Unless arranged otherwise with a specific faculty member, please plan to use the Angel system to communicate with faculty about all course related issues. Course faculty will respond to your s Monday through Friday within 48 hours. s sent after noon on Friday (through Sunday) may not be addressed until Monday. Course Description NUR 820/850 focuses on the advanced assessment component of the nursing process in order to describe the health status of patients. The emphasis of the course is on the development of advanced health history and physical assessment skills across the lifespan. Advanced assessment involves the collection of comprehensive subjective and objective data for practice as a family or adult advanced practiced nurse. The student s use of interviewing techniques, communication, interpersonal skills and psychomotor skills will be refined. Emphasis is placed on interpretation of data for the purpose of differentiating normal variations from normal and recognizing actual and potential health problems. The problem-oriented system of organizing and recording data is covered. Course content includes discussion of the impact of the cultural diversity of patients and ethical issues related to the assessment process. 3
5 Course Objectives Upon completion of this course the student will: 1. Demonstrate appropriate communication skills for data collection with varied populations. 2. Demonstrate comprehensive health history and physical assessment skills. 3. Discuss cultural variations when collecting assessment data. 4. Demonstrate comprehensive documentation of assessment data. 5. Apply selected assessment skills in a community-based clinical experience. Instructional Method This hybrid course is taught utilizing online modules via the ANGEL or Evolve websites, assigned readings, on-campus workshops, and 20 hours of a precepted clinical experience. As adult learners, it is expected that students will do relevant reading and study prior to the oncampus workshops and clinical experiences. Faculty directed readings and web links will be provided for specific content areas. The on-campus workshops are mandatory. If you are unable to attend a session due to an emergency situation, you must notify the course chair and your clinical faculty. Make-up for this time will be arranged at the discretion of your clinical faculty in coordination with the course chair. Clinical days are also mandatory. You will arrange the specific day and hours for your experience with your clinical preceptor. If you need to miss a day due to an emergency situation, you need to notify your clinical preceptor and clinical faculty and arrange for a time to make up the hours with your clinical preceptor. Required Equipment You will be required to obtain the following equipment for use in this course, and several of these items will be used in your clinical settings. Stethoscope 2 Clear plastic rulers, 5-6 inches long Pen light Manual BP cuff Reflex Hammer Lab coat Watch with second hand or second function MSU student nametag Optional Equipment These items can be signed out for the semester at the first on-campus workshop and returned at the last on-campus day for your final performance check-off. You will receive an incomplete in the course until all equipment is returned, and will be required to purchase any lost equipment or equipment that is not returned by midterm of the following semester. Oto/ophthalmoscope Specula for Otoscope Insufflator bulb for Otoscope Tuning Fork The following items will be provided for you for your on-campus experiences. You may want to purchase some to use when you are practicing: Tongue blades Cotton balls Exam gloves 5
7 Evaluation & Grading The overall grading system in this course is Pass/Fail. The following components of the evaluation methods are mandatory, and each must be achieved with an 80% minimum in order to pass the course, with the exception of the Final Performance Evaluation, which requires each component to be passed with a 90% minimum. All components of the evaluation methods are mandatory and must be completed and passed at the indicated levels in order to pass this course. The failure to pass any component will result in a failure for the course. Evaluation components: 1) Module check-offs/documentation: At each on campus workshop students will be required to check off on skills that have been learned from the assigned readings and online modules and submit written documentation of the assessment obtained. P/F 2) Health History: Each student will perform an initial health history on a standardized patient while on campus for the second workshop, and submit written documentation for this assignment. The student may be asked to repeat this assignment if not adequately completed. Students will also be required to do a self-review of their health history utilizing WebSp. If a redo is required, the student will need to videotape themselves obtaining a health history and submit to their clinical faculty for grading and feedback. 80% required to pass experience, if less than 80% student will be required to repeat. 3) Clinical Experience/documentation: 20 hours of clinical are required in order to pass this course. Written documentation is required for each clinical experience and completed clinical log with preceptor evaluation is required to be submitted at the end of the clinical experience. P/F 4) Quizzes: Students will take four online quizzes worth 20 points each; these are based on information from assigned reading and modules. 80% required to pass 5) Written final examination: A 100 point exam can either be taken on campus or proctored at an arranged site. 80% required to pass 6) Final Performance Evaluation: A score of 90% or better is required for each individual component in order to pass the course This evaluation may not be repeated. A complete health history and physical examination will be conducted following completion of the course requirements on a standardized patient utilizing the Learning Assessment Center. This is a timed exam, the student will have 30 minutes to complete the history portion, 45 minutes to complete the physical exam and 60 minutes total to complete the history and physical documentation components, components of each exam not completed during the allotted time will be marked as unsatisfactory. The performance will be digitally recorded for the purpose of instructor grading. Students will not have access to this recording. The final performance evaluation consists of four components: 1) Obtaining a complete health history; 2) Performing a head to toe physical exam; 3) Documentation of the history and 4) Documentation of the physical. The documentation components may be repeated one time only, the student will need to arrange with their clinical faculty an on campus time to complete the re-write. 90% or better for each individual component is required to pass this course. 6
8 Examination Proctor Guidelines Students may take examinations off campus as long as security measures for administering the test are strictly maintained before, during and after testing. Off campus examinees should be tested under the same circumstances as their peers on campus to ensure the academic integrity of the examination. The student is responsible for selecting a qualified proctor. The preference is that the exam be given by a proctor at a testing center. I have listed two centers we utilize below. It is the student s responsibility to contact the center. The test is a timed two-hour test. The date and time for the test will be posted in Angel after the beginning of the semester. List of testing centers available online for inclusion in courses FS The examination should be administered in a quiet, well-lighted office or classroom setting conducive to concentration. Exam administration in private homes is not appropriate. Proctor Responsibilities Paper Exams Store the exam in a secure location where it cannot be accessed by other students. Check student photo ID to ensure the examinee is actually the right person. Ensure no copies of the exam are made. Ensure that the student does not access notes, books, or articles during the exam unless the test specifies otherwise. Time the test and stop the examinee after time has been expended. Note any testing irregularities on a separate sheet. Secure the exam until mailing. Ship completed test to designated address within 4 hours of test completion. Use traceable shipping procedures (UPS, FedEx, etc.). Student is responsible for cost of shipping. If a fee is charged for exam supervision, request payment from the student at the time of testing. MSU will not pay proctor fees. Proctor Responsibilities Computer (Angel based) exams Store the password where it cannot be accessed by other students. Check student photo ID to ensure the examinee is actually the right person. Ensure no copies of the exam are made. Ensure the student does not access notes, books or articles during the exam unless the test specifies otherwise. Time the test and stop the examinee after time has been expended. Note any testing irregularities on a separate sheet, and to the instructor after the test has been completed. 7
9 If a fee is charged for exam supervision, request payment from the student at the time of testing. MSU will not pay proctor fees. MSU reserves the right to verify proctor qualifications, require additional evidence of eligibility or select a different proctor. The Proctor Agreement may be terminated for any cause by the proctor, student or MSU upon written notification to all parties. Note: You will find the Examination Proctor Agreement Form on the next page. 8
