Beta has undoubtedly shaped my life into what it is today and has allowed me to make many lifelong friends throughout the journey of my years in Beta Club. 5 THE NATIONAL BETA CLUB Media Consent Agreement The National Beta Club takes pride in recognizing our members by highlighting their achievements in various documents and publications. Members may be featured on our website, marketing publications, training materials and recruiting initiatives that include but are not limited to newspapers, television advertisements or commercials, the Internet, brochures and social media. The National Beta Club would like your permission to inspire others by featuring your scholar. As the parent or guardian of, I herby grant The National Name of Student Beta Club and its officers, agents, field representatives and authorized media organizations permission to photograph, video or otherwise record my child and agree that such images of my child may be used by the National Beta Club in connect with its activities. I am signing this document with the clear understanding of the following items: i. The National Beta Club nor any affiliate will use the said photographs, audio or videos to receive monetary gain nor will I receive monetary gain from any reproduction of the images. ii. I will not hold National Beta responsible for any liabilities that may arise from the use of such images. 6 The National Beta Club Permission and Release of Liability Beta Leadership Camp is a program that offers student leaders an opportunity to participate in activities to improve goal-setting, communication, problem-solving, and character building. To achieve these goals, the program consist of a wide variety of recreational activities including, but not limited to, games, obstacle courses, water activities (swimming/canoeing), wood working, drawing, painting, team building activities, travel and more. I am fully aware of and acknowledge the potential risks of personal injuries associated with these activities. I hereby elect to voluntarily allow my child to participate in said activities with full knowledge that activities may be dangerous. I, hereby give permission for my child Name Parent/Guardian Name of Minor to attend National Beta Leadership Camp and to fully participate in all activities involved. I give permission for medical attention to be administered to my child by the camp staff in the event of a medical emergency. I give my consent to have my child transported to the nearest hospital emergency room where the staff has my authorization to provide any treatment, at my expense, that a physician deems necessary. I voluntarily assume full responsibility for any risks including death that may be sustained by my minor, or any loss or damage of property owned by me, as a result of attendance to camp.
