This is the time of year for making our New Year s Resolutions. Some of the more popular resolutions are health related like losing weight, stop smoking and starting an exercise program. These resolutions are important but difficult to maintain. Now Is The Time To Make Your Professional Resolutions Now is the time to make and commit to a professional resolution. You can start small by committing to attend an educational opportunity or learning a new skill. Or, you can jump in feet first and join a committee or run for a board position in PAPAN. Any of these goals will satisfy your need to improve and grow. We celebrate PeriAnesthesia Nurses Awareness Week (PANAW) the first week of February. The theme this year is Caring Every Step of the Way. There are many ways to celebrate. Units have had luncheons, educational offerings or food drives. Let us know how you celebrated. Send the information to the newsletter editor. ASPAN s 33 rd National Conference, Dealing with Challenges Winning with Power, Practice and Purpose, is being held in Las Vegas, Nevada on April 27 through May 1, Attending this conference would be a great way to motivate and renew your passion for your profession. 3 Education Update Are you looking for an exciting challenge? PAPAN is looking for a new Education Chair! I have been the PAPAN's Education Chair since At this time, I would like to step down but stay on the committee as a consultant. I feel that we need "new blood" in this role for different ideas to serve PAPAN in the best way possible. I have thoroughly enjoyed this role working with the great friends I've made in PAPAN and hope to continue to be a part of this awesome group. The Education Chair position is very rewarding. The expectations are outlined in the PAPAN Policies & Procedures. I've also developed a Primer of timelines and contact lists to help define the duties of this position. Please me with any questions: castle2016@aol.com. Willingness to Participate forms can be found on the PAPAN Thank you to everyone who helped me over the years in this position and especially to the ones who encouraged me to "step out of the box As the saying goes, If I could do this, so can you. It Is All About Education! Nancy Glass-Education Chair We are always planning to be proactive for perianesthesia education in Pennsylvania. It is very exciting to be a part of it all! PAPAN is very fortunate to have ambitious people who are willing to volunteer their time to plan our yearly PRIDE conference. It is held each fall when it is a beautiful time of year to do a "road trip". The weekend offers a day & a half of conference with vendors (both professional & personal) and often a social time is planned for Saturday night. This year the PRIDE conference is being held in Gettysburg, PA at the Eisenhower Hotel & Convention Center, September 27-28, Brochures will be sent out to all PAPAN members.. Plan to attend! I always enjoy the road trips to attend a conference.i look forward to seeing old friends & making new ones, obtaining knowledge about our specialty, having fun and networking. It is amazing how we often share the same issues: Is it OK to give an LPN report or does it have to be an RN? Do we have to accompany every patient back to their room post-op or just certain ones? Should we document a monitor strip on every patient? How long should we keep an outpatient with a history of OSA? What is a non-licensed personnel's role in PACU? 3 Check out the following websites for contact hour information specific for Perianesthesia. (click on "Education" and see the many different methods that ASPAN has to offer) Our web site is in the process of being updated but keep checking back as there are many different offerings through PAPAN, SCPAPAN & MEAPAN (all perianesthesia groups in PA) There are many ways to get perianesthesia education and/or the contact hours for certification. ASPAN publishes the Journal of Perianethesia Nurses with many educational articles. ASPAN also provides FREE Contact Hours during PANAW. Breathline and the Pulse of PAPAN list the calendar of many educational offering. ASPAN has an OnDemand Library- Education on the Web which provides webinar modules for purchase. Host or attend an ASPAN Seminar. This will provide the host with free contact hours. The responsibility and motivation is found within yourself. No matter how experienced you are or how many years you have worked, there is always something new to learn. It is difficult to find the time, money and energy to accomplish your goals but, take some time to realize how important your goals are and find the ways to obtain them.

4 By Nancy Glass In Memorium-Theresa Lingafelt Theresa A. Lingafelt, passed away unexpectedly on January 7, 2014, at her home. She was born in Lilly, PA on October 19, Surviving is a son, Patrick and his wife Linda (Locust Grove, GA), 3 grandsons: Patrick, Zachary and Jackson, 3 brothers, 4 sisters and numerous nieces and nephews. Theresa was a 1968 graduate of Conemaugh Valley Memorial Hospital School of Nursing. She worked in critical care at Altoona Hospital before transferring into the Recovery Room around Theresa seemed to find her niche there as so many of us do. She stayed in the perianesthesia arena until she retired in She was a good friend and coworker. She always encouraged us to achieve more. Tweesa, as we fondly called her, can be described as being fun, stubborn and generous. Education as well as certification in perianesthesia was very important to Theresa. Louise, her sister, who is also a nurse at UPMC Altoona recently found Theresa s CPAN certificate dated PAPAN was also high on her list of favorite people and activities. T had spoken frequently in the beginning of 2002 about holding a PAPAN PRIDE conference in Altoona. We can do this!!!! she d insist. After a few health problems between us, in 2005, we finally got to co-chair conference together. She took care of contact hours and I worked on the hotel, social events, and door prizes. Our co-workers assisted with much needed help. Theresa was very persistent with the speakers about getting their information returned to us in a timely manner. After PRIDE, Theresa got involved in PAPAN and served on the Board of Directors. She went to every PRIDE Conference that she could and several national conferences. She even played an absent-minded anesthesiologist in a Kim Noble skit (and she was perfect for that role). Tweesa made many friends in this realm and enjoyed every time she got together with them. Theresa made us laugh A LOT! She d be the one to make sure we had cheese, crackers, other snacks and tasty beverages on our road trips. Theresa had a way to make us feel loved with her food especially her famous chicken. Whether it was because of the loss of a loved one or a celebration at the PACU Christmas party, we always found comfort in Theresa s delicious chicken. There are so many stories about her, ranging from: puka beads, Go-Go boots, getting caught with carving knives in her luggage at the airport after 911(innocently). She would always sing They Call the Wind Mariah. Our nurse manager had to practically drag her to the ED when she had symptoms of a CVA at work. She had many friends at home and at work. She was close to her family especially her son, Patrick and his wife and their 3 boys. She enjoyed the visits she made to see them. She will be missed by all who knew her she made a difference!

7 Plan to Attend the 2014 PeriAnesthesia PRIDE XXIII You are invited to beautiful historic Gettysburg for our next PAPAN PRIDE conference. Gettysburg is located in South Central, PA just north of the Mason Dixon Line. You'll delight in the rolling hills and pleasant atmosphere. Here is where the North and South met in a horrific battle that was a turning point in our Civil War. We will be hosting the conference for the first time and we want it to be memorable. You will have opportunity to tour the battlefield, perhaps walk Picketts Charge, take a ghost tour, visit any number of places of interest such as the Jenny Wade museum, the visitors center or walk through the National Cemetery and stand where President Lincoln gave his brief yet meaningful address, (one that many of us had to learn in grade school). Our 2014 PAPAN PRIDE will be held at the Eisenhower Conference Center located near the battlefield and just 2 miles from downtown. Parking is free and they will have a shuttle to take you into town. We plan to have some interesting and informative speakers to keep our practice current. Mark your calendars now for September 27 and 28, 2014! The 2014 PRIDE Planning Committee: Gloria Slaybaugh, RN, Mary Green, RN, BSN, Rhoda Hawk, RN, BSN, CPAN, Barbara Hade RN, CPAN, Brenda Rotz RN, Barbara Kyler RN, Joyce McGhee, RN, BSN, CCRN, CPAN President s message continued from page 1 Maybe Las Vegas is a little far for you to travel this year. Our annual fall conference, PeriAnesthesia PRIDE, will be held in Gettysburg September 27-28, This is a wonderful way to network, expand and update your knowledge or just enjoy what this area of the state has to offer. If learning a new skill is your professional goal, there are many opportunities for that in PAPAN. One suggestion is to write an article for the newsletter. Our Pulse of PAPAN newsletter editor is always looking for someone to write a clinical article. She would work with you to develop the article. PAPAN has many committees that are always looking for new members to get involved. A Willing to Participate application is available on our website papanonline.org or in the newsletter. Think about joining the Newsletter, Government Affairs, Education, Membership or Research Committee. We would welcome your participation and it might be the way for you to reach your professional goal. If you really want to jump in feet first, fill out a Willing to Participate Form and run for a Board of Director position. PAPAN is always looking for members to run for a board position and bring fresh perspectives and new talents. Unlike other New Year s Resolutions that you usually try to accomplish alone and often fail, making a professional resolution is so much easier. When you get involved with PAPAN the leaders of PAPAN want you to succeed and will work side by side with you. This is your year! Reach your goals! Let PAPAN help you get more involved with your profession. You will find it very rewarding and fulfilling. A special thank you to Lynn Sekeres I have had family issues this year and needed assistance with the duties of presidential obligations. Lynn has been a tremendous help as a resource, confidant and a very special friend. 7

I really enjoyed it. I remember, in the beginning, when PAPAN meetings were held in a motel room. We have come a long way. It sort of reminded me of two TV shows who are celebrating 30 years too: Jeopardy and Wheel of Fortune. When we started the organization it was sort of like Jeopardy; will it succeed? should we join ASPAN?, will nurses participate?, will it last? Now it is like a fine Wheel of Fortune; A beautiful wheel of many colors and finely tuned working organization. Congratulations on thirty wonderful and challenging years. Have a great conference and at least 30 more years. I can't wait to see what happens next with PAPAN. Miss you all. Love, Joyce Myers ( PAPAN President ) Thanks to Peggy Ellison Thanks for hosting such a great party, I mean conference. It was well organized and had a great venue and speakers. PANAW at UPMC Altoona Despite the stress of strike issues, the PACU had a cake and lunch delivered to the unit. The staff bought it themselves because they felt the need to celebrate their specialty. Nancy Glass WHAT DID YOU DO TO CELEBRATE PANAW? PANAW is a week each February for all perianesthesia nurses to celebrate our commitment to our patients and our specialty. Hospitals and departments celebrate this event in many unique ways. We have heard of pizza parties, breakfasts, poster presentations, lectures and dinners. For several years PAPAN has celebrated this event by coordinating food drives throughout the state. This year we are suggesting perianesthesia units coordinate service projects such as: food drives, collecting gloves for the homeless or pop/soda tabs for Ronald McDonald Houses. Be creative in your service project but please let us know what you did by sending a picture and description of your project to newsletter editor, Evelyn erm1136@verizon.net and PAPAN president, Kandi katdilee@aol.com. 11 COMMONWEALTH OF PENNSYLVANIA OFFICE OF THE GOVERNOR GREETINGS: It is my pleasure to join with the Pennsylvania Association of PeriAnesthesia Nurses (PAPAN) to support February 3-9, 2014 as PeriAnesthesia Nurse Awareness Week. Our nation s PeriAnesthesia nurses deserve to be recognized for their contributions to the healthcare industry. PeriAnesthesia nurses have extensive education and training and they practice in all phases of preanesthesia and postanesthesia care, ambulatory surgery and pain management. PeriAnesthesia nurses play an important role in the quality of patient care and I thank the PAPAN, and those the organization represents, for maintaining high professional standards and advancing this specialty through education and research. As Governor, and on behalf of all Pennsylvanians, I am pleased to support February 3-9, 2014 as PeriAnesthesia Nurse Awareness Week. I commend the dedicated PeriAnesthesia nurses in our Commonwealth, and throughout the nation, for their commitment to the health and welfare of patients. 12 Government Affairs Update Pennsylvania State Nurses Association (PSNA) has wide range of issues that our legislators need to address this new year. The most pressing issues are: House Bill 1603 ( Administration of Anesthesia) and 1631 ( Safe Staffing). PSNA is opposed to HB1603 because they did not have a public hearing and they question the language the use of "under supervision. At a time when other states are looking for innovative ways to respond to health care reform, PA needs to realize and utilize the full economic value of the nursing profession. House Bill 1631 (Safe Staffing) awaits a vote by the House Health Committee. This Legislation allows nurse a voice in designing a staffing plan that best fits their particular hospital unit and patient care needs. We as nurses need to be in tune of what is going on in the new year regarding issues in the health field. Please go on Harrisburg web sites to learn more about what is happening on the Hill. Find out who your State and National Representative are and get involve by , letter, or making a phone call. Some of Representative have their own website or even Facebook. Let them know nurses do care and we want our voices heard. Go to psna.org for information about Legislative Day March 31, 2014 Legislative Alerts Look for blasts to members regarding pertinent and pending legislative issues. Check the PAPAN website papanonline.org for legislative issues that need your attention. Nurses Make a Difference: Speak up Be a patient advocate and keep informed regarding nursing and healthcare issues. The legislators are not healthcare professionals. We are the experts and can make a difference! Contact your legislative representative to voice your concerns! *The role of the 211,000 nurses and as a citizen is to vote. ATTENTION: Legislative Day will be held March 31, 2014 in Harrisburg, Pa. Go to panurses.org for further information. 12

13 Research Corner Why use systematic literature reviews? Kim A. Noble, PhD, RN, CPAN ASPAN Director for Research Patient care delivery is currently undergoing a fundamental philosophical transformation: moving from an expert-based patient care foundation to a paradigm that is now evidence-based. We are moving from the bedside care focused on intuition, tradition and clinical expertise to the incorporation of empirical evidence to guide care.(1) This transformation has taken place across the working careers of a generation of working nurses and has created an acute need for a new way to appraise and value scientific knowledge. I believe this is an opportunity for nursing to define its own practice and provide the evidence to support nursing roles in our patient s safe care and positive outcomes. ASPAN has embraced the Joanna Briggs Institute (JBI) as a travel guide for this transformation. The ASPAN Representative Assembly voted to increase its membership dues in 2012 for access to the JBI database and materials. The Research Committee has developed newly revised ASPAN JBI evidencebased practice (EBP) review tools for the critique of literature. Barbara Krumbach and Kathy Daley, the EBP Strategic Work Team (SWT) Coordinators, are currently revising the Journal Club page on the ASPAN Web site to facilitate member practice and mastery of literature review and ranking using these tools. During the post-conference ASPAN Board of Directors (BOD) meeting at the 32nd ASPAN National Conference in Chicago, funding was appropriated to support the development of the JBI Training Grant Program to support ASPAN Research Committee and EBP SWT members in obtaining JBI Comprehensive Systematic Literature Review (CSLR) training. Finally, a JBI workshop was held at the Leadership Development Institute in September in Kansas City, and one was held at the Standard & Guidelines face-to- face meeting in October of this year. The goal of these ASPAN activities is to facilitate the provision of perianesthesia bedside care that is based in quality evidence. of review requires two individuals with expertise in both the critique of literature as well as the field of study to independently complete the evidence search and review to minimize bias.1 But why go to all of this trouble? Aren t there ways of completing literature reviews without all of this control? The Institute of Medicine, at the request of the Congress of the United States, has established standards for systematic reviews in their 2011 document Finding What Works in Healthcare: Standards for Systematic Reviews.(2) This document sets the standard for SLR and the development of evidence-based practice guidelines. The Unwanted Sedation Practice Recommendation (US-PR) will be a work of many ASPAN hands using the JBI CSLR protocol and processes for evidence review. As a pilot test of this new process, the US-PR will help to determine ASPAN s research vision for evidence-based documents and recommendations. REFERENCES 1. Holly C, Salmond SW, Saimbert MK. Systematic literatures reviews (SLR) incorporate a rigorous, explicit and systematic approach to the critical appraisal of current evidence. A SLR is considered to be secondary research as it provides the highest level of evidence based on the systematic, transparent, and reproducible strategy used to search, critique, evaluate and report evidence. 14 Clinical Corner Making a Difference at Pennsylvania Hospital, Philadelphia, Pennsylvania As the trend in healthcare focuses on greater preventive care, registered nurses at Pennsylvania Hospital in Philadelphia, instituted a proactive approach that ultimately improved the postoperative recovery of their patients. Erin Hellyer and Stefanie McConomy, staff nurses in the postanesthesia care unit, took notice of an increasing number of preoperative diabetic patients arriving to preadmission testing with elevated glucose levels. The problem was not isolated to any one particular type of diabetic patient. Upon inquiry, there became a growing concern that many of these patients were uncertain what medications were appropriate to take preoperatively. Hellyer and McConomy began to track the glucose levels of the presurgical patients. It wasn t that everyone was noncompliant with their medications, said McConomy. The approach was more of a safety tool for patients to have greater awareness of managing their diabetes and to be informed on the signs and symptoms related to their condition. Knowing that today s patients are presenting with greater comorbidities, these nurses were looking to diminish the likelihood of post surgical complications and the risk for readmission to the hospital. reexamined to arrive at a consensus appropriate for the management of all diabetic patients. Guidelines were established with the priority set for knowing the A1c level of all diabetic surgical patients. A suitable candidate would qualify for surgery with an A1c of 8.5 or less. This test measures the glucose in hemoglobin known as glycosalated hemoglobin. The preoperative patient must present with a glucose of 180 mg/dl or less, the day of surgery. An unacceptable glucose was treated and reexamined, while those greater than 300 mg/dl were ordered a consult with endocrinology. By revising the standards at preadmission testing for today s consumer, the significance of having an acceptable A1C level has led to healthier and informed patients being discharged from the hospital with greater concern for monitoring and maintaining their own well-being. Hellyer and McConomy received a hospital safety award for their hard work by improving preoperative patient screening and subsequently impacting the recovery and healing of their diabetic patient. The duo studied evidence based data of preoperative patients along with tracking their own diabetic patient population. Forming a multidisplinary team the nurses not only worked with preadmission testing, but also with the Chief of Endocrinology, Dr. Stephen G. Rosen. Other members included the diabetic nurse educator and those from pharmacy, anesthesia, surgery, the PACU and floor nurses. Together, preadmission guidelines were Angela Henry, RN, BA, CPAN What are you or your unit doing about Evidence Based Practice? Information needed for future newsletter articles. 15 Dianne Marino, CPAN, RN While attending a perianesthesia conference last year, a question was asked of the group of 140 nurses from across PA, "How many of you work call alone? This year in response to the same question there were less than last year. If there was just one, it would be one too many. I would like to encourage those of you working call alone by telling our story. In our 177 bed hospital, we had been doing call alone. We were doing call only on weekends. As our case volumes increased there was tralk of starting call seven days a week. This elicited a very dramatic emotional response from the PACU staff. The concerns were of safety for the patients and staff. I suggested we do something about it. I contacted our CEO to request a meeting. In preparation for the meeting we reviewed the on call policy which was last updated in We then printed out the ASPAN standards for staffing in the PACU. These standards are law in our neighboring state of NJ. In PA., not so We then formulated a policy the way we thought it should look. Are You Still Working Alone On Call? The CEO agreed to a meeting. In attendance were three PACU nurses, an anesthesiologist, the lead CRNA, the CEO and CNO. I had the old policy, new proposed policy and ASPAN staffing standards with copies for all in attendance. We presented our concerns, answered questions and told them the results of our survey we had taken. The result of the meeting was a writing of a new policy following ASPAN standards. During the care of a patient in the PACU there will be two or more professional staff members (at least one who is proficient in the care of the post anesthesia patient in Phase One). present physically in the room at all times. We were all very pleased with the outcome of our meeting. Because of our relatively small number of PACU nurses, our Phase Two nurses do call with us so that each of us usually do no more than one call per week. This has worked out very well. We also called every PACU in every hospital in our area to take an informal "mini survey". Some examples of the questions were: "What is the average number of cases your PACU cares for in an average day?" "How many bays do you have in your PACU?", "What is your staffing like?", "Do you do call alone?", "Does your on call anesthesiologist stay during the recovery period until discharge?", "Who decides whether you get called in for a case?", "Is your anesthesiologist in house at all times?". Answers were very interesting with varied results. About one half of the nurses did call alone. Schedules varied from 20 to 50 or more cases a day. The number of bays were This all happened in I spoke to Laura Kling and she suggested I write the story to encourage others. When I hear of PACU nurses still working alone, I am saddened and frightened for them. I would like to encourage you to take action in a peaceful professional way. Dianne Marino, CPAN, RN Spotlights, FYI, Updates continued from page 10 Leadership Development Institute In September, four members on the PAPAN board, Marge Jackson, Ursula Mellinger, Roberta Wywiorski and Kandi Koon, attended the Leadership Development Institute in Kansas City, Missouri to learn to better serve the members of PAPAN. The ASPAN sponsored conference dealt with learning about our leadership strengths and weaknesses. Included in the conference was information on planning a conference, establishing a good budget and succession planning. We were able to meet with ASPAN and component leaders to discuss ways to meet member s needs and use the ideas obtained to continually improve our component. 19 Region IV Repo% Laura Kling, MSN, RN, CNS, CPAN, CAPA Region IV Director ( ). January 2014-New Intentions and Resolution January is a time we all think of new intentions and resolutions; some personal and some professional. Intentions are defined as a course of action that one intends to follow; an objective!. Resolutions are the act of answering, solving, and the act of determining". ASPAN President, Twilla Shrout, has selected Dealing with Challenges: Winning with Power, Practice and Purpose as her Presidential theme. I would suggest that we also consider at the beginning of this New Year, we add Promise in the context of intentions and resolutions. Explicitly written in our organizational Envisioned Future Statement, ASPAN will be recognized as the leading association for perianesthesia education, nursing practice, standards and research. As such, ASPAN continues to provide resources, forums and opportunities in these key areas. Over 16,000 members strong, we need to individually and collectively contribute to these efforts, by lending our voices, energy, experience and expertise validated with certification. So I am asking you: What challenges have you identified? What are your intentions for 2014? What subsequent resolutions will you make? How will you actualize those promises? Do you intend to be a more active member? Perhaps increase your presence by contributing to your component or to the national organization. As professionals, we constantly assess needs and identify gaps in plans and actions. Is there one thing that we can do to meet a need? Opportunities can also present as a desire to increase your own knowledge in an area you are less sure of or are an expert in. The Specialty Practice Groups (SPGs) serve as a means to exchange knowledge and new ideas. Committee work, whether on the component or national level, is a network of partners who share the group work. Join a committee to learn from others and expand your knowledge. Membership is fluid, so new members bring great creativity. opinions as members of a professional organization, are not only sought, but expected. In April at our national conference, we will be having our national elections. The Representative Assembly will vote upon the new ASPAN Standards, Practice Recommendations and Interpretive Statements. The organizational strategic plan is discussed and vetted by your elected national officers and committee chairs. ASPAN can only be successful and responsive when you add your voice, either individually or through your representatives. Silence and apathy are not an option for professionals. It is our responsibility to contribute. So for 2014, I strongly recommend we think about our professional organizational intentions. We must purposefully determine how we wish to approach those intentions. Resolve to be ready to actively offer your opinion. Make a promise, to follow-through. Begin with small steps; garner success through positive outcomes and partnership with your perianesthesia colleagues. We must approach these challenges, as President Shrout states with power, practice and purpose. Only then, can we realize the promise of the ASPAN organizational vision. Happy 2014! References:!
