9 I. EXECUTIVE SUMMARY A. Report Overview The 2016 CHAMPS Region VIII Health Center,, Turnover, and Vacancy Survey Report provides an analysis of data from Region VIII Health Center Program grantees and FQHC Look-Alikes ( Health Centers, primarily Community, Migrant, and Homeless Health Centers), enabling these organizations to determine whether their salary and benefits packages are competitive in comparison with similar organizations within the region. The report also illustrates regional workforce trends allowing Health Centers and their support organizations to monitor changing Health Center needs. Between January and March 2016 CHAMPS collaborated with an Advisory Committee of Health Center and Primary Care Association staff members to inform improvements to the survey tool, instructions, and final report (see page 10 for a list of Advisory Committee members). In Region VIII, Colorado Community Health Network (CCHN) utilized the updated tool and instructions to collect data from Colorado Health Centers. Association for Utah Community Health (AUCH), Community HealthCare Association of the Dakotas (CHAD), Montana Primary Care Association (MPCA), and Wyoming Primary Care Association (WYPCA) worked with CHAMPS to distribute the survey tool to, and collect data from, the rest of the region (see Appendix B). CHAMPS coordinated the compilation and analysis of data from all six states. Respondents Fifty-five (82%) of the 67 Health Centers in Region VIII during the spring and early summer of 2016 participated in this survey. Survey participants included: 100% of CO Health Centers 82% of MT Health Centers 100% of ND Health Centers 80% of SD Health Centers 62% of UT Health Centers 57% of WY Health Centers These 55 participating organizations will spend over $432 million on wages and almost $92 million on benefits for over 7,092 FTE employees in Contents 2016 survey participants were asked to refer to Job Description Summaries developed, reviewed, and updated by the Advisory Committee when reporting data (see page 105). All health centers utilizing the data compiled in this report are strongly encouraged to refer to these same summaries. The compiled salary data includes information for 120 total positions including over 3,060 employees in 59 administrative positions and over 4,750 employees in 61 clinical positions. The 17 survey respondents with annual budgets of less than $4 million employ 8.6% of the administrative employees and 6.3% of the clinical employees represented by this survey. The 19 survey respondents with annual budgets of $4-$10 million employ 17.3% of the administrative and 16.6% of the clinical staff. The remaining 19 survey respondents have annual budgets of more than $10 million and employ 74.3% of the administrative and 77.1% of the clinical workforce. Participants were asked to report information about additional pay separately from base pay for each employee. 16.5% of reported employees earned some sort of bonus/incentive in % earned some sort of duty-based differential pay. 2.9% were paid for overtime/extra shifts. Participants were also asked to provide details about the benefits packages offered to their employees. 98% of health centers offer some contribution to the cost of an employee health insurance plan. 98% provide some sort of retirement/pension plan. Reporting health centers provide an average of 34.4 total days off annually. Report Overview, Contents, continued The survey also collected a variety of metrics and needs relating to recruitment, retention, and additional workforce-related issues at participating health centers. Recruitment findings include: Fifty-four organizations provided information about positions that had seen challenging recruitment; all 54 participants felt one or more positions had been challenging to recruit, up from 79.6% of participants in o Health centers continue to report having more trouble recruiting clinical positions than administrative positions. Fifty-four participants provided information on vacant positions for executive-level, provider, and mid-level staff; the average length of recruitment for positions that had a finite recruitment time (those that were not continuously recruited) was 1.84 months for executive-level, 4.54 months for provider staff, and 1.67 months for mid-level staff. o For the first time in 2016, participants were asked to identify which vacancy was considered of highest priority; 70% of the highest priority vacancies were for medical providers, most often Physicians (54%) and Nurse Practitioners (20%). The most often cited group of recruitment tools that health centers would like to implement involved some sort of monetary increase for staff (bonuses, salary increases, funds for continuing education, etc.). Retention findings include: Fifty-four organizations participated in a Years of Service (YOS) survey by providing dates of hire for their 7,785 staff members (3,052 administrative and 4,733 clinical). o 5,388 (69.2%) of these have been employed for less than five years (68.6% of the administrative and 69.6% of the clinical staff). o 207 (2.7%) have been employed for 20+ years (2.8% of the administrative and 2.6% of the clinical staff). o The average YOS for all reported staff was 4.71 years, down from the average of 5.02 years in Retention findings, continued: Fifty-three participants provided information about positions that had exhibited challenging turnover; 87% felt one or more positions had seen challenging turnover, up from 70% in o Health centers continue to report more trouble retaining support positions than leadership and provider/mid-level positions. Fifty-two organizations participated in a region-wide Turnover Survey of 2015 data, providing average numbers of employees and separations for various staff types; the average turnover rate for all staff at Region VIII Health Centers in 2015 was 25.6%, up from 20.6% in The most often cited group of retention tools that health centers would like to implement was being able to offer improved salaries via raises, bonuses, incentive pay, etc. Additional workforce findings include: Fifty-four organizations responded to additional workforcerelated questions: o 81.1% are planning to offer some sort of salary increase between May 1, 2016 and April 31, 2017, down from 85.7% that reported planning to offer an increase in the 2014 survey. o 68.5% know that they currently employ one or more veterans within their organization, up from 53.1% in Just over 5% of participants were unable to provide this data. o Participants reported that between 2% and 90% of their employees were racial/ethnic minorities (39% on average). Just over 5% of participants were unable to provide this data. o Participants reported that between 7% and 90% of their employees held college or advanced degrees (49% on average). Just over 7% of participants were unable to provide this data. 15 A. and Data Overview II. SALARY AND BENEFITS INFORMATION Introduction The portions of the 2016 survey relating to salary and benefits were organized in four key ways: 1. Job Description Summaries: The survey instructions included Job Description Summaries for all titles to ensure health centers reported employee data appropriately. These summaries are included in Appendix A. Job Description Summaries and Index of Titles (page 105) to ensure health centers are referring to the most appropriate titles when utilizing the final salary and benefits information contained in this report. 2. Annual Data: Health centers were asked to report separate figures for current annual base salary and current annual additional pay for each employee in 2016 (before 2014, health centers reported one combined salary figure including both base and additional pay). The salary figures included in this report s main salary/benefits tables (pages 17-70) are an analysis of base salary data only; please refer to the Comparison of Recently Published Data: Total Compensation table (page 71) and Additional Pay section (page 72) for a separate analysis of additional pay data. 3. Annual Data: Health centers were asked to report annual employer contributions to health, dental, vision, life, and disability insurance, and retirement/pension plans only in 2016 (before 2014, health centers reported employer contributions to these items as well as employer FICA payments). The benefits figures provided in this report s main salary/benefits tables (pages 17-63) are an analysis of employer contributions to insurance/retirement programs only. 4. Packages: Health centers provided additional details about their benefits programs relating to insurance, retirement, time off, and additional incentives. Please refer to the Packages section (page 74) for an analysis of this additional benefits information. Highlights for All and Data* The largest groups of employees were: Medical Assistant, with Credentials (770), Receptionist/Front Desk/Appt. Clerk (629), Registered Nurse (437), Dental Assistant (306), and Physician Assistant (303) The five highest paid positions were, on average: Physician Other ($284,742), Physician OB/GYN ($231,040), Psychiatrist ($198,434), Medical Director/CMO ($189,516), and Physician Family Practice, without OB ($183,405) The five lowest paid positions were, on average: Janitor/Custodian/Housekeeping ($23,676), Nursing Aide/CNA ($27,132), Lab Assistant ($27,892), Receptionist/Front Desk/Appt. Clerk ($28,169), and Call Center Representative ($28,443) *All salaries and benefits have been equalized to 1.0 FTE (Full-Time Equivalent). At least five positions must be reported in order to report average salary and benefits levels for any given title. Positions with fewer than five reported figures were not included in this overview. See page 14 for information about Other titles CHAMPS Region VIII Health Center,, Turnover, and Vacancy Survey Report - 14 The positions receiving benefits as highest percentage of salary were, on average: Nursing Aide/CNA (38.39%), Physician Resident (35.86%), Physician Other (35.85%), Ultrasound Technician (34.86%), and Phlebotomist (34.73%) The positions receiving benefits as lowest percentage of salary were, on average: Lab Assistant (7.67%), Other Director (Clinical) (9.89%), Physician OB/GYN (10.41%), Other Clinical (11.57%), and Nursing Supervisor (13.26%) The highest and lowest paid Leadership Team positions were, on average: Medical Director/CMO ($189,516) and Human Resources Director ($75,232) Salaries in Region VIII are slightly lower than the Region X and national medians for health centers, and lower than the medians paid in hospitals and private practices.
16 A. and Data Overview, continued Highlights by Annual Budget Ranges* Each survey year, the annual revenue information submitted by participating organizations is reviewed to determine the most useful small, medium, and large budget ranges. Previous to 2016, these breakdown levels were less than $1.5 million, $1.5-$7 million, and more than $7 million annually. In 2016, these budget ranges were updated to ensure a relatively equal distribution of participating health centers within each level. The 17 health centers with annual budgets of less than $4 million employ 7.2% of the staff in 85 of the 120 total positions presented in this report. Largest groups of employees: Receptionist/Front Desk/Appt. Clerk (59), Registered Nurse (44), Outreach/Enrollment Staff (43), Nurse Practitioner (35), and Medical Assistant, without Credentials (31) Highest paid positions, on average: Medical Director/CMO ($195,344), Physician Family Practice, without OB ($190,886), Dentist ($150,928), Pharmacist ($116,809), and Physician Assistant ($97,393) The 19 health centers with annual budgets of $4-$10 million employ 16.9% of the staff in 109 of the 120 total positions presented in this report. Largest groups of employees: Receptionist/Front Desk/Appt. Clerk (130), Registered Nurse (100), Medical Assistant, with Credentials (69), Nurse Practitioner (68), and Medical Assistant, without Credentials (67) Highest paid positions, on average: Physician Family Practice, without OB ($182,579), Medical Director/CMO ($165,394), Dentist ($137,654), Dental Director/CDO ($131,412), and Executive Director/CEO ($117,656) The 19 health centers with annual budgets of more than $10 million employ 75.9% of the staff in all of the 120 total positions presented in this report. Largest groups of employees: Medical Assistant, with Credentials (680), Receptionist/Front Desk/Appt.
