1 Lyla University Health System NURSING DEPARTMENT EDUCATION STIPEND GUIDELINES Please fllw these guidelines when submitting an applicatin fr cnsideratin f funds frm the Nursing Department Educatin Stipend Fund. Any questins, cntact Barb Hering at bhering@lumc.edu. 1. A Registered Nurse, Licensed Practical Nurse, Certified Medical Assistant r Operating Rm Technician emplyed in a benefit eligible psitin (> 0.5 FTE) fr 1 year at Lyla, including management & APN psitins, are eligible fr reimbursement fr cnference fees (up t $300), certificatin/recertificatin fees (up t $300) and presenter expenses (up t $1,000). All must be in gd standing and free f crrective actin r wrk imprvement in the past 12 mnths. There can be n ther funding surce available fr applicant. See # 3 belw fr Registry RNs. 2. All applicants, regardless f their FTE, are eligible t apply fr $300 fr certificatin/recertificatin fees. See # 3 belw fr Registry RNs. 3. Registry RNs at Levels 3, 4, 5 and 6 are eligible fr $200 per year fr certificatin/recertificatin and cnference fees. 2. Only 1 apprved applicatin per persn per year, January 1 st - December 31st. Exceptins fr presenters will be cnsidered. Yu may submit several fferings n ne applicatin, up t yur ttal eligible amunt. Request fr funds must be submitted within ne year f the activity. 3. Applicatins are accepted anytime, but will nly be reviewed in batches n a quarterly basis. Applicatin deadlines are January 15, April 15, July 15, & Octber Fr every applicatin, cmpletely fill ut Nursing Department Educatin Stipend Applicatin Frm, Applicant Ntificatin Frm and Expense Reprt Frm. Dwnlad frms at lyla.wired > Departments > Nursing > Educatin Stipend. Missing frms will result in the applicatin being returned t yu fr resubmissin the fllwing quarter. See #5, #6 and #7 belw fr additinal items that need t be submitted fr each specific educatinal activity. Submit yur applicatin t Jdy Blatchfrd, Main Hspital, Nursing Administratin, Rm 1328, by Gift Shp. If drpping applicatin ff in persn, there is a Stipend bx at the entrance t Rm Apprximately ne mnth after the applicatin deadline, yu will receive an as t the status f yur applicatin. 5. If attending a live r web-based cnference/curse, cnference fee up t $300 will be cnsidered fr reimbursement. Certificatin Review Curses are cnsidered a cnference. Submit payment receipt and CEU certificate. 6. Certificatin expenses (certificatin/recertificatin) may be reimbursed up t $300, r $200 fr Registry RNs. Submit payment receipt and certificate r letter as prf f cmpletin. 7. If presenting at a cnference (lecture r pster presentatin), fees will be cvered up t $1000 including airfare r car mileage, rm & bard and cnference fee if applicable. Only riginal receipts will be accepted. Presenter cnfirmatin letter r brchure will be accepted as prf f presenting.

2 8. Reimbursement will be granted after prf f attending activity. An Expense Reprt Frm (lyla.wired > Departments > Nursing > Educatin Stipend) will need t be filled ut and attached t EVERY applicatin fr payrll reimbursement. Indicate dllar amunt in Sectin A, OTHER EXPENSES and als in Sectin C, TOTAL EXPENSE SUMMARY; sign & date in Sectin D, CERTIFICATION. We d nt reimburse gas/car mileage, parking fees r tlls, unless yu are a presenter. The mney will appear n yur paycheck as a nn-taxable reimbursement, which may take up t 3 pay perids. Als, if yu are n lnger emplyed at Lyla, yu will NOT receive Stipend Funds, even if the qualifying activity was during yur active emplyment. 9. Please understand that we may nt be able t fully reimburse everyne in a particular review perid. Reimbursement will depend n the number f applicatins received fr a given quarter. Every effrt will be made t fairly distribute funds, with certificatin/recertificatin the pririty. T date, all applicatins have been accepted, except fr staff with disciplinary actin r incmplete applicatins.
