Dear Colleagues, Looking back by Dr Ting Sea Leong Looking back over past one year since new committee took of ce, it has been a hectic year. Besides usual CME jointly organised with Perak MMA, we had numerous activities and funtions. Community service under Dr Kamalanathan Raju has been active. In sports Dr T Kumar is actively promoting sports among doctors. CME under Dr N Prasad managed to bring OA symposium to Taiping. A joint venture with postgraduate medical education society of Hospital Raja Permaisuri Bainun, Ipoh successfully organised 7th Asean Conference on Primary Health Care. A CPR cum management of venomous bites was organised by Dr Harbaksh Singh. Dr Kamalanathan and Dr Ting attended several TWG meetings. Dr Subramanian Suppiah and Major Dr Jeyaratnam also attended some TWG workshops. Two tours to India, a trip to Taman Negara and a cruise in Penang was organised. A commmittee meeting was held during cruise. Over one hundred members are in arrears. Some are uncontactable. It is duty of every member to update secretariat of any address changes, telephone/handphone number, address and to settle ir annual subscription promptly. Of late, widening gap between income and expenditure is worrying and needs to be addressed urgently to avoid depleting our reserve fund. By creating a new class of members with membership exemption is going to furr decrease income. To offset this is to have minimal defaulter rates, increase new memberships or increase subscription as a last resort. New income generating activities or ventures should also be looked into. A reduction in committee members meeting allowance need also be considered. We are fortunate and I must thank Ipoh Specialist Hospital for free use of its meeting room during committee meetings. In time, this may change and in future we may not be so lucky. turn to page 2 1

2 from page 1 PMPS is already seventy-two years old. By time we celebrate its 80th anniversary she deserves her own building. Towards this venture a fund should be set up as soon as we possibly can. Due to tak nak 1care action by FPMPAM and citizen coalition for health care and vigorous oppositions in internet, MOH has shifted its stand and vehemently denied that 1care is at an advanced stage of gestation. Watch out for 1care when BN wins 13th General Election. Without doubt FPMPAM is doing its best ghting for professional independance, ethical practice and autonomy of private practice. Sad to say that many states have yet to form its own society and in states that have, we welcome all doctors and dentists into fold, for toger we stand and divided we fall. We are contributing to JIHC fund which provides legal assistance to our members involved in disputes with MCOs (managed care organisations) and private hospitals. The introduction of 1Malaysia clinic has affected patient load of most private clinics. Dato Sri Mohd Najib has promised to establish more nationwide. This is good news for public but not for GPs. Private practice is now at a crossroad. Should 1care be introduced, it will be a new experience. In view of possible transformations and uncertainties, we must be united. PMPS 2012 Informal Night PMPS 2012 Informal Night was held at Riverfront Hotel on 25th March We had overwhelming and encouraging response from members and guests. Dr Ting, our President rst presented a speech on 1care, followed by prize-giving ceremony. This year, members did not have an opportunity to perform on karaoke. However, occasion was lled with laughter as several members entertained everyone with witty jokes. We hope more members will turn up for future PMPS Informal Night events as this is time when y can unwind and catch up with fellow colleagues. Organising Chairman, Dr Kamalanathan: Due to ineffective PA system, most members could not hear President's speech during informal night. It is reproduced here. President, Dr Ting: This is indeed a night to remember. With over one hundred members, families and friends joining in fellowship for informal night, I think this a record. Everytime something is organised, committee will be worried that turnout will not be good. Tonight, you have proven us wrong. This is a night for all medical practitioners including dentists to take a rest and meet fellow practitioners from or towns, renew friendships and make new acquaintances. Once a year we must meet, to have fellowship and discuss issues of mutual interest or just to catch up. A very important issue this year is 1care. All of us must stay united to face challenges ahead. I hope that in future, Informal Night can be cherished to unite all of us, something to look forward to every year as a homecoming to meet our brors and sisters so that it become a tradition for members not to miss. I must thank everyone for coming and hope you have good fellowship. Enjoy yourself, forget about your worries and be merry. Thank you again. 2

3 by Dr Ting Sea Leong Health care and education are two basic needs which our goverment has a duty to provide for its citizens. Sensing that a change is due soon, our duty as health care providers is to inform public of bene ts and disadvantages if implemented. As proof that goverment is contemplating to revamp health care system, let me quote from Concept Paper MOH 1Care For 1Malaysia, Restructuring The Malaysian Health System, 11 August 2009 page 28 article 57: With consensus from economic council and approval from cabinet, MOH, with assistance from various partner agencies and stakeholders, will n undertake furr systematic planning towards development of a full blueprint for 1Care national health system within a two-year timeframe. Upon development, phase implemention of programme will be introduced with full evaluation and monitoring to ensure that objectives of 1Care proposal are achieved. Next quote is from Berita MMA, February 2012 issue, where President Dr Mary Cardosa wrote in page 8: So we are participating in technical working groups (TWGs) which have been carrying out workshop discussions since over a year ago, to work out details of implementation. Therefore, unless 1Care is aborted it will soon be delivered to Parliament for endorsement to become law. Knowing voting trend in BN-dominated parliament, it will become law. Let me also quote from Albert Einstein, The world is a dangerous place not because of people who do evil, but because of good people who look on and do nothing about it. 1CARE single entity called 1Care which is claimed to be superior. But our system is still good. Let me quote from concept paper page 1 article 2: Malaysia s health care system is acknowledged internationally as a successful modern government regulated health system that provides effective health services. FACILITIES AND PERSONNEL Everyone in health care industry are involved: doctors, dentists, pharmacists, nurses, hospitals, labs, paramedical staffs. RESTRUCTURING WHY? Quote from concept paper page 1: 1) However, current and future challenges will affect sustainability and relevance of system. This is main reason, meaning government may not have means to subsidise health care in future. Due to scal mismanagement, leakages and corruption, our government has a de cit budget every year. Just look at yearly unfavourable auditorgeneral s report and reports in newspaper regarding corruption and illegal money out ow. As a result, our government is tempted to pass a recurrent increasing expenditure in health care to its citizen. The or factors are: 2) HEALTH CARE COSTS Increases due to medical innovations, new technologies and drugs, and high end treatments are costly. 3) WORLD WIDE HEALTH FINANCING Most upper middle and high income countries have a national health insurance in place. Malaysia being in upper middle income is desirous to ape m. It is a concept of MOH to integrate and restructure our dichotomous (private-public) health care into a 3

4 4) GOVERNMENT VERSUS OUT OF POCKET EXPENDITURE IN HEALTH CARE By year 2004, se two are equal. The latter has progressively outstripped former. The consensus is that this may become unsustainable. 5) MONEY SPINNER By introducing health care nancing, a continuous huge sum of money is available. 6) CORRECTING IMBALANCES By introducing 1Care, resources like human resources (specialist) and facilities can be bridged. 7) URBAN AND RURAL DISPARITY In primary care this can be taken care of when opening of new clinics is 1care controlled. 8) UNIFY PEOPLE Helping one anor. Rich helps poor, healthy helps sick, employed helps unemployed. 9) DATA COLLECTION With computerisation under 1Care, a complete data on illness is possible for future planning and research. 10) W.H.O. RECOMMENDATION Advises to all governments in its manual Health System Financing Path to Universal Coverage. It concludes that to achieve universal coverage, only way is by pooling and prepayment orwise poor cannot afford. FINANCING 1CARE & NHFA Financing is through a special fund pooled from various sources and operated by National Healthcare Financing Authority (NHFA). Reimbursement to doctors and pharmacists etc will be from this fund. Through an Act of Parliament, it becomes mandatory for all Malaysians to subscribe towards this fund with no avenue to opt out. Contribution will come from employer, employee, government and or sources like self-employed. For 2007, estimated health care expenditure with 5% administrative charge is RM44.23 billion. Now, it could be even more. Administrative charges can be shifted upwards. As percentage of household income, it is 9.5%. The gure is quoted from concept paper page 23. QUOTA All Malaysians must register with a doctor of choice. When quota is reached, must n move to next doctor. When more clinics open, quota will reduce. CONSULTATION AND DISPENSING Consultation and dispensing is separated. Doctors act as gatekeeper to hospital and specialist care. SPECTRUM OF DISEASE COVERED It is a comprehensive cover from womb to tomb. Some special diseases may not be covered. IS 1CARE DESIRABLE? 1) Cost is high. 9.5% of total income (from personal, employer, government contributions) with no rebate even if you do not use service. 2) May not have a choice of personal doctor, specialist or hospital. 3) Medicines may be inferior from a list of essential drugs. If you prefer patented drugs you pay difference in cost. 4) So far, no single country has developed an ideal national health care system. 5) Very important: yearly over 44 billion ringgit will be collected. Is our money in safe hands? OUTCOME GOOD AND BAD Bene cial Outcome 1) Universal coverage 2) Financial risk protection or safety net in catastrophic illness 3) Quality care 4) Accessible 5) Equitable 6) Affordable 7) Improved ef ciency and performance 8) Responsive Adverse Outcome 1) Increase cost of health care a. doctors consultation fee increases b. with monopoly, every item can expect increase in price. 2) Inconvenient to patient 3) Most important, it will spark an in ation. An employer will raise price of his goods after 4

5 helping employees to pay part of his insurance premium leading to a domino effect. 4) Cost of living goes up, having to pay health insurance and in ation. ALTERNATIVE TO 1CARE IS TO RETAIN AND IMPROVE THE CURRENT SYSTEM. PUBLIC SECTOR 1) Reduce subsidy by charging a reasonable fee for outpatient and inpatient care on those who could afford. This will also reduce workload. 2) Reduce leakages. Improve ef ciency and performance. 3) Retain staff by better working condition, increase pay, fairer promotion prospect. 4) Inject funds for development including manpower. PRIVATE SECTOR To cap hospital fees (doctors fees are already capped). CONCLUSION There may be some bene ts but very serious is cost of living will go up and people may be denied own choice of doctors and hospitals. Challenges faced by doctors under 1Care A better system is promised so it is for doctors to deliver by: 1) Upgrading skills and quali cation in family medicine to specialist level if possible 2) Participate in CME 3) Provide emergency care 4) Follow clinical practice guidelines 5) Computerisation of clinic 6) Clinic improvement and staff upgrading. Clinic accreditation every few years 7) 24-hr service? with help from nearby clinics 8) Patient management under audit As a setback, you lose professional independence as an employee. Also under employment, is re a mandatory retirement age for doctors? TA`NAK 1CARE CAMPAIGN by Dr Kamalanathan AG Raju Community Service Chairman In view of impending 1Care proposal being soon implemented, FPMPAM (Federation Private Medical Practitioners Association Malaysia) in collaboration with Citizen Coalition For Health Care started a Ta`Nak 1Care campaign nationwide. This is to promote public awareness and education on 1Care and its effects on Rakyat. In support of this campaign, PMPS organised three 1Care public forums namely: Feb 2012: at Rayan Cultural Hall from 4:30 a.m. to 7:15 p.m. Response was about attendees Feb 2012: Citizens Coalition for Health Care organised one with cooperation of PMPS (providing speakers only) in Taiping TC Hall Mar 2012: third Public Forum arranged at Chung Hwa Association, Kg Koh, Manjong. Response was 90 people. 3. Sad to say despite best of efforts, response from public was not as anticipated. Worse, to say, from members of PMPS whose livelihood, future and self-respect will be affected. On same subject, Dr Ting also gave a talk to Sitiawan Rotary Club on 22 Feb 2012, and to Manjung Doctors on 25 Feb Dr Chan Choong Guan who gave a public forum on Diabetes Mellitus at Pioneer Methodist Church on 04 Mar 2012 also gave a talk on 1Care in Mandarin and its effects on public. Due to all se publicity generated by FPMPAM and its member associates in Selangor/KL, Penang, Perak, Sarawak and Pahang, and intense opposition on Ta`Nak 1Care website by Public, MOH has now gone and going on roadshows to explain that 1Care is still under discussion with its stakeholders and nothing has been nalised and that it is still in study phase to nd out best system for health care in Malaysia. To also give our members a Government s point of view of 1Care, PMPS had arranged a dialogue on 25th March 2012 with Dr Hj Noordin Salleh, Deputy Pengarah in Planning & Implementation Unit of MOH and also in charge of 1Care Secretariat, after CME talk and prior to PMPS AGM in Kinta Riverfront Hotel. We are eargerly waiting to continue our dialogue in TWG meetings to see if re will be any change in Government s proposal for 1Care. 5

6 Tour to India October 2011 Day 1 New Delhi Arrival in New Delhi. Sight seeing to Lotus Temple, President s residence, Parliament followed by shopping Day 2 Srinagar From Delhi to Srinagar by ight, by road to Dall Lake n by boat (shikira) to boathouse. Its an unique experience staying in boathouse. Night was very cold. Wooden furniture are hand crafted. In afternoon, we visited Shalimar and Nashit garden. Shopping followed. At night several petty traders came to sell ir wares at boathouse. Day 3 Gulmarg By road to hill station followed by poney ride for two kilometers and cable car (gondola) up to mid-station. Snow covered most of ground. The wear was better after we descended and could admire beauty of Gulmarg. Petty traders came again at night. kilometers. Arrival at hotel and late dinner at 11 pm. Day 5 Taj Mahal One of seven wonders of world. It is a symbol of love. Shopping for souvenirs Taj Mahal and or marble products. Afternoon was spent at Agra Fort where Shah Jehan was imprisoned by his son until his death. Day 6 Jaipur Between Agra and Jaipur, visited Fort Fatephur Sikri. Many marble and granite carving factories are located here. Passed through pink city and had a glimpse of Hawal Mahal. In afternoon, visited Jantar Mantar and city palace. Day 7 Amer Fort Saw water palace along way to fort. Took elephant ride up slope to fort. Coming down was by jeep. A very impressive fort with a great wall around it. Shopping at Rajastan Textile Development Cooperation. Could not enter Nahargarh Fort as it was past closing time when we arrived. One could view beautiful sunset here. The whole Jaipur town could be seen here. At night, it is spectacular with city s twinkling lights. Day 8 Back to New Dehli Saw Kutub Minar. Rest of time was window-shopping at Select City Walk (one of Asia s biggest). Could not cover or destinations. An extra day is needed to see more places. A farewell dinner with proprietor of in-bound tour agency. Each of us got a present from him. Then to airport for ight home. Day 9 Safe arrival with fond memories of passage to India. Day 4 Agra Morning boat ride was accompanied by petty traders trying ir luck at sale. Back to New Delhi. From here to Agra was a nightmare. We were caught in exodus from capital for depavali. It took us eight hours to cover two hundred 6

7 2nd Tour to India Who have not heard of Taj Mahal? Hence when a second trip to north India was organised by our beloved President Dr Ting for those who missed rst trip in October 2011, many jumped at chance (13 to be precise). The rst day on arrival at New Delhi was spent on recovering from ight as visiting Qutub Minar (earliest Muslim architecture complex in India) and Lotus Temple (Bahai house of worship). Everyone was eager for following day s trip to fabled Kashmir. Enthusiasm for Kashmir next day was dampened somewhat on hearing overnight temperature in capital Srinagar was 10 C! Still, we had to go. The magic began even before plane touched down, with snow-capped mountain peaks whetting our appetite for more. The landscape was rar bare, considering it was early spring. We were told by locals that summer will bring luxuriant owering (a sly hint to make anor trip in future!) However, sight of famous Dal Lake was balm for eyes. We checked into two adjacent boat houses after crossing lake in a shifaka (boat). A brief lunch and it was back across lake to see Mughal Gardens in process of replanting for summer. It was n back to boathouses for a meal in a dining room ambience straight out of Olde England chandeliers, attentive house captain waiting for our every request and curry mutton to die for. A tful sleep (even with electric blankets and warmer than minus 10) was scant preparation for a trip to Gulmarg and snow elds reminiscent of Swiss Alps. It was one of highlights of tour. A few feet of snow covering ground, gorgeously clear skies enabled taking some of best photographs of tour for memories. The previous group rode donkeys to cable car terminal from bus dropoff point. We rode in sleds pulled by locals. It was disconcerting to have humans as beasts of burdens but we consoled ourselves we were providing employment. A long trip followed next day: ight back to New Delhi, 5-hour bus to Agra and everyone dreaming of Shah Jahan as we went into slumber at near midnight. What can one say about one of most famous buildings in world? I suppose it is because of this that opinions are divided, even polarised. Overexpectations and spin-doctored romantic notions will forever make visitors viewing it eir as slabs of marble or an oriental Camelot. Best to leave it for individuals to judge. 10 COMMUNITY SERVICE It has been a busy year! Chairman, Dr Kamalanathan AG Raju reports PMPS in collaboration with Sri Satya Sai Baba Centres of Perak and State Health & Dental Department, has been very active. About 5 medical camps, 2 public forums, 1 medical camp by PMPS Sitiawan Branch in association with Pantai Hospital Ipoh, and nally a public forum to be arranged on 19 February 2012 regarding 1Care. In all camps, in addition to height, weight, BMI, blood pressure screening, random blood glucose screening, eye screening, dental screening, health exhibition, papsmear & breast examination and deworming of children, doctors screened and gave advice on healthy lifestyles, diet, on DM & HT control. Most camps saw Dr Yek Sing Chee, Dr Ting Sea Leong, Dr Poovathy, Dr Mariamah Nair, Dr P Rajeswaran, Dr Kamalanathan, Dr Prem, Dr S Subramaniam (Eye Consultant) being regulars. Ors who participated when can were Dr Amarjeet Kaur, Dr Lai Thian Seong, Dr R Jeyaratnam, Dr Liew San Foi, Dr Agnes Heng (Skin), and doctors from MOH: Dr Rekha (ENT-HBI), Dr Letchuman (Taiping Hospital), Dr Shantini and Dr Subashini. 1. The rst medical camp was held on 20th March 2011 at Batu 14, Tapah-Cameron Highlands road. It was for an Orang Asli Settlement and neighbouring village. About 130 people were screened by Health & Dental team from Tapah Hospital. 2. 7th August 2011, saw PMPS members and Sai Group in collaboration with Utar students doing service at Sungai Siput Selatan new village for ir members and surrounding Orang Asli camp. About 180 people including Utar students were screened. Dietician, Miss Mok from Fatimah Hospital gave a talk on nutrition and healthy diet. She also did counseling for few obese students and N/V residents. Doctors had a nice durian feast in addition to buying from Orang Asli th August 2011, saw us doing a medical camp at Buntong Indian Recreation Club. Done toger with NGOs for Merdeka Day. About 150 people were screened by 4 GPs, 2 Specialists, Dr Letchuman and Dr Jayabalan (O&G, Pantai Hospital Ipoh) and health team from Klinik Kesihatan Buntong. 4. 2nd October 2011, saw PMPS members travelling by 4-wheel drive and on back of a small lorry over winding and hilly terrain with narrow roads and bridges to Orang Asli camp at Kg Ulu Geroh, km from Gopeng. This camp was organised by Legal Aid Ipoh Bar Council, Amnesty International and or NGOs in relation to law awareness week by Bar Council. While screening about 150 Orang Asli members, we were entertained by Orang Asli musicians, dancers, blow pipe demo, hunters showing ir traps and food. Dr S F Liew and Dr Agnes Heng had ir hands full. Or doctors were Dr Rekha, Dr Amarjeet, Dr Jeyam, Dr Yek, Dr Ting, Dr Prem, Dr Shantini, Dr Nathan, and Health and dental team from Klinik Kesihatan Gopeng th November 2011, saw us leaving early morning at 6:30 am for Simpang Empat, Hutan Melintang where anor medical camp in collaboration with Utar students was held in new village re.
