4 TEXAS TSA Event Personal Liability Release Form No conference attendee will be allowed to participate in all events as designated in the Texas Rulebook. Students will not be allowed to participate unless this form is completed and presented to the Texas TSA judges during inspection for that event. Instructions: Complete thus form and be prepared to present it at Event Check-in. This form may be photocopied. I do voluntarily authorize the Texas TSA state conference director, assistants and /or designees, student s sponsor, to administer and/or obtain routine or emergency diagnostic procedures and/or routine or emergency medical treatment for the above named person as deemed necessary in medical judgment.
