a General Manager, Miss Tanith Ellis and her team. Dr Beryl Adler, Consultant Pediatrician is the Divisional Director for Women and Children s, and Miss Trish Ryan the Head of Midwifery. The team meets every week. They are supported by the Chief Medical Advisor, Dr Danielle Freedman and other Medical Directors. OBSTETRICS There are 22 consultants in the obstetrics and Gynaecology on call rota. Nine colleagues, including the most senior obstetrician and clinical lead for Obstetric risk management contribute to the resident night rota. The unit is committed to providing 122 hours of consultant presence on delivery suite and looking to increase this in the near future. Delivery Suite and Midwifery Led Birthing Unit The eleven room Delivery Suite includes a special needs room including usage for bereavement. Two operating theatres are present on the Delivery Suite, one of which is currently shared with elective minor gynaecology. Adjacent to the delivery suite is a 12 bed Delivery Suite Annex where late pregnancy problems and elective caesarean section patients are admitted. The maternity service had 5200 deliveries in A four bedded midwifery led birthing unit opened in July 2010, this is co-located next to the delivery suite. There are two maternity wards, including transitional care cots, with the majority of women transferred home early after delivery. The department runs monthly PROMPT training sessions for midwives, obstetricians, maternity care assistants and Anaesthetist. In addition the midwives attend the monthly obstetric study day every year.

11 Antenatal Clinics There are currently ten consultant antenatal clinics each week. Most women have community based antenatal care, much of the care being undertaken in general practice surgeries by community midwives. Approximately 25% of hospital births are to women of Asian origin. Link Workers are available at the Unit and in the Community to help with communication between patients and staff. There are currently specialist antenatal clinics for gestational and pre existing diabetes, mental health, twins, haematology, hepatitis and HIV and the department is planning to advertise more specialist midwifery posts. There is a monthly obstetric multidisciplinary meeting held with the obstetricians, anaesthetists and colleagues such as hepatologists or haematologists, as required, to discuss and plan management for women who are pregnant with complex health issues. GROW scans are in place now. Fetal Medicine Mr Stephen Burrell, Miss Jacqueline Bamfo and Mr Sam Das run the fetal medicine service, together with a dedicated foetal medicine midwife. There is a higher than average rate of fetal abnormality in the local population. Over 300 referrals/year are seen in the Fetal Medicine Clinic. Chorionic villus sampling is undertaken regularly and referrals are accepted from surrounding districts. There is regular liaison with the geneticists and three times a year a joint perinatal meeting is held with the geneticists from Northwick Park Hospital and perinatal pathology consultants from Royal London. Fetal cardiac echo is provided by Rachel Andrews, a visiting consultant from Great Ormond Street Hospital. A weekly fetal medicine management group meeting is held attended by the fetal medicine consultants and neonatal consultants to discuss the arrangements for labour for women with known fetal abnormalities, or issues such as drug dependency/ use of medications in pregnancy which may affect fetal well being at birth. Ultrasound All pregnant women are offered a nuchal translucency scan at eleven to thirteen weeks and an anomaly scan at eighteen to twenty weeks. Anomalies found at the twenty week scan are seen in the fetal medicine clinic and referred to specialist centers as necessary. Ultrasound scanning for obstetrics and gynaecology is undertaken in the within the unit. Neonatal Intensive Care Unit The Neonatal Unit is situated next to the delivery suite and consists of thirty-six cots, of which nineteen are designated for neonatal intensive care. A temporary build to expand the unit opened in May 2008 and a new NNU is planned for the future. This is a Level 3 Unit accepting both in-utero and postnatal transfers. The Consultant Obstetricians are responsible for the care of women transferred to the Unit because of severe fetal compromise and for those who accompany their infants when they are transferred postnatally.

12 Obstetric Anaesthetic Clinic There is a dedicated anaesthetic clinic run by a group of Obstetric anaesthetist where women with anaesthesia-related medical co-morbidities such as cardiac patients, the morbid and supermorbid obese women, refusal of blood transfusion, and some women discussed in Obstetric MDT are referred for further review during their antenatal period. GYNAECOLOGY Gynaecological Oncology Luton and Dunstable Hospital is part of the Mount Vernon Cancer Network. The current arrangements are that major gynaecological cancer surgery is centralised at the Herts and South Beds Cancer Centre based at Watford General Hospital. Mr Alasdair Drake is the NSSG lead for the Mount Vernon Cancer Network. Mount Vernon is the major referral centre for both radiotherapy and medical oncology, and there is close collaboration with the Gynaecological Oncology service. Professor Hoskins is the consultant oncologist from Mount Vernon who undertakes joint clinics at Luton and Dunstable. The oncology team consists of Mr Owens Owens, gynaecologist with a special interest in gynaecology oncology. Miss Imelda Flanagan is a whole time experienced clinical nurse practitioner in gynaecology oncology. Colposcopy The colposcopy for the local health community is organised through the Clinical Cytology administrative department, which is physically closely integrated with the Cytology Laboratory. Miss Pushpa Maharajan is the Lead Colposcopist. The unit is an active colposcopy department undertaking basic and further colposcopy training and actively promotes screening and training in primary care. All colposcopists are active and accredited members of BSCCP. The Colposcopy service is currently provided by three consultants and one specialist nurse colposcopist, with seven clinics each week at present. The colposcopy secretary leads the colposcopy secretarial team, which is essential to the smooth running of the service. The lead for colposcopy is responsible of for the audit and clinical governance of the colposcopy service. Cytology services are organised currently through Bedford Hospital. Dr Fraser Mutch is the consultant pathologist in overall charge of the cytology at Bedford and is the Cervical Screening Coordinator. Gynaecology Ward and Theatres Ward 34, is a dedicated gynecological ward of seventeen beds. Gynaecological emergencies are seen in a dedicated daily emergency gynaecology clinic. An early pregnancy assessment clinic functions each weekday and currently weekends. Two clean rooms on this ward are used to provide ambulatory Gynaecological services such as colposcopy, manual vacuum aspiration and outpatient hysteroscopy. There are Gynaecology operating lists each week with many theatres in the Trust. Major Gynaecology operations run whole day Mondays and Wednesdays and half day Thursdays with extended times. Major cases and laparoscopic cases are performed within the main theatre complex in the surgical block and minors are done within the unit where the gynaecology ward is located. Four consultants currently perform laparoscopic hysterectomies and other advanced laparoscopic procedures.

13 Early Pregnancy and Emergency Gynaecology unit This includes an early pregnancy nurse led service in the mornings and afternoon emergency gynaecology clinic. There are 3 qualified nurses, 2 HCA s and 1 Admin staff. The unit treats about 5,500 patients every year and offers modern management of miscarriage such as MVA clinics, dedicated recurrent miscarriage clinic; Consultant led scans and outpatient management of Bartholin s abscess. The post holder will work with Miss N Mukhopadhaya Lead for emergency Gynaecology services and Mr SS Das. Urogynaecology The Urogynaecology unit is led and managed by Mr A Fayyad, RCOG accredited sub specialist in Urogynaecology. He is supported by another locum urogynaecologist and a nurse specialist. The unit offers tertiary level urogynaecology services to the surrounding counties including the investigation and management of women with urinary incontinence and pelvic organ prolapse. This includes ambulatory urodynamics, pelvic floor ultrasound and the latest techniques in the management of recurrent organ prolapse and incontinence. Particular expertise is provided in minimal access urogynaecology (laparoscopic sacrocolpopexy and colposuspension) and the use of meshes for advanced pelvic reconstructive surgery. The unit runs a monthly joint pelvic floor clinic with the colorectal surgeons. Fertility The fertility service is based within the new Fertility Unit which opened in May This service is managed by Miss Akbar and Mr Jose and offers joint male/female consultation and treatment with a full range of investigations including detailed ultrasonic assessment of ovarian and endometrial function. The unit provides satellite IVF services working in conjunction with Bourn Hall, where around twenty Luton patients attend each month for egg collection and embryo transfer. Miss Akbar is the Person Responsible to the HFEA. Gynaecology Ambulatory Care Unit This new unit opened in August 2013 and is co-located within our gynaecology ward, staffed by our gynaecology nurses. It consists of two large clean rooms with two consulting rooms and a large recovery area with five couches. One clean room is dedicated to colposcopy with out patient hysteroscopy and urodynamic clinics provided in the second clean room. There are currently three outpatient hysteroscopy clinics each week including one dedicated to the investigation of post menopausal bleeding. The service is currently expanding so that the majority of hysteroscopic examinations are carried out here instead of in day case theatre. Outpatient Novasure, Myosure resection of endometrial polyps and submucosal fibroids and ESSURE are also provided here. TEACHING/UNDERGRADUATE/POSTGRADUATE MEDICAL EDUCATION Luton and Dunstable Hospital NHS Trust is an associate teaching hospital taking medical students from University College London. Currently nine medical students are attached to obstetrics and gynaecology and Miss Mukhopadhaya is the undergraduate tutor. There is a continuing responsibility for the teaching and proper training of the junior medical staff. The modern Postgraduate Centre provides excellent facilities for continuing education of hospital medical and dental staff and general practitioners. Within the Centre there is an excellent medical library with internet facilities. There is a weekly Grand Round to which the Obstetrics and Gynaecology Department is a regular contributor. There is also a dedicated bleep-free teaching for the department on Friday afternoons The University of Bedfordshire currently has 2 programmes for student midwives both at degree level.

14 CONTINUING PROFESSIONAL DEVELOPMENT The Trust is committed to supporting CPD for consultants. Consultants are encouraged to attend meetings and courses in their areas of special interest. CLINICAL AUDIT An audit programme is well established within the department with a yearly forward audit plan. All medical staff in the Department are expected to attend audit and clinical governance meetings and it is anticipated that the new consultant will contribute on a regular basis. Monthly perinatal morbidity and mortality meetings are held jointly with the neonatologists with gynaecology meetings and obstetric meetings in the intervening weeks. Mr Burrell is the lead for Audit and Clinical Governance. RESEARCH Research interests are encouraged. The hospital has academic links with Bedfordshire University and Hertfordshire University. Dr Travill is the Director of Research and Development. The new University of Hertfordshire Postgraduate Medical School has been opened with the remit of actively supporting consultants in pursuing their research interests. PRIVATE PATIENT FACILITIES The Cobham Clinic is a thirteen bedded Private Patient facility on the first floor of the surgical block. Two fully equipped consulting rooms are also available for out-patients. 5. General Information about Luton & Dunstable University Hospital NHS Foundation Trust The Luton & Dunstable University Hospital NHS Foundation Trust - more familiarly known as 'The L&D' - is a highly-rated Acute Hospital located conveniently close to major hubs in south Bedfordshire. In 2011 we became a University Teaching Hospital of University College London. We have a total of 641 beds across all disciplines. Today the L&D has an international reputation as a pioneering leader in a number of specialty fields of medicine and surgery. We serve a local population of over 400,000 from Luton, Dunstable, together with other towns and villages in the south Bedfordshire and north Hertfordshire region. With convenient road, rail and airport connections, the L&D is also well placed as a regional centre of excellence for key specialties such as Neo-Natal Intensive Care, Obesity Research and Surgery, Neurophysiology, Maxillofacial and Cancer Services. The L&D is also home to one of the largest Breast Screening Centres in the country and the regional Limb-Fitting service. At the Luton and Dunstable Hospital patient safety is our highest priority. Consultants have a key role to play in leading work to improve the safety of patients both in terms of demonstrating exemplary practice and in participating in improvement initiatives. Job plans will reflect this emphasis and a key objective will be to demonstrate your contribution to driving up patient safety at the hospital and the L&D s aim to Lead the NHS in Patient Safety. We are conveniently situated at Junction 11 of the M1. Luton lies conveniently close to London, and has perfect villages to the surrounding areas. Road and rail links to London and the Midlands / North are excellent, and of course London Luton Airport is with fifteen minutes of the hospital. If travelling by car, we have 400 parking spaces. About 41% of Luton s population are from cultural and ethnic minority backgrounds. The main groups are Pakistani, Bangladeshi, Indian, African-Caribbean and Eastern European. Luton is an area of high deprivation; consequently our local population display diseases and conditions which are more usually associated with those of an inner city area. The hospital provides a full range of acute services and has: Level 3 Neonatal Intensive Care Unit with 11 bedded Intensive Therapy Unit and 8 bedded High Dependency Unit. There are plans to redevelop the L&D site which include a new,

16 purpose built Neonatal Intensive Care Unit with increased capacity and improved facilities for parents. 13 bedded Coronary Care Unit The hub for the Oral and Maxillofacial Network for Bedfordshire The Regional Centre for ENT and Head & Neck Surgery. One of the two NHS accredited Specialist Bariatric Centres for East of England 28 bedded hyperacute stroke unit serving, Luton, Bedfordshire and Hertfordshire. 6 operating theatres in the main theatre complex. An orthopaedic theatre complex and Eye Treatment Centre with 4 theatres Beds and Herts Breast Screening service, serving a 170,000 patients. A new 5.5 million Cardiac Centre (Cath Lab) There is a wide range of radiological diagnostic facilities provided by 12 consultant radiologists, including CT scanning, ultrasound, isotope imaging and MRI. There is a 23-bedded Dialysis Unit on site managed by West Hertfordshire Hospital with nephrology consultant advice available. The Education Centre (COMET) houses the hospital library, which has a wide range of resources, books, journals and computers to facilitate literature search and helpful librarians. The hospital has 3 convenience shops and two Cafés in the reception areas. The restaurant for staff, patients and visitors is well attended. There is a Link cash point in the hospital reception area, which is available for staff, visitors and patients. A Barclays Bank cash point is within 5 minutes walk from the hospital, and a Tesco supermarket is within 15 minutes walk from the hospital. The hospital offers temporary accommodation on site. The accommodation services are contracted to a private company. There is a car park for senior medical staff adjacent to the Emergency Department. 7. Main Conditions of Service The appointment will be subject to the Terms and Conditions Consultants (England) 2003 as approved by the Trust Board and as amended by the Board from time to time. The appointment is on a whole-time basis. A candidate who is unable for personal reasons to work full-time will be eligible to be considered for this post. The ability to work part-time only

17 should be indicated when applying and, if such a person is appointed, modification of the job plan will be discussed and agreed between yourself and the clinical director/general manager. If the consultant appointed chooses to contract on a whole-time basis and wishes to undertake private practice, this private work must be carried out in accordance with the Code of Conduct for Private Practice. Consultants are expected to provide cover for colleagues for leave and other authorised absences from duty upon a mutually agreed basis. Arrangements for annual leave: This is arranged by mutual agreement of consultant colleagues and approval of the Clinical Director, in accordance with standard Trust/NHS regulations. It is essential that as a minimum six weeks notice is given to allow for proper planning and prevent cancellations of patients appointments/surgery. All Consultants have a continuing responsibility for the care of patients in their charge and for the proper functioning of their department, and are required to undertake the administrative duties associated with that responsibility. The successful candidate will be required to reside within ten miles or 30 minutes travelling time of the Luton & Dunstable University Hospital NHS Foundation Trust. The Trust requires the appointee to have and maintain full registration with the General Medical Council. Medical and Dental Staff are advised to continue to be members of one of the medical defence organisations. The present salary scale is 76, ,490 per annum, plus an on call availability supplement if appropriate. The successful candidate will commence on the minimum of the scale unless advised differently during the appointment process. 19 Confidentiality In the course of their duties employees will have access to confidential material about patients, members of staff and other Health Service business.
