1 2018 Safety Coloring Page Contest Coloring Page Guidelines 1. The Safety Coloring Page Contest is open to any youth ages 4 to 6 years old. The contest is not limited to children or grandchildren of Farm Bureau members, but they must be a Pennsylvania Resident. 2. Only the provided coloring page will be accepted. Download available at: 3. Entries will be judged on the following: Neatness Appropriateness of coloring Appearance Coloring within the lines 4. ALL entries MUST include the following information on the back of coloring page to be considered eligible: Child s Name Phone Number Age Category Parent s Name(s) 5. Coloring Pages must be submitted to your County Women s Committee by May 1, Counties will choose ONE winner, and submit that winner to the PFB Office. Multiple entries will not be accepted. Deadline to submit county winner to PFB Office is June 1, Please mail to: PFB Office, Member Relations Division, PO Box 8736, Camp Hill, PA (Only those entries submitted with complete information, and received by June 1, will be considered eligible.) 7. Of the entries received in the PFB Office, a first AND second prize winner will be selected. The first prize winner will receive a $25 Visa gift card and the second prize winner will receive a $15 cash award. First prize winners, and their parents, will be invited to the PFB Annual Meeting on Monday, November 12, Winners will be announced in the Country Focus, and will also be displayed at Ag Progress Days and PFB Annual Meeting.

3 2018 Safety Poster Drawing Contest Safety Poster Drawing Guidelines 1. The Safety Poster Drawing Contest is open to any youth ages 7 to 12 years old. The contest is not limited to children or grandchildren of Farm Bureau members, but they must be a Pennsylvania resident. 2. Counties will select ONE winner from EACH of the following age categories: 7-9 years old years old 3. Entries will be judged on the following: Drawing must be 8 ½ x 11 inches in size - NO larger. Drawing must be in color. Drawing must include slogan of ten words, or less, which addresses the theme. Drawing must be original. Tracing is NOT allowed, and no collages or 3-D drawings will be accepted. Entrants may only win one time in each age category. 4. ALL entries MUST include the following information on the back of the drawing to be considered eligible: Child s Name Phone Number Age Category Parent s Name(s) 5. Drawings must be submitted to your County Women s Committee by May 1, Each county should choose ONE winner per age category and submit the two winners to the PFB Office. 4 2018 Safety Photo Contest Safety Photo Guidelines 1. The Safety Photo Contest is open to any youth 13 to 18 years old. The contest is not limited to children or grandchildren of Farm Bureau members, but they must be a Pennsylvania resident. 2. Counties will select a FIRST and SECOND place winner from EACH of the following age categories: years old years old 3. Photo Contest Rules: Color OR a black and white photo are acceptable Only one entry per person, per age category Photo must be taken by the entrant Photo must be 8 x 10 inches in size Photo cannot be altered / NO professional photography will be accepted Photo must have been taken within the last twelve months Photo must be related to the 2018 safety theme Entries will be judged according to quality, composition, focus, and how well the photo conveys, or relates, the 2018 Safety Theme Think Safe, Be Safe " Entrants may only win ONE time in each age category 4. ALL entries MUST have the following information taped to the back of the photo to be considered eligible: Child s name Phone number Age category Parent's name A signed photo release (should be attached) 5. Photos must be submitted to your County Women s Committee by May 1, Each county should choose TWO winners per age category and submit the four winners to the PFB Office. 5 2018 Safety Photo Contest Release form I hereby grant employees or agents of the Pennsylvania Farm Bureau (PFB), permission to use my likeness, name or images I have photographed, and submitted, for this photo contest, in print and/or electronically, without payment or any other consideration in perpetuity. I agree that Pennsylvania Farm Bureau (PFB) may use the submitted photograph of my likeness, name or images, with or without my name, and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and web content that the Pennsylvania Farm Bureau (PFB) may determine to be appropriate and lawful. My photos may also be used for my personal or professional use. I release the Pennsylvania Farm Bureau (PFB) and its affiliates, employees, agents and assigns of any and all liability related to the taking, creation, editing, handling or use of the photograph taken by me, digital, electronic or other productions or reproductions of me and/or individuals in the photograph, or my statements or actions for this contest. I am 18 years of age or older, and am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning, and impact of this release. If under the age of 18, I will have consent signed by my parent or guardian.
