We were seeking those who have made a significant impact on heath service poicy, heathcare deivery or cinica advances in the ast 12 months or who were ikey to do so in the next 12 months. Our ong ist was created from March to May through a combination of a pubic nomination process and input from an expert pane. Our judges reviewed the ist and added their suggestions, deciding on the fina 100 at a judging evening hed in May. During this session, they aso ranked the top 20 cinica eaders among this 100. Judges considered the foowing criteria in coming to their decisions: Poitica infuence To what extent has the individua infuenced heathcare poicy nationay in the run up to the eection? To what extent is he or she ikey to infuence poicy in the coming 12 months? Service transformation To what extent has the individua infuenced improvements in services as envisaged in the NHS Five Year Forward View or are ikey to do so in the coming 12 months? Innovation To what extent has the individua used his or her cinica eadership to infuence innovative soutions in heath and care? Infuence in these areas is highighted in the suppement and our onine graphic using the coour coded symbos beow. Judges who were nominated excuded themseves from the seection process. However, in the interest of producing a credibe ist, judges who met the criteria as cinica eaders were considered by HSJ editor Aastair McLean after the judging process. 4 1 Professor Sir Bruce Keogh MEDICAL DIRECTOR, NHS ENGLAND HSJ Cinica Leaders is now in its third year and it is the third year that Sir Bruce has sat atop it. No one woud argue against this former cardiac surgeon being named Engand s most important cinica eader, other than perhaps the man himsef. This represents both Sir Bruce s incontrovertibe infuence, and the modesty with which he wieds it. Seven day services and emergency care have remained a key focus for Sir Bruce this year. Promises on the former found their way into the Conservative manifesto, and have become a theme of many post-victory speeches by David Cameron. On the atter, the new care modes outined in the NHS Five Year Forward View drew heaviy on the recommendations of the care review ed by Sir Bruce. This year, he wi take on another review: this time of the professiona codes of practice for doctors and nurses. Instigated by Jeremy Hunt, it is in part a reaction to the report on faiings at University Hospitas of Morecambe Bay Foundation Trust. Mr Hunt wi be optimistic that the review wi have an impact. In February, statistica anaysis by heathcare information body Dr Foster suggested that the Keogh specia measures trusts have since seen asting fas in their mortaity rates. 2 Professor Sir Mike Richards CHIEF INSPECTOR OF HOSPITALS, CARE QUALITY COMMISSION No one woud argue against him being Engand s most important eader, other than perhaps the man himsef Sir Mike Richards and Dame Say Davies (3) swap paces in this year s HSJ Cinica Leaders, with the chief inspector of hospitas moving up a sot to number two. The new inspection mode is now 18 months od, and by the end of the year Sir Mike hopes that a hospitas wi have been through the regime receiving individua ratings for urgent and emergency services, medica care, surgery, critica care, services for chidren and young peope, end of ife care and outpatients and diagnostic imaging. He contends that this constitutes a better view of NHS quaity than ever before, and certainy the process is more rigorous. There is aso no doubt over his determination to improve the inspection regime. Consistency remains a probem though one about which Sir Mike is refreshingy open. He used an HSJ webinar to ca on trusts to hep address the issue by reeasing their top cinicians to ead CQC inspections. The introduction of the fit and proper person test ast November introduces another responsibiity for Sir Mike. As is characteristic, it is one he pans to discharge sensitivey he tod HSJ he did not think it right to name individuas at a triage stage but with a cear focus on quaity. He argued that the shortage of NHS senior managers was no reason not to pursue action against those about whom concerns are raised. 3 Professor Dame Say Davies CHIEF MEDICAL OFFICER FOR ENGLAND, DEPARTMENT OF HEALTH Dame Say can surey take some of the credit for the recent, beated appearance of menta heath at the top of the poitica agenda. It is no coincidence that the chief medica officer s most recent annua report, pubished in ate 2014, focused on the ongtime Cinderea service. In it, she caed for more funding and much greater emphasis on menta heath and webeing. The report aso suggested the introduction of waiting time targets. Less than a year ater and such targets are now in pace for access to taking therapies and for peope who have experienced a first episode of psychosis. It is evidence of the extent to which, when Dame Say speaks, peope sit up and pay attention. For the past two years, she has emphasised her support of the creation of babies with DNA from two women and one man dubbed three-parent IVF as an option for women with mitochondria disease. 5 4 Professor Steve Fied CHIEF INSPECTOR OF gps, CARE QuALITY COMMISSION Heathcare is a word in which many firsts are caimed, often inaccuratey. But Professor Fied can take credit for something genuiney new: the nationwide process to inspect the standards of care provided by famiy doctors. Its fu aunch has not been without its teething probems. Amost as soon as the first inspection reports were pubished in November, there were concerns about inaccuracies. One in 10 5 Dr Sarah Woaston GP practices were assigned the highest risk rating, but the reveation of mistakes in the cacuations ed to an apoogy. The use of banding has now been dropped, and Professor Fied promises to speak not of risk but of variation. What was perhaps more reveaing was the one inspected practice decared to be providing outstanding services across the board. Saford Heath Matters runs three surgeries in Greater Manchester, prioritises prevention, and runs an extended set of services. By highighting it, Professor Fied payed an important roe in mapping out a post-forward view mode of primary care. Expect him to continue to do so. MP FOR TOTNES AND CHAIR OF HOuSE OF COMMONS HEALTH SELECT COMMITTEE Sarah Woaston first broke into the top ranks of HSJ Cinica Leaders ast year. Back then we wrote that if she continues her spirit of taking a stand, expect to see her here again next year. She has. Dr Woaston has aways insisted that she is not a triba poitician. Her continuing wiingness to pubicy speak out when she disagrees with her party eader constitutes compeing proof. In recent months, she passionatey opposed David Cameron s suggestion that those who cannot work due to obesity or substance misuse shoud have their benefits cut if they refuse treatment. She suggested the pan was iega and unworkabe. The change argey represents the importance of this former midwife as the NHS responds to the findings of the Morecambe Bay investigation report. Pubished in March, Dr Bi Kirkup s review concuded that serious faiures in cinica care ed to unnecessary deaths of mothers and babies. It aso spoke of a dominant group of midwives whose over zeaous pursuit of natura chidbirth at any cost sometimes ed to unsafe care. That means Professor Warwick wi need to work hard to prevent members of her profession becoming demonised in the way nurses did post-mid Staffordshire. Morae is aready chaenging. Midwives went on strike for the first time in the history of the coege ast October, protesting over pay, and there have ong been concerns over staffing eves. Many eyes wi be on the RCM and on Professor Warwick to hep pick the profession back up. simon PoTTs on great eaders The move towards a 24/7 cinicay ed sustainabe NHS aong with the impact of the 2012 Heath and Socia Care Act is pacing greater demands upon our cinica eaders. In the current cimate, to deiver what s right for patients, we need to optimise the opportunities for cinica eaders to respond with pioneering, contemporary and innovative soutions whie creating and shaping the environments for change to happen. Providers, once sovereign organisations, now operate on a changing andscape made more compex by increased reguation, changing commissioning priorities, workforce chaenges, risk management and financia constraints. Future sustainabiity of the NHS is firmy dependent upon the quaity of its future cinica eadership and whoe systems working together. 6 7 Dr Cifford Mann President, Roya Coege of Emergency Medicine Cifford Mann enters the top 20 ranks for the first time, a representation of how centra he now is in the discussion over emergency care. During a winter in which A&E performance against targets was the worst on record, his was a powerfu voice defending his coeagues. He has argued that the key issue was not A&E departments but inadequate fow through hospitas, which saps morae and eads to probems with staff retention. The risk to patient care in the future wi not be whether they ve met the four hour target or missed it by five minutes, it s whether there are experienced cinicians to see them in the emergency department when they first attend, he tod HSJ in Apri. He has proved adept at paying the media game a post-christmas suggestion that poice crack down on binge drinking to reduce pressures on A&E was a perfecty timed headine-grabber but the judges recognised him as doing a huge amount to transform emergency care. As A&E departments begin to approach winter he wi be caed on to do a huge amount more. 8 Dr Kate Granger consutant in edery medicine, Mid Yorkshire Hospitas Trust Dr Granger has aready been named one of HSJ s Most Inspirationa Women, a Socia Media Pioneer, and a member of 2014 s HSJ100. Now she joins our Cinica Leaders and right at the top of the ist. It is difficut to overstate the impact she has had. Diagnosed with a rare and termina cancer in 2011, she soon started writing about her experiences of moving to the other side from doctor to patient. One of the major themes is the difference sma acts of compassion can make. Her #heomynameis campaign which encourages staff to introduce themseves and their roe to patients buit on that thought, and has become a truy nationa movement embraced by nationa poitica figures. Later in 2015, NHS Empoyers wi run the Kate Granger Awards for Compassionate Care for the second year. Despite her impact, Dr Granger remains a humbe presence. When she got the etter announcing her MBE, she said: I thought I d been chosen for jury service. In two years at NHS Engand, she is widey recognised as having more or ess singe-handedy kept menta heath at the top of the agenda. Her tenacity is widey admired and if poiticians interest might have been a mixed bessing, the announcement of increased menta heath funding and the Menta Heath Taskforce is surey unreserved good news. Few woud have envied him as he sought to navigate the A&E crisis, which was rarey out of the headines this winter. Her guiding aim, she says, is to improve heath and heathcare internationay and to hep doctors make better decisions wherever they are in the word. She is not afraid to speak up about sometimes controversia issues. He has couped his expanation of the issue with suggested soutions, embodied in the urgent and emergency care review and the forward view. Having been director of nursing at the Department of Heath and Pubic Heath Engand, in Apri she took on the new roe of chief nurse for PHE and the tite of chief government adviser on pubic heath nursing and midwifery. It was a move wecomed by our sister tite Nursing Times, which argued that giving nursing a strong voice in this arena can bring about tangibe, positive change in the heath of the nation, and ensure that NHS services face ess demand in the future.
