2 Pocono Health System Excelling to New Heights POCONO HEALTH SYSTEM BOARD OF DIRECTORS In 1915, a new era in healthcare was born in Monroe County with the opening of General Hospital in East Stroudsburg. Today, more than a hundred years later, we are Pocono Health System (PHS), and Pocono Medical Center (PMC) and the healthcare services we provide have grown to provide the most advanced and integrated care to the patients in our region. In the past year, PHS s numerous achievements include the opening of our new West End Healthcare Center in Brodheadsville, where outstanding primary care doctors, physician specialists, and key services are all located in one building for ease of access to the best care for our patients. In addition, PMC s Dale and Frances Hughes Cancer Center was named a Certified Quality Breast Center of Excellence by the National Quality Measures for Breast Centers (NQMBC), and our Bartonsville Healthcare Center was designated a Breast Imaging Center of Excellence by the American College of Radiology, paving the way as a regional leader in women s health with advanced technologies including 3D mammography. PMC is a leader in emergency care. Featuring a Level III Trauma Center in one of the busiest Emergency Departments in Pennsylvania, our hospital offers services for all ages, including pediatrics. This year, PMC s Emergency Department is undergoing a $3.8 million renovation that includes additional treatment areas to further enhance our patients overall experience. Among our greatest accomplishments this year, U.S. News & World Report named PMC in the top 10 percent of hospitals in Pennsylvania for the first time ever, a reflection of PHS s commitment to provide excellent care, close to home. Similarly, PMC s ESSA Heart and Vascular Institute ranked among the High Performing Hospitals in both Heart Failure and Heart Bypass Surgery. PMC also received accreditation as a Primary Stroke Care Center of Excellence by the Joint Commission, and the American Heart Association/American Stroke Association awarded PMC with the Stroke Bronze Level Quality Achievement Award. Looking to 2016 and beyond, our promise to our community is that PHS will soar to even greater heights. 3 Excelling to Improve Health Collaboration During the past year, PHS established many new partnerships to offer patients greater access to clinical excellence. PMC s Dale and Frances Hughes Cancer Center and Lehigh Valley Health Network are working together to enhance access to radiation oncology services. In addition, the collaboration between PHS and Thomas Jefferson University Hospital gives our patients fighting ovarian and cervical cancers access to the most technologically advanced gynecologic surgical therapies, and through a partnership with the State University of New York (SUNY), PHS now brings specially-trained oncologists to our community to treat conditions such as prostate and bladder cancers through minimally invasive robotic urological surgery. POCONO HEALTH SYSTEM Expanding Services, Bringing New Technologies to the Region Improved Technology PHS added the Stereotactic Upright 3D Breast Biopsy System to detect and treat breast cancer. This advanced 3D screening instrument allows patients to remain in an upright position while being tested. Also, PHS enhanced traditional mammography with two of the most advanced 3D breast ultrasound units that allow physicians to detect early-stage cancer with pinpoint accuracy, before cancer cells have spread to other parts of the body. New this year, our low-dose CT AIDER allows physicians to get detailed x-rays of early-stage cancers while reducing radiation exposure by 50 percent. The Dale and Frances Hughes Cancer Center was named a Certified Quality Breast Center of Excellence by the National Quality Measures for Breast Centers (NQMBC). Growth PHS opened its new, state-of-the-art West End Healthcare Center in 2015, two new medical office locations in the Stroudsburg area, and one in Mountainhome. In addition, PHS opened a new Immediate Care Center, our fourth in Monroe County providing urgent, non-life threatening care. Plus, PHS has already begun development on a new ambulatory healthcare center in East Stroudsburg. Perhaps most notably, plans are underway for our new hospital in Tannersville. New Physicians This year, 22 new doctors and advanced practitioners joined PHS, increasing our provider network by 25 percent. In response to increasing demands in our community for primary care doctors, PHS welcomed seven family and internal medicine physicians. New physician specialists include cardiothoracic surgeons; an endocrinologist; medical oncologist; neonatologist; neurologist; pulmonologist; and a radiation oncologist. In 2015, PHS also expanded the team with new clinical psychologists, nurse practitioners, and physician assistants. Bartonsville Healthcare Center was designated a Breast Imaging Center of Excellence by the American College of Radiology. Advanced Services PHS is the first and only healthcare system in the region to provide advanced services for wound care. Led by vascular surgeons Dr. David Meir-Levi and Dr. Eric Wilson, our Advanced Wound Care Center at the Bartonsville Healthcare Center has treated more than 1,200 patients with Hyperbaric Oxygen Therapy, allowing patients with chronic wounds to heal faster. Saving Lives In one of the busiest emergency departments in Pennsylvania, PHS s Level III Trauma Center treated more than 73 thousand patients in the past year, and expansion and renovation to our Mattioli Emergency Department is already underway. Additionally, through PHS s stroke telemedicine program in our Emergency Department, patients across the region get the most high-tech and effective stroke treatment available anywhere. In 2015, PHS opened its new West End Healthcare Center in Brodheadsville. This 30,000-square-foot facility provides a patient-centered experience for medical care, disease prevention, and chronic disease management, all under one roof. 3

4 Our Quality Achievements PHS is proud of its many distinctions in the past year that demonstrate our commitment to quality and safety for our patients. Some of these include: U.S. News & World Report ranked PMC among the top 10 percent of all hospitals in Pennsylvania, and as a High Performing Hospital for Heart Failure and Heart Bypass Surgery. PMC was accredited as a Chest Pain Center of Excellence by the Society of Cardiovascular Patient Care. This program recognizes centers offering the best practices and highest standards of care. PMC received Advanced Certification for Primary Stroke Centers and Accreditation as a Primary Stroke Care Center of Excellence by the Joint Commission for following best practices for stroke care. In addition, the American Heart Association/American Stroke Association awarded PMC with the Stroke Bronze Level Quality Achievement Award for following their quality performance measures for stroke treatment. Quality Breast Center of Excellence! PMC s Dale and Frances Hughes Cancer Center was named a Certified Quality Breast Center of Excellence by the National Quality Measures for Breast Centers (NQMBC). PHS s Bartonsville Healthcare Center was designated a Breast Imaging Center of Excellence by the American College of Radiology. PHS earned the Joint Commission s Gold Seal of Approval for Hospital Accreditation by demonstrating continuous compliance with its performance standards. William Rutula Research Award A landmark infection control research study led by Charles K. Herman, MD, Chief of Surgery at PHS, received the prestigious William Rutula Research Award. Hospitals & Health Networks named PHS one of HealthCare s Most Wired for consistently adopting health information technology to improve patient care. Our People PHS is proud to announce that it s once again been named one of the Top 150 Great Places to Work in Healthcare by Becker s Healthcare. With more than two thousand employees, 209 active medical staff members, and close to 500 volunteers, we strive to create an environment that brings out the best in each of us and those we serve. In order to motivate our team and ensure we work together to offer safe, compassionate care, this year PHS developed the P.R.I.D.E. Program, which stands for Privacy, Respect, Involvement, Dignity, and Engagement. Each month, PMC employees who exemplify these principles are given a Service Excellence Award. By calling upon the values represented in the PRIDE acronym, all PHS team members aim to serve our patients with kindness and compassion. 4 The PHS Board of Directors, led by Chairman W. Andrew Worthington, continues to identify new opportunities to ensure our health system s continued success well into the future and to offer the best healthcare for the Pocono region and its nearly 170,000 residents.

5 PHS Community Benefit: Moving Forward Together In keeping with our Mission to provide excellent care, close to home, PHS continues to initiate a variety of community benefit programs and partner with community organizations that are helping to improve the health and well-being of the people in the Pocono community. Improving Community Health In the past year for our community, PHS invested $35,519,959 in community benefit, community health education, professional and patient education, and community partnerships. This includes a total of 374 hours of community service by PHS employees; 173 families served by our Nurse Family Partnership; and 64,763 hours of service by our 492 committed volunteers. Additionally, PHS sponsored more than a hundred community events in the Pocono community in 2015 that s an event one out of every three days during the year. PHS s various endeavors ensure that we develop programs targeting identified needs: PHS unveiled its new Healthy Living Program, designed to support people in their efforts to adopt a healthier lifestyle. In this innovative program, participants benefit from education, screenings, and healthy lifestyle programs. Led by Musa Tangoren, MD, an anesthesiologist at PMC, our Biggest Winner Healthy Weight Lifestyle Program gives people the support they need to adopt a healthy lifestyle through motivational speakers, weekly weigh-ins, and access to registered dieticians and other medical professionals at PHS. To help patients access healthcare more efficiently, this year PHS launched an expanded access Call Center with a toll-free number, 866-PMC-DOCS. The Hospital and Health System Association of Pennsylvania, in partnership with the Gift of Life Donor Program, recognized PMC with its Platinum-Level Achiever Award. PMC was one of only 12 hospitals in Pennsylvania to receive this award that recognized us as a leader in building public awareness about the ongoing need for organ donors. A total of 400 participants in the first two Biggest Winner Challenges lost more than five thousand pounds. The program is a PMC Healthy Living Series program. Pennsylvania State Senator Lisa Baker recognized PMC in a citation for providing medical care and support to the Pennsylvania State Police, first responders, and police K-9s during the manhunt for accused criminal Eric Frein. Through the Parks for Good Living Program, PHS has joined with three regional park commissions to encourage people to incorporate the power of nature in their lives. For the second consecutive year, PMC provided medical care and support to the American Red Cross Run for the Red Marathon. Education PHS enhanced emergency medical service education and training by hiring an EMS liaison and educator, certified EMT instructors, and offering classes in community-based settings. 6 Access to Care PHS recognizes that many people in Monroe County and surrounding areas are underinsured or uninsured, and we remain committed to helping to remove financial barriers so that every member of our community has access to excellent healthcare. To do this while providing outstanding care for our patients, PHS is always exploring more efficient ways of delivering care. This includes working closely with the physicians on our medical staff in the areas of wellness and prevention as well as early detection. For instance, throughout the year our Dale and Frances Hughes Cancer Center offers many cancer screenings with our physicians and advanced practitioners. These screenings are free and open to the community. This year, PHS donated $112,202 to Nurse Family Partnership, a program where specially-trained nurses visit expectant mothers in their homes until the time their children are two years old. The goals of the program are to foster healthy lifestyle habits and good parenting skills while offering career counseling to these incomeeligible families in Monroe County. The Valerie M. Hodge Dental Clinic provides primary dental care, basic restorations, preventative care, and oral hygiene education for children from uninsured or underinsured families. Last year, more than 2,500 children benefitted from the care supported by the Valerie M. Hodge Memorial Endowment Fund. PHS is also dedicated to providing care for some of our most vulnerable patients. Responding to the needs of our community, PHS expanded our Level III Neonatal Intensive Care Unit (NICU) to twice its size, adding more beds to accommodate premature infants and those born with serious illnesses. PHS is proud to have the most robust and comprehensive team of Providers and PMC Physician Associates Network in the region. 8 Pocono Health Foundation The Pocono Health Foundation (PHF) assists in positioning Pocono Health System as the leader in healthcare in Monroe County. With funding from local businesses, fundraising groups, and caring individuals, PHS is able to continue serving the Pocono region with excellent care, close to home. Through the philanthropic work at the Foundation, families in our community can access care traditionally available only in larger cities. In the past several years, and continuing to present day, PHF has made significant contributions to the ESSA Heart and Vascular Institute, the Mattioli Emergency Center, Neonatal Intensive Care Unit, Trauma Center, the Dale and Frances Hughes Cancer Center, Bartonsville Healthcare Center, Pocono Ambulatory Surgery Center, and the West End Healthcare Center. Also, PHF annually awards grants that support health and human service programs as well as students pursuing a career in healthcare. In celebration of our 100th year, PHF awarded more than $100,000 in grants and scholarships to community organizations this year. Cramer, Esquire as its 13th Gold Medal Award recipient for his significant contributions to the advancement of healthcare at PHS. The first Gold Medal Award recipient since 2007, Mr. Cramer served as chairman of PHF and was a founding board member. He also served as the chairman of the Dale and Frances Hughes Cancer Center capital campaign which raised more than $26 million for PHF. The Auxiliary of the Pocono Medical Center held its 49th Charity Ball in November at Kalahari Resorts and Conventions at Pocono Manor. The annual Charity Ball is just one of the many highly anticipated community events that the Pocono Health Foundation, along with its Board of Directors and the Auxiliary of the Pocono Medical Center, hosts each year.
