4 Coordinating Care Across the Continuum October 15 This session will provide a clear understanding of the role of Social Workers and Care Managers in various aspects of the healthcare continuum in current NY state initiatives and nationally recognized programs. Additional topics will include: patient centered medical home, Medicaid 101 and DSRIP, a day in the life of care coordination in hospitals, ACO s, PCP and Specialist s offices, home care and community-based organizations, Transitions of Care and Disease Management. - Part 2 of 4 Sessions Series - - Part 3 of 4 Sessions Series - Coordinating Care Using a Whole Patient Approach November 19 Various approaches to care coordination will be explored in this session including techniques of Motivational Interviewing and a special focus on social determinants of health. Children, Trauma and Recovery: Clinical Interventions with Children Friday, November 18 9:00 am 12:00 pm Presenter: Kathleen Monahan, D.S.W., LCSW Children are exposed to and experience a wide variety of traumatic events. We now understand that children can suffer devastating consequences by experiencing and/or witnessing traumatic events, both in the short-term and well into adulthood. 5 Clinical Skills Working with TGNC Children and Youth Monday, January 23, :00 am 1:00 pm 4 Continuing Education contact hours for licensed social Research has illuminated the fact that the two most prominent times in a TGNC person s life where gender dysphoria arises is 3-5 years old and at puberty. When confronted by a gender variant child, a parent may be caught very much off guard, with no ability to rehearse the best response to such an unanticipated circumstance. In addition, for a parent, it may feel like a loss of the daughter or son to whom they became so bonded. Moreover, some families are aware of their child s gender dysphoria in early childhood while others are surprised to learn about it when their child is in their teens. In this workshop we will work to help create a safe and supportive environment for TGNC individuals and their families to grow and thrive in life. COMING SOON - Part 2 of 3 Sessions Series - - Part 3 of 3 Sessions Series - Clinical Skills Working with TGNC: Coming Out Later in Life Spring 2017 date TBA 4 Continuing Education contact hours for licensed social Much of the current research has focused on gender-nonconforming children or on transsexuals seeking gender confirmation surgery (GCS). Recent research has noted that many male-to-female identified persons are contemplating gender transitions in their later years. This workshop will address the importance of working with elder TGNC individuals and will present specific modalities in working with the TGNC Elders. - Part 4 of 4 Sessions Series - Continuous Quality Improvement in Care Coordination January 7, 2017 In the final session of this course, we will explore CQI programs that Social Workers and Care Managers can develop and monitor. A detailed look at how Design Thinking methodology can be used to solve patient concerns will also be taught. **Note: Students that have taken at least 2 other sessions are permitted to take this final session. This topic will be interactive and involve student case examples for exploration. Diane Monks, LMSW, Senior Education Specialist, Child Welfare Training Program School of Social Welfare, Stony Brook University This workshop will offer a combination of experiential and didactic components. Participants will be introduced to innovative approaches (e.g., energy psychology, movement, sound) to working with creative expression in the treatment of child sexual abuse. Techniques demonstrated will be transferrable to the practitioners work with clients. Participants will engage in activities specifically designed to elicit unconscious information held within the inner world of children. This workshop will provide skills and modalities in helping child victims heal from the trauma of sexual abuse. 8 5 Cost: $60
6 Trauma Overview, Assessment and Intervention Wednesday, October 19 9:00 am 12:00 pm Presenter: Joan Beder, DSW, LCSW Professor of Social Work, Yeshiva University, Adjunct Instructor, School of Social Welfare, Stony Brook University Focusing on three client trauma cases, participants will explore various definitions of trauma, risk factors for trauma and symptoms of trauma. Differences between post-traumatic disorder and acute trauma will be discussed and routes to helping will be explored. An array of trauma treatment theories will be explained and illustrated using the three cases presented in the beginning of the workshop. Series Overview: Transgender and onforming (TGNC) people are those who have a gender identity that is not fully aligned with their sex assigned at birth. They often face widespread discrimination and mistreatment within the healthcare system because providers may lack adequate training. Much of the existing information regarding TGNC has been extrapolated from studies of people who are lesbians, gay and bisexuals (LGB). While many of the issues may overlap, there is a gap in the literature and therefore understanding regarding issues specific to the lives of transgender persons. This workshop is intended to enhance social understanding of (TGNC) status as a lived human experience. This three-part series can help social become more culturally and clinically proficient in their work with TGNC individuals across their lifespan.
