1 Jersey General Hospital, States of Jersey Individual Placement (Job) Descriptions for Foundation Year 2 Placement The type of work to expect and learning opportunities Where the is based Clinical Supervisor(s) for the Main duties of the F2 - Anaesthetics Anaesthesia General Anaesthetics, ITU, Obstetric Anaesthesia, Pain clinics, airway management. Pre-operative assessment of patient. History and examination, reviewing investigation results. Opportunity for intubation under supervision - Endotracheal intubation. LMA insertion. Venous cannulation. Central line insertion. Arterial line insertion. Safe prescription. Pain management. Local anaesthesia. Spinal anaesthesia. Epidural anaesthesia. Peri-operative monitoring. Post-operative care. Jersey General Hospital, States of Jersey. Dr Alan Thompson / Dr Andrew Woodward As above. Supernumerary. Work involves shadowing anaesthetics registrar or consultant. Well Supervised attachment overall, plenty of opportunity to gain confidence in venous cannulation, arterial line insertion, intubation and airway management. Typical working pattern in this 8am to 5pm Monday to Friday. Unbanded job. At least 3 full days of theatre list a week. Also some day theatre sessions. Maternity once a week/2week, when shadowing general on call anaesthetist. Epidural anaesthesia. ITU 1 day per week. Also covers calls from emergency department. Ward round in the morning, then reviewing and presenting patients to responsible on call consultant or registrar. There is also opportunity to do a 24hr on call shift with a registrar (day off for subsequent day). Pain clinic with consultant or registrar. There s also

2 opportunity to be in theatre for pain list (e.g. facet joint injection). The Rota is very flexible and should a trainee prefer any specific part of anaesthesia, it can be arranged accordingly. (E.g. more on calls). At no point of the attachment is the trainee left on their own to perform procedures. The employer for this post is Jersey General. The post will be based in Jersey General, which is a medium sized acute district general and provides an extensive range of secondary services for the growing local community of approximately 90,000. Look after ward patients (mainly stroke and chronic neurological patients requiring rehabilitation); also cover the cardiology and diabetic clinic; also cover the acute admissions unit twice a week. Samares Ward, Centre of Rehabilitation, Overdale Hospital. Clinical Supervisor(s) for the Dr. Michael Richardson (Consultant Care of the Elderly) Dr. Howard Gibson (Consultant Neurologist) Dr. Andrew Mitchell (Consultant Cardiologist) Dr. Peter Bates (Consultant Diabetic physician) Main duties of the Typical working pattern in this You will be required to work as part of a large multidisciplinary team consisting of physios, occupational therapists, SALT colleagues, social workers and allied health staff to look after patients who are medically stable but not fit for discharge due to their dependence on ADLs due to their illness. You will conduct a daily ward round and trouble-shoot issues raised by patients and then take part in a twice weekly MDT meeting conducted by the consultant. You have the opportunity to raise any concerns regarding mobility, housing or, medical follow-up or medication. Moreover as part of the rota, you will do a cardiology and diabetic clinic once a week. In the cardiology clinic, you see new and follow-up patients and discuss the cases with the consultant before instigating management plans. You also get involved in elective cardioversion of patients with arrythmias and help out in pacemaker insertion once a week. Get involved in transferring patients to John Radcliffe Hospital in Oxford requiring PCI via the air ambulance transfer. In the Diabetes clinic, you will have the opportunity to see both type 1 and 2 patients in the review clinic. Here you discuss their BM diaries with their latest HBA1c readings, together with renal function, microalbumin levels in urine and their lipid profile. On top of this the rota involves F2s to head the Emergency Medical Admissions unit two evenings a week. Here you get to clerk both GP and A&E acutely unwell referrals before initiation of further treatment. Mon: 8.30am registrar-led ward round followed by ward work Tues: 8.30am registrar-led ward round, 10am pre-admission clinic and afternoon ward work Wed: 9am consultant-led ward round, ward work Thurs: 8.30am post take registrar-led ward round followed by ward work Fri: 8.30am registrar-led ward round followed by ward work On call requirements: When on call you will be covering all surgical, orthopaedic and ENT/MaxFax/ophthalmology admissions (usually between 2 and 8 a day in total). You also cover the 2 surgical and orthopaedic wards for general jobs out of hours. The on-call rota includes one week of nights and one weekend on call every nine weeks. 6 F2 - Obstetrics & Gynaecology The Obs & Gynae Department comprises 4 Consultants who provide all obstetric and gynaecology services for the island of Jersey, population approx 90,000. is closely linked with tertiary services in Southampton. The type of work to expect and learning opportunities Where the is based Clinical Supervisor(s) for the Main duties of the As an F2 you will work alongside your GPST1 colleagues on the SHO rota. You are the junior doctors in the rota and will be supervised by a Staff Grade and Consultant. During the normal working day you will be either on call for either Gynae or Obstetrics and based on Rayner Ward or Maternity respectively. If you are not on call for the wards you may be based in ante-natal clinic, day surgery unit, main theatres, assisted reproductive unit, and Gynae outpatients. In addition you also have responsibility for ward rounds and discharges of the patients under your particular consultants care. The overall educational objectives of the F2 year are to provide the trainee with the knowledge, skills and attitudes to be able to: Take a history and examine a patient Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers. They will have the opportunity to work with the consultants in outpatients clinics, delivery suites, scanning and theatre. You are also responsible for the Pre-Admission Clinic. They are expected to attend the structured teaching programmes provided by the department. The doctor will be responsible for such other specific clinical

There are interests in Epilepsy, Allergy, Asthma, and Diabetes. All pediatric and neonatal pathology is represented serves a population of 90,000. is closely linked with tertiary services in Southampton. All F2 Doctors in hospital posts will generally be ward based during the normal working day and expected to deliver the daily medical care of all the patients on their ward irrespective of specialty. Whilst on the ward the F2 will be involved with the generic clerking of patients being admitted and the ongoing care of the patients in the unit. Dr Tim Malpas The F2 doctor is responsible with other staff for the ward care of patients and the maintenance of the patient s medical record. They will have the opportunity to work with the consultants in outpatients clinics for at least one day each week, and also take responsibility for problems arising in General Paediatrics or neonatal patients on the ward. They are expected to attend the structured teaching programmes provided by the department. The doctor will be responsible for such other specific clinical duties as allocated by consultants including performing other duties in occasional emergencies
