4 Executive summary This report sets out the findings of a project titled Qualifications Supply Analysis - The New Zealand Health Sector. This project was jointly sponsored by Gillian Durham, Deputy Director General of The Ministry of Health (MOH) and Ann Clark, General Manager of the Tertiary Education Commission (TEC). It was undertaken between August 2003 and July The Clinical Training Agency, DHBNZ and tertiary education providers were key contributors to the project. The purpose of the project was to provide a comprehensive insight into: the nature, volume and mix of tertiary education sector s current delivery of health sector qualifications, and related clinical training; and, how these qualifications are funded by various government agencies and students and others. The project included consideration of the full spectrum of human health qualifications, including mental health, sports & fitness and complementary health disciplines. The year 2002 was selected as the focus of analysis as it was the most recent year for which a near complete set of information was available. The insight that the project sought to provide was not definitive. It was expected that it would raise many more questions and elicit more information that would add to our knowledge of health qualifications. The particular questions that the project sought to address, and the answers to those questions are set out in brief below: What range and volume of health qualifications are currently provided by the tertiary education sector? The New Zealand tertiary education sector provided qualifications spanning 2 major occupational classes in health. There were 350 different qualifications offered in There were 490 instances that these qualifications were delivered. 2 Where volatility between years was apparent, some smoothing was applied. Clinical Training data was drawn from the 2002/2003 financial year. 2 The number of instances equals the total number of different qualifications multiplied by the total number of providers that delivered the qualifications. 4

5 Where and by whom are these qualifications provided? In 2002 these 350 different qualifications were provided by 60 Tertiary Education Organisations (TEOs) comprising 8 Universities, 20 Polytechnics, 3 Wananga, 28 Private Training Establishments (PTEs) and College of Education. These providers represented approximately 0 % of active tertiary education providers, which accessed government funding in For what health qualifications is clinical training provided and how is it funded? Explicit funding for clinical training was provided by the MOH via the Clinical Training Agency (CTA). The bulk of this investment in clinical training was directed toward medical, but some clinical training was also funded for Dentistry, Nursing, Pharmacy, Mental Health Workers and other health professionals. DHBs indirectly funded clinical training through activities such as the provision of clinical supervision. How is the cost 3 of these qualifications shared between the Crown, the learner and others? The Crown met around 78% of the cost of tertiary health qualifications through the investment of the TEC, the CTA and District Health Boards (DHBs). Learners contributed the balance of 22% in the form of tuition fees. The private sector contributed a small amount, and work to identify this sum is continuing. For those learners who received clinical training, their contribution as a percentage of the total cost of the qualification was considerably lower. How is the cost to the Crown shared between various Crown funders? The TEC provided around two thirds of the total Crown investment in health qualifications. The CTA provided around one third of the total Crown investment in heath qualifications. It is estimated that DHBs contributed approximately about 5% of the total Crown investment in the funding of health qualifications. 3 Throughout this report the term cost means the price paid by Crown funders and /or others to Tertiary Education Providers. Note that this is different from the cost incurred by tertiary education providers in delivering qualifications. The latter is unknown. 5

6 What is the total cost to Crown funders of health qualifications, on an absolute annual and per graduate basis? The TEC invested $9.2 million of funding in health qualifications. The Clinical Training Agency invested $86.6 million in clinical training. It is estimated that DHBs contribute approximately $5 million toward the cost of health qualifications. Background The needs of New Zealand health consumers are rapidly changing. This change is due to the combined impacts of: consumer demographics service delivery models technology consumer expectations. There are considerable risks to New Zealand should a misalignment develop between the needs of health consumers and the skills and competencies of the health workforce. These risks might include increased morbidity and mortality, poor consumer satisfaction, shortfalls in consumer education and poor use of best practice health technologies. These may result in reduced efficiency and effectiveness, increased costs and diminished public confidence. The Minister of Health, the MOH, and DHBs have a number of health sector workforce initiatives to ensure that the competencies of the health work force develop and keep pace with the changing needs of New Zealand s health consumers. These include: The Health Workforce Advisory Committee; The DHBNZ Workforce Action Plan; and, The Health Practitioners Competence Assurance Act Central to the issue of the skills and competencies of the health sector workforce is the Crown s investment in training and education. In 2003 officials of TEC and Ministry of Health identified value in obtaining an insight into the nature, volume and mix of health sector qualifications being currently produced by the tertiary education sector in New Zealand. 6

7 The Tertiary Education Commission, a Crown entity established in January 2003, is responsible inter alia for the funding of tertiary education and training in New Zealand. The TEC is responsible for: Implementing the Government s Tertiary Education Strategy 2005/7 and the Statement of Tertiary Education Priorities; Allocating $.9 billion to tertiary education organisations according to a new integrated funding framework; Building the capability of tertiary education and training to contribute to national economic and social goals; Advising the Government on policies, priorities and sectoral performance; Negotiating a new system of Charters and Profiles to steer the tertiary education sector. In 2003 the Ministry and the TEC signed Terms of Reference for the project titled Qualifications Supply Analysis - The New Zealand Health Sector. This is the first substantial project by TEC whereby sector-specific analysis has been undertaken by the TEC with the intention of informing key stakeholders and ultimately facilitating the steering of the tertiary education sector, through instruments such as Charters and Profiles. Purpose and scope The purpose of the project was to provide a comprehensive insight into: the nature, volume and mix of the tertiary education sector s current delivery of health sector qualifications, and related clinical training; and, how these qualifications were funded by various government agencies and students and others. Thus the scope of this project was intentionally narrow, being limited to Qualification Supply Analysis. In effect, it was intended to provide a stocktake of the qualifications produced by the tertiary sector at the most recent date for which a full data set was available. It was not the intention of the project to undertake what might be described as workforce planning for the health sector, primarily because the value of workforce planning at a micro level is questionable due to: the rapid changes in the health sector as described in the first paragraph of this paper; and, the complex dynamics involved in the supply of labour. 7

This further work may include: Workforce demand analysis: examining the likely configuration of the future health workforce, having regard to consumer demographics, new service delivery models, new technologies and emerging consumer expectations Workforce supply analysis: consideration of all possible sources of the mix of skilled professionals needed within the future health workforce Qualification demand analysis: consideration, at a macro level, of the volume and mix of qualifications necessary to be produced in New Zealand. Qualification review: reviewing the structure and content of existing qualifications to improve their fit with the skill requirements of the future health workforce. Steering: the stakeholders and the TEC engaging with the tertiary education sector to align qualification supply with future qualification demand, primarily through the process of profile negotiation, but also, where appropriate, by the use of other steering tools. 8

9 The work described above, and its relationship to qualifications supply analysis is shown diagrammatically below: Workforce Supply Analysis Workforce Demand Analysis Qualification Supply Analysis Qualification Demand Analysis Qualification Review Steering Methodology Approach The project methodology was designed to source, analyse and logically present information the current supply of qualifications for nominated health workforce areas, including the clinical training component required for some qualifications. Qualifications were classified into one of four groups. These four groups are: Certificate/Diploma Degree Postgraduate Certificate/Diploma Postgraduate Degree. 9

10 The project methodology broadly involved: The sourcing of all relevant data concerning health related qualifications. The grouping of the data by qualifications. The linking of qualifications to one of the following 2 major occupational groups: o Medicine - includes medical practitioners, general practitioners, gynaecologists and obstetricians o Dentistry includes dentists, dental therapists, dental assistants and dental technicians o Nursing o Midwifery o Ancillary/Support Workers includes health assistants and caregivers o Pharmacy includes pharmacists and pharmaceutical assistants o Allied Health Professionals includes physiotherapists, occupational therapists, osteopaths, podiatrists, chiropractors, dieticians, nutritionists, optometrists, medical laboratory technicians, medical radiation technologists, ambulance officers, public health workers, health associate professionals and other health associate professionals o Mental Health Workers includes mental health workers, counsellors, psychotherapists and psychologists o Social Workers includes social workers and child care workers o Health Administrators includes health management and informatics o Sport /Fitness/Recreation includes, among others, sport coaching, sport and fitness, physical education, recreation and leisure studies o Complementary/Alternative Medicine includes, among others, aromatherapy, allied therapies, massage, reflexology, naturopathy and acupuncture. The construction of a model showing the linkages between qualifications (e.g. students would first complete Bachelor of Health Science (first year Medicine), then go on to study Bachelor of Medicine). Extension of the analysis to include the clinical training received posttertiary education. The Research & Analysis team at the TEC constructing the analytic model. The linking of qualifications to occupational groups required input from MOH personnel. The source information and the mapping of qualifications to occupations was verified by the relevant TEOs. 0

11 Data validation The project team identified 60 different TEOs that offered health related qualifications in Of the 60 TEOs, there were 28 PTEs, 20 Polytechnics, 8 Universities, 3 Wananga and College of Education. All 60 providers were sent a letter to verify the accuracy and completeness of the data. The verification process involved: Confirming that all the qualifications listed in the table were in fact offered by the TEO in 2002; Identifying any health related qualifications that were offered by the TEO in 2002 that were not included in the table; Confirming that the qualifications were mapped to the correct occupational group; and, Confirming that the total EFTS, completed students, subsidy and fees (if indicated) for each qualification were correct (in this instance within + or 5% of the actual data). Presentation The findings of the project are presented in this report and its appendices. These comprise: Summarised findings, set out in the body of this report; and, A set of tables providing greater detail. These are included as Appendix to this report. Veracity of data In considering the findings of this project, the following points should be kept in mind: Figures were drawn from the year 2002 there may have been some changes since that time TEO data was drawn from the SDR 4. The SDR inevitably has some inherent gaps and errors. 5 The possible impact of these gaps and errors was mitigated by the data validation process described above Actual students fees information was provided by the TEOs. Where actual fees data was not available, fees were estimated using the 2004 Fee and Course Cost Maxima (FCCM). The FCCM are the maximum fees and course costs TEOs can charge students to retain approval for government funding. 4 An electronic collection of learner and learner tuition information, similar to Health s Minimum Data Set. 5 The TEC is giving priority to working with the MOE and TEOs to improve the quality of the SDR, as it will become an increasingly important data source for projects such as this.

12 The CTA data related to the financial year July June 2003, rather than the academic/calendar year All CTA figures were GST inclusive. The DHB investment was an estimate and might be very conservative. Notwithstanding the points above, the project team believes that the findings of the project are fit for purpose. 13 Throughout these findings, occupations were primarily aggregated at the level of 2 major occupational classes. This classification was mostly based on the New Zealand Standard Classification of upation 999 (NZSCO99). In areas where the major occupational class was not included in NZSCO99 the project team allocated the category. In some parts of the report however, the findings look at a lower level of aggregation by occupation, and in such cases the tables are titled upational Class rather than Major upational Class. More detailed information can be found in the Powerpoint Slides and Data Table that form Appendices and 2 respectively to this report. Analysis of Qualifications. This further work will occur in three phases, these being: Phase I: Update of the qualification supply analysis This will involve the TEOs that provide health qualifications together with other informed stakeholders (including the MOE) confirming that the qualification supply analysis accurately portrays the status quo. It is intended that the qualification supply analysis will be updated and republished to coincide with progress in Phases II and III. 22

23 Phase II: Understanding the Need: The objective of Phase II will be to equip the Ministry of Health and DHBNZ with information with which to engage tertiary providers in a meaningful and effective dialogue about future health sector tertiary education and training needs. This will be achieved by assembling a comprehensive set of information about the likely configuration of the future health work force and the associated needs for tertiary education and training Phase III: Achieving Alignment with Future Need: The objective of Phase III of the project will be to bring about change in the provision of health sector qualifications so that they are aligned with expected future needs. This will be achieved by effective engagement with providers of tertiary health education and training. 23
