4 Clinical Skills The Emergency Directorate (ED) offers the student nurse excellent opportunities to learn new and improve on previously learnt clinical skills. It is anticipated that much of what is learnt whilst in ED will be transferable across all areas of Paediatric, Mental Health and Adult nursing. The whole emphasis of the placement is for the student to enjoy their time with us and use the nursing and medical staff as a resource to learn and develop different nursing skills. Transferable Skills: Measurement and recording of systemic and neurological observations including pulse oxymetry, GCS and how to interpret findings. Assessing, planning, implementing and evaluating care, including general hygiene, oral care, nutrition and hydration Being frequently involved with the resuscitation procedure both adult and paediatric. Airway management, suction and administration of oxygen therapy; 12 lead ECG s, recording and cardiac monitoring Learning to interpret blood results and their significance upon treatment. Aseptic technique including catheterisation Infection control including safe disposal of sharps, clinical and non-clinical waste Pain Management Drug administration (orally, inhalation, subcutaneous, intra-muscular and suppositories) Management and administration of IV antibiotics/fluids/blood, including cannula site management Management of infusion equipment Blood glucose testing and recording of, including Preparation and administration of insulin Safe moving and handling of clients Collection and correct labeling of specimens Contribution to the Health and Safety of the environment for staff and patients Multi Agency referral systems Rapid admission and assessment of patients including safe discharge/transfer Specialist Skills Learning to adapt to high volume of patient admission and discharge/transfer Triage Wound Assessment & Closure Techniques, including sutures, steristips and glue closure Plaster of Paris application Toxicology management Trauma resuscitations Limb Injury Assessment Child Protection Issues
5 Helicopter Reception/Patient retrieval (RPH only) Learning Opportunities Available: The Emergency Department sees approximately 100,000 patients a year across both Preston and Chorley sites. Preston sees approximately 90,000 of those patients. ED offers many learning opportunities including: You will have the opportunity to work within the following areas of the department: Majors: This area comprises stretcher bays and assessment rooms. (One of the assessment rooms is for the care of children and another for the assessment of patients with psychiatric complaints) Minors: This area comprises individual bays, two of which are designed for children Triage: The aim of the Emergency Directorate is to respond to all patients who attend. We believe that the patient is an individual who should be treated holistically and is considered the main focus of our attention. We believe that life threatening or potentially life-threatening problems are our first priority, thus we operate a dedicated systematic nursing assessment, known as a triage system. It is situated in the main waiting room for the assessment of all patients arriving either by public or their own transport. Clinic: A follow up clinic is run from until approx 12 midday Monday to Friday except bank holidays. Resuscitation Room: Consisting of bays with overhead x-ray gantry. One bay is dedicated for Paediatric use with state of the art paediatric guidelines on a touch screen monitor. EMDU: 24 Bedded assessment area for the use of medical, surgical and ED assessment patients, offers the means of fast pace of patient assessment treatment and transfer to appropriate wards. You will have the opportunity to assist with the retrieval of patients from the helicopter paramedics. The Emergency Department at Preston has a concreted helipad approved by the civil aviation authority. The Coastguard
6 search and rescue service along with the Police and Ambulance service helicopters bring patients to the department You will learn how to rapidly admit, assess and undertake systemic observation of patients, notifying nursing and medical staff of results. Being involved with the admission procedure is an ideal opportunity to learn about your patients and develop an effective therapeutic relationship. You will learn about the importance of minimising trolley waiting times within ED Being able to assist medics perform a variety of medical procedures, including ECG s, lumbar puncture, insertion on of chest drains, venepuncture and cannulation, cardio-pulmonary resuscitation, insertion of central lines. You will also learn how to use and interpret monitoring equipment, alerting staff to any abnormalities You will learn about airway management and the importance of oxygen and nebuliser therapy; including how to care for patients with chest injuries and acute and chronic pulmonary disease. You will contribute to the multi-disciplinary team, including medics, Clinical Nurse Manager/Modern Matron, Emergency Nurse Practitioners, Advance Nurse Practitioners, Trauma Department Technicians, Bed Managers and other agencies to affect nursing intervention, management and evaluation. The team aims to provide competent, compassionate and effective care to patients and their relatives/ carers. Decisions are based on research based practice or best evidence, and practice is audited to ensure that it is clinically effective. At some time during your placement you may get chance to shadow the sisters/charge nurses, staff nurses, ENP s and HCA s all of whom fulfill unique clinical roles within the department. This MDT working will develop your communication skills, which can also be enhanced by contact with many different members of the public. You will be given the opportunity to develop your organisational skills and
7 learn about time management, whilst developing your interpersonal skills, by helping to take responsibility of group of patients, following triage, for a span of duty. You will participate in drug administration and develop an understanding of the most common medicines used within this department. You will learn how to effectively manage pain, especially cardiac pain, using intramuscular and oral medication, thrombolysis and IV infusion pumps. You will also learn about the importance of management of intravenous fluid replacement including care of cannula site. You will develop your IT skills, by obtaining electronic lab results and learn how interpret and realise the significance of blood results and how they impact upon treatment You will develop a greater understanding around issues of consent. You will develop an understanding of different aspects of wound management including major and minor injuries, splints, slings, bandages, tubigrip etc. This will include managing infection control and safe disposal of clinical waste. Should any casualties need to be admitted to this Emergency Department following a chemical incident department also has the facilities to arrange decontamination of affected patients. You will be given the opportunity to assist with the assessment of injured and/or ill children. ED staff follows their philosophy that children are unique individuals with specific physical, developmental and psychosocial needs. They aim to provide holistic care for the child and family in an environment suitable for their needs within the Emergency Department. A named, experienced nurse working in partnership with the child, family or carer and other health care professionals where necessary plans the care. You may be given the opportunity to attend any educational sessions given by any member of staff and the specialist nurses/tutor.. The professional development of staff is recognized as being vital in order to equip them with the necessary skills they require in order to provide a high quality standard of care to patients and carers. You may be involved with treating patients who then die. This will give you the opportunity to assist with the support and counseling given to the relatives/significant others. You will learn how to safely discharge patients to the care of ward/department/theatre staff for continuation of care. This will include assisting with a comprehensive hand-over of treatment and care management to date. You may have the opportunity to be involved in the safe transfer of patients to the Critical Care Unit and Coronary Care Unit You will receive training in Manual Handling, bandaging, plastering, safe disposal of clinical waste and Health & Safety issues.
8 Visits that can be arranged. The Emergency Department does offer the student an opportunity to work alongside Paramedics with Northwest Ambulance Service. The opportunity to observe the Coroners Officer. Radiography department including x-ray, CT and MRI scanners. Eg) Health & Safety policy, manual handling, Marsden manual etc. Student induction pack Infection Control policy Moving and Handling policy Consent to treatment Health and Safety Policy Trolley waiting times Notice boards for new developments Journals, books and teaching packages in the staff training room Shift Times start and finish at (½ hour lunch break) start and finish at 2115 (½ hour lunch break) start and finish at (40min lunch break) Students will be allocated a mentor and associate mentor, that they can plan their rota around by self-rostering. We are flexible and shifts can be negotiated. The Emergency Department provides opportunities to review shift patterns in order to accommodate people s home circumstances. It is suggested that you do consider working nights at some time during your allocation, as this will give you the best overall view of ED work.

We believe that we can empower patients by working in partnership with them, providing information and facilitating informed decision making with regard to their individual needs. Accident prevention, providing information for the reduction of slips, trips and falls Alcohol consumption advice Smoking cessation and nicotine replacement therapy advice Helping to educate patients on correct mobilisation and exercising following injury and application of bandages, splints, plaster etc Deep Venous Thrombosis prevention Reduction of cholesterol and dietary advice Medication advice Educating relatives and carer s in the above issues and in future rehabilitation of the client, providing information leaflets, booklets and contact numbers in case of further enquiry or problems Referrals to multi-agencies as necessary Last updated: Monday, June 19, 2017
