Describe how the proposed practice will address the following issues in the population(s) of focus, while retaining fidelity to the chosen practice: demographics (race, ethnicity, religion, gender, age, geography, and socioeconomic status; language and literacy; sexual identity (sexual orientation and gender identity); and disability 10
