4 SUMMARY General objectives Organ transplantation (OT) is a consolidated therapy from which thousands of patients benefit in the EU. However, important variations in activity exist between Member States (MS). ACCORD intends to strengthen the full potential of MS in this field and to improve cooperation between them. With this general aim, ACCORD is focused on three specific areas as follows: 1. Developing live donor registries and fostering international data sharing on live donation. 2. Increasing the availability of organs from deceased donors by strengthening the cooperation between intensive care units (ICUs) and donor transplant coordinators (DTCs). 3. Promoting exchange of best practices and consolidated expertise in organ donation and transplantation through twinnings experiences. Finally, ACCORD, as a whole, will facilitate the consistent implementation of the Directive across the whole Union, provide MS with concrete assistance in meeting the objectives of the Action Plan and enhance cooperation between MS. Strategic relevance and contribution to the public health programme ACCORD represents added value by achieving its objectives and allowing the consequences of the action, but also by assisting MS in reaching the full potential of deceased and living donation. The JA represents a strategic opportunity in time, since it is now when the national provisions of the recent Directive are being worded and ACCORD will undoubtedly support the alignment of provisions and procedures. Best practices and networking activities in all three fields undertaken will be promoted. Additionally, elements of innovation are to be introduced through ACCORD in the field of donation and transplantation across the EU, for instance, a very first common basis for the development of live donor registries, a unique description on the different approaches to end-of-life practices and its impact on organ donation as well as a tailored model to facilitate the optimized realization of the deceased donation process, and an a set of recommendations for future twinnings will be developed. 5 Methods and means summary (see annex Ib) ACCORD requires a combination of methods distributed among the WPs: Setting down recommendations for a live donor registry and for international data sharing will require a specific survey to gather information on current registries, expert discussion, and piloting the recommendations. Strengthening the relationship between ICUs and DTCs requires a specific study for the description of the varied care pathways applied to patients with a devastating brain injury regarding organ donation and the identification of success factors as well as the creation of an improvement methodology with a toolkit of recommendations. Twinning initiatives in specific areas and MS will require the development of ad-hoc protocols. The experience will allow developing a guide for further twinnings in the field. Dissemination and sustainability is a relevant specific objective based on effective tools. A plan driving the design of these tools is devised to be produced at the beginning of the project. Expected outcomes period summary (see annex Ib) Recommendations for setting up living donor registries (LDR) will be provided, facilitating international data sharing, which will provide MS without LD with standards and expected results. LDR will increase the safety of LD programs. The variability in end-of-life practices applied to brain injured patients and their impact on organ donation (OD) will be described; the areas for improvement, identified; and an ad hoc toolkit methodology, designed. Training to achieve repeatable rapid improvement will be provided. The donor potential is expected to expand and the deceased donation process, optimized. Through twinnings, the expertise or tools developed by a MS are to be transferred to other/s requesting it, depending the areas to be reinforced on the MS involved. A model for further twinning actions will be built, facilitating new initiatives in the field. Consistent implementation of the Directive and the Action Plan, as well as cooperation between MS in the field will be facilitated. Page 5/14
