This center will integrate health and wellness into the treatment of patients with mental disorders. #131 Behavioral Health Services General Fund This fund will support the education and research projects for Behavioral Health Services, including new equipment, seed money for pilot projects and educational tools. #144 C.A.R.E. Initiative General Fund A general fund for unrestricted donations made in support of the C.A.R.E. (Caregiver Assistance Resources and Education) Initiative. #160 Center for Integrative Medicine The Center provides a healing environment in a space of total relaxation and comfort. It is a state-of-the-art facility where highly trained practitioners provide therapies drawn from the best of the world's healing traditions to help you achieve overall wellness and optimal health. The Center's treatments include: neuromuscular therapy, massage therapy, acupuncture, and herbal and nutritional counseling. #630 Benson Ford Fund - Where Funds are Needed Most This trustee-supervised fund directs your gift where it is needed most throughout the System and helps make possible improvements to medical facilities, fund research programs and provides the necessary tools for managing the health needs of people in our community. #888 Black United Fund of Michigan (external fund; funds do not stay at HFHS) The Black United Fund of Michigan was established in 1970 on the principles of community enrichment, self-help and education. This organization supports more than 50 local organizations annually through contributions made from workplace fundraising campaigns. #320 Bob and Sandy Riney Helping Hands Fund Helping Hands is a charitable initiative sponsored and funded entirely by employees and physicians to help co-workers, system volunteers and retirees in times of need. Since its inception in 1992, the program has provided financial assistance to hundreds of people in need. It has achieved amazing results including helping pay for food, clothing and shelter for families who have lost their homes to fire as well as helping those who are sick or injured to pay for medical care unavailable through other means. #133 Bone and Mineral Research - Endocrinology This fund will support Bone and Mineral research projects for Endocrinology, including new equipment, seed money for pilot projects and educational tools. #134 Bone Marrow Transplant Emergency Needs Fund This fund will support the education and research projects for bone marrow transplants, including new equipment, seed money for pilot research projects, new technology and education tools. #140 Cancer Patient Prescription and Treatment Fund (HFCI) This program was established by a cancer patient who wanted to help patients being treated at the Henry Ford Cancer Institute with limited resources. The program assists cancer patients who lack the economic resources to cover their medication, necessary treatment supplies or transportation for appointments.

3 #142 Cardiac Catheterization Lab Fund This fund will support the education and research projects for the Cardiac Catheterization Lab within the Division of Cardiology including new equipment, seed money for pilot projects, new technologies and educational tools as well as funds for meetings and symposiums. #143 Cardiology Education Fund Contributions to this fund help support the ongoing improvements and equipment that lead to better diagnosis and treatment of the Heart and Vascular Institute patients. #141 Cardiothoracic Surgical Division Fund Donations to this fund will help support Henry Ford's Cardiothoracic Surgical Division through educational program and funding for special research projects. #145 Center for Autism and Developmental Disabilities (CADD) This fund supports Cottage Medical Center initiatives toward the health care needs of children in Pediatrics and Autism research. The Henry Ford Center for Autism and Developmental Disabilities (CADD) is a world-class center committed to improving the lives of children and youth with special health care needs by providing a comprehensive diagnostic evaluation, evidence-based and accessible ABA therapy services, and a medical home standard of primary care. #150 Center for Health Services Research The Center for Health Services Research conducts studies on new ways to improve how the health care system functions. It studies innovative ways to improve quality, control cost, and improve patient outcomes. Center investigators work collaboratively with doctors and nurses in studies on asthma, diabetes, and cancer care, as well as on racial/ethnic disparities in care for a variety of clinical conditions. #382 Center for Medical Imaging This fund supports medical imaging technology and programs for Henry Ford Wyandotte Hospital and the Brownstown facility. #180 Center for Simulation, Education and Research at Henry Ford Hospital A comprehensive simulation, education and research center for doctors, nurses, students and other health care providers, to learn and practice real medical situations and enhance the provision of high quality, safe medical care. This state-of-the-art, 12,000 square foot space is the largest simulation center in the Midwest, and is located in the basement of Henry Ford Hospital. #185 CLEAR Conversations Project The Clear Communications Project supports the implementation of the connect-listen-empathize-align-respect (CLEAR) Conversations model to continually improve physician communications with patients and families. #194 Detroit Institute of Ophthalmology Friends of Vision Fund Donations to this fund supplement monies raised by our volunteer arm, the Friends of Vision. #195 Detroit Institute of Ophthalmology Support Group Fund This fund covers expenses related to running support groups at the Detroit Institute of Ophthalmology (excluding transportation expenses). #197 Detroit Institute of Ophthalmology Research Fund This fund will support research development. #213 Dr. Colleen Grace Cancer Fund The purpose of this fund is to honor the memory of Dr. Colleen Grace, late daughter of William and Sue Conway, and to fund general purpose cancer programs, to include but not limited to, research, education, equipment or other capital needs.

The funds raised will be used to support the expansion, renovation and equipment needed to complete this project. #322 Cottage Medical Center Emergency Department This fund would help support the education and research projects for the department, including new equipment, seed money for pilot projects, new technology and educational tools.

5 #325 Cottage Medical Center Mary Gene Buhl Opperman Women's Diagnostic Center Henry Ford board-certified radiologists and a specialized staff of technologists provide film-screen and digital mammography, 4-dimensional ultrasound, bone densitometry, and minimally invasive breast biopsy in a female-friendly, attractive and comforting environment. A Registered Nurse Breast Care Coordinator gives women guidance and support when further testing or surgery is needed. Mammopads are provided to patients free of additional charge to help minimize compression discomfort during mammograms. #327 Cottage Medical Center Rehabilitation This specially designed unit offers a home-like environment where patients can regain strength and independence following debilitating injury or illness through an individualized plan of care that may include physical, occupational, and speech language therapy. Family members or other support persons are encouraged to participate with the patient and their rehab team which includes physicians, nurses, therapists and social workers. #328 Cottage Medical Center Special Needs Fund Donations to this fund will cover the cost of new equipment, programs unique to the center and facility upgrades. #333 Henry Ford Continuing Care Resident Needs Fund Funds to be used to support resident/facility activities. Activities to include but not limited to: resident outings; facility improvements; and resident activities. #335 Henry W. Lim, M.D., Endowed Chair in Dermatology Research Supports research consistent with the mission of the Henry Ford Hospital Department of Dermatology. The research should further enhance the position of the Department as a leading academic department of dermatology in the US, a department with a long tradition of providing excellent patient care through research and education of the next generation of dermatology. #336 Henry Ford Macomb Hospitals Trauma Fund Supports the trauma program at Henry Ford Macomb Hospitals. #338 Henry Ford Macomb Hospitals Strategic Fund The Henry Ford Macomb Hospitals Strategic Fund will receive annual donations that will be restricted for the general operating and/or capital needs of the hospitals. The Chief Executive Officer (CEO) of Henry Ford Macomb Hospitals will identify areas of need, and the funds raised will be spent at the discretion of the CEO. #386 Leslye Indenbaum Fund for Neurosciences This fund will support research in neuroscience. #388 Henry Ford Wyandotte Hospital Men's Health Fund Education community outreach, support for medical costs for men's health concerns. #389 HFCI Downriver Capital Building Fund Supports the capital building of the expansion of the current Henry Ford Cancer Institute - Downriver Campus. #390 Hypertension Education & Research Fund The Hypertension Research and Education Fund supports the education and research projects for the department, including new #611 D. Emerick and Eve Szilagyi Chair in Vascular Surgery This fund was established to support the legacy of this outstanding clinician and scientist. Funds support the research and educational endeavors of the Division of Vascular Surgery at Henry Ford Hospital.

6 #219 Department of Emergency Medicine Education and Research This fund is for the development of research and education and will be used to further the program in the Department of Emergency Medicine. #190 Dermatology Education and Research Fund This fund will support the education and research projects for the department of Dermatology, including new #200 Diagnostic Radiology (Eyler Fund) This fund will support the education and research projects for the department of Diagnostic Radiology, including new #209 Dialysis Patient Nutrition Assistance Fund This fund provides supplements at cost or free of charge to Greenfield Health Systems dialysis patients who are financially unable to purchase them otherwise. #215 Each Patient First - Community Care Services Community Care Services is the HFHS business unit dedicated to providing health services outside the medical centers of the Henry Ford Medical Group and the inpatient settings at our hospitals. #221 Emergency Department - Sterling Heights Medical Center This fund would help support the education and research projects for the department, including new equipment, seed money for pilot projects, new technology and educational tools. #230 Endocrinology and Metabolism Research Fund This fund will help support the education and research projects for the department, including new equipment, seed money for pilot projects, new technology and educational tools. #233 Epilepsy Program Fund This fund will support the education and research projects specific to epilepsy, including new equipment, seed money for pilot projects, new technology and educational tools. In addition, this fund assists patients with epilepsy who lack the economic resources to cover their medication, necessary treatment supplies or transportation for appointments. #350 Fairlane General Fund Fairlane currently has two main projects in need of funding: the expansion and renovation of the ambulatory surgery area, and the expansion and renovation of the emergency department. Both of these areas are very important not only to the patients at Fairlane, but to all of the patients in the Southern region. #348 Fairlane Medical Center Emergency Department This fund will support the Fairlane Emergency Department, providing funding for new equipment, seed money for pilot projects, new technology and educational tools, and a variety of other activities that are vital to the productivity of the department. #250 Family Practice Education and Research Fund This fund will support the education and research projects for the department of Family Practice, including new #265 Game On Cancer Fund An unprecedented initiative to significantly raise awareness and support for the fight against cancer. Funds raised go directly to cancer research and patient needs.

7 #270 Gastroenterology Education and Research Fund This fund will support the education and research projects for the department of Gastroenterology, including new #278 Gene Therapy Cancer Research This fund will support the research projects for the department of Gene Therapy Cancer, including new equipment, seed money for pilot projects, new technology and educational tools. #279 General Internal Medicine Fund This fund will support the education and research projects for the department of General Internal Medicine, including new #304 Head and Neck Cancer Program These funds will be used to support the enhancement and expansion of the Head and Neck Cancer Program within the department of Otolaryngology. The needs of this program include patient and staff education as well as funds for research, equipment and capital needs. #290 Healing Art Program This program uses art to promote health and healing by enhancing the physical environment for everyone using System hospitals and medical centers. Through the addition of artwork in care delivery rooms and waiting areas, the programs looks to create an atmosphere that is calming and nurturing for patients as well as staff. Donations to this fund will support the purchase, installation and maintenance of artwork throughout the System. #303 Heart and Vascular Institute Research Fund This fund supports research projects for the Heart and Vascular Institute at Henry Ford Hospital. #302 Heart Failure Rehab Fund This fund will support the education and research projects for Heart Failure Rehab, including new equipment, seed money for pilot projects, new technology and educational tools. #301 Heart Failure Research Fund This fund will support the education and research projects for Heart Failure, including new equipment, seed money for pilot projects, new technology and educational tools. #329 Henry Ford at Home Development Fund This fund is used for education and research initiatives that benefit patients in their homes and the staff who care for them. #380 Henry Ford Hospice Hospice is a unique philosophy of care designed to meet the needs of patients who are in the final stages of terminal illness. Home care supported and guided by a multidisciplinary team of professionals is at the heart of the hospice philosophy. The goal is to manage terminal disease in a way that makes it possible for dying patients to remain at home in the care of their families for as long as possible. #340 Henry Ford Hospital - Tom Groth Patient Medical Needs Fund This is an amazing opportunity to help people who desperately need help but can't afford the cost of health care. At Henry Ford Hospital, the Patient Medical Needs Fund was established for patients who need urgent help. It might be a prescription for an elderly patient's needs. Or it might be a piece of life-saving equipment for a young mother. Hundreds of low-income patients, including the elderly, young adults and underinsured receive prescriptions, equipment and other medical necessities through this program which they would otherwise not be able to afford.

8 #330 Henry Ford Hospital Building Fund Over the next ten years we need to raise funds to pay for major upgrades to our campus and for important programs unique to the hospital. A gift to this building fund will help renovate this historic facility in ways that will touch hundreds of thousands of lives for decades. #220 Henry Ford Hospital Emergency Department Renovation Fund The Department of Emergency Medicine is the largest gateway to Henry Ford Hospital. The Department has a distinguished history of both basic science and clinical research. This Level I trauma center will embark on a major renovation project to expand the space and services to better serve the city of Detroit and all Southeast Michigan. #343 Henry Ford Hospital Memorial Garden The Memorial Garden offers a quiet place near the hospital for parents and family experiencing a loss of a loved one to express love and grieve in natural, peaceful surroundings. #540 Henry Ford Hospital Research Program The Research General fund supports System research initiatives, including new equipment, seed money for pilot projects, new technology, educational activities and infrastructure improvements (i.e., renovations). The System's research studies fall into three main categories: 1) basic science studies in important areas of biomedical research; 2) clinical trials of new treatments and devices; and 3) health and population studies related to the prevention, diagnosis, treatment and management of chronic and acute conditions. #381 Hermelin Brain Tumor Center The Hermelin Brain Tumor Center has been recognized by the National Cancer Institute since 1993 for delivering the most advanced brain tumor therapies. The Center is on course to become one of the top five innovative brain tumor centers in the United States. #402 HIV Patient Needs Fund This fund supports the efforts of studies of cause and prevention of infection in hospitalized patients, prevention of food-borne infections, infections in the immune compromised patients and HIV/AIDS research and education. #390 Hypertension Education and Research Fund The Hypertension Research and Education Fund supports the education and research projects for the department, including new #400 Infectious Disease Education and Research Fund The Infectious Disease Education and Research Fund supports the efforts of studies of cause and prevention of infection in hospitalized patients, prevention of food-borne infections, infections in the immune compromised patients and HIV/AIDS research and education. #399 Innovation Institute Established in 2012, HFII is the flagship of Henry Ford Health System's commitment to innovation and serves as both a physical and virtual resource. It provides System innovators access to an array of intellectual asset-related resources and programs that include technological opportunity assessment, engineering services for prototypes, seminars designed to convey opportunities, programs aimed at developing specific medical products and broad educational offerings in the realms of translational medicine and the entrepreneurial arts. #410 Internal Medicine Academic Development Fund This fund will support the education and research projects for the department of Internal Medicine, including new #418 Movement Disorders Research Fund To support research being conducted by the Department of Neurology. #421 Nephrology Education Fund

9 This fund will support the educational projects for the department of Nephrology, including new equipment, seed money for pilot projects, new technology and educational tools. #398 Interventional Radiology Fund This fund will be used to support the equipment, new technology and patient needs of the Interventional Radiology program throughout Henry Ford Health System under the direction of the Chairman of the Department. #393 Henry Ford Cancer Institute This fund supports the education and research projects for the Henry Ford Cancer Institute, including new equipment, seed money for pilot projects, new technology and educational tools. #395 Henry Ford Cancer Institute Endowment Fund This fund is endowed to provide ongoing support of oncology research through Henry Ford Cancer Institute. As an endowment, only the interest made off the corpus investment will be spent, allowing research to be supported in perpetuity. #392 Henry Ford Cancer Institute Downriver Patient Needs Fund The Patient Medical Needs Fund was established for patients who need urgent help. It might be a prescription for an elderly patient's needs. Or it might be a piece of life-saving equipment for a young mother. Hundreds of low-income patients, including the elderly, young adults and underinsured receive prescriptions, equipment and other medical necessities through this program which they would otherwise not be able to afford. #406 Karen J. Stuck MD Radiology Resident Endowment Fund The annual interest accrued from this endowment will be provided each spring as a stipend to the outstanding senior radiology resident who demonstrates excellence in teaching, research, and education #397 Kidney Cancer Research Fund Funding will be used for the continued development and research of minimally invasive surgery techniques and their use in kidney surgery. #396 Kidney Transplant Emergency Needs Fund This fund supports kidney transplant patients who are in medical or financial need. #522 Lung Transplant Fund Gifts to this fund will help support the education and research programs for lung transplantation in the Transplant Institute. #356 Macomb Hospital Medical Staff Fund The Henry Ford Macomb Hospital medical staff provides high quality and personal medical care to thousands of people who live and work in the surrounding area. Donations to this fund will support the needs of Henry Ford Macomb Hospital based on recommendations made by members of the medical staff. #351 Macomb Hospitals Behavioral Medicine Fund The Henry Ford Macomb Hospital behavioral medicine staff work with patients to provide innovative and effective treatments. This fund will be used to support the needs of the Behavioral Medicine Program. #352 Macomb Hospitals Circle of Caring Fund This fund previously directed by Sr. Bernadette will be used where it is needed most at Henry Ford Macomb Hospital to provide the necessary tools for managing the health needs of people in our community. 10 #358 Macomb Hospitals Employee Emergency Seton Fund This fund recognizes that all of us face emergency situations from time to time that may require assistance in meeting these unexpected expenses. Henry Ford Macomb Hospital has established the Seton Employee Emergency Fund to help employees meet financial needs in a time of crisis. #353 Macomb Hospitals - Heart Fund Donations to this fund help support the ongoing improvements and equipment that lead to better diagnosis and treatment of Heart and Vascular patients. #354 Macomb Hospitals Neighbors Caring for Neighbors General Fund The Neighbors Caring for Neighbors Outreach Clinics provides basic medical care and health education to uninsured lowincome Macomb County residents. The clinics offer a wide range of services including treatment of acute conditions, management of chronic illnesses, physicals, and prescription assistance. Children and adults alike benefit from the sacred care given at the clinic. #357 Macomb Hospitals Neuroscience Fund This fund will support the education and research projects for the department of Neuroscience, including new #355 Macomb Hospitals Nursing Fund Nurses are important and vital to the success of Henry Ford Macomb Hospital. Your support of Pastoral Care at Henry Ford Macomb Campus can enable our Chaplain to continue providing families of loved one with these types of items whenever needed. #412 Maplegrove Patient Care Fund This fund will provide support and treatment to patients and the families in need of the services through the alcohol and substance abuse program at Henry Ford Maplegrove in West Bloomfield. #417 Medical Education Fund This fund will be used specifically for the education and advancement of our staff through a variety of activities. #642 Minimally Invasive Program Development For the promotion and development of staff interested in minimally invasive women's health surgery - robotics. #413 Multiple Sclerosis Research Fund This fund will support the education and research projects specific to Multiple Sclerosis, including new equipment, seed money for pilot projects, new technology and educational tools. In addition, this fund assists patients with Multiple Sclerosis who lack the economic resources to cover their medication, necessary treatment supplies or transportation for appointments.

Peripheral neuropathy is a disease which results in the loss of motor function and sensitivity, which affects millions of people in the US. #282 Neuroregeneration Center Fund This center develops novel restorative therapies for the treatment of stroke, neural injury, and neurodegenerative diseases, such as multiple sclerosis and dementia. Nearly all people will develop a neurological disease during their lifetimes. The highly innovative and effective approach of this center is that the focus of the development is on ways to remodel and rewire the central nervous system after an injury or onset of disease. This center is developing the means to stimulate the body's intrinsic restorative processes to effectively treat neurological disease. #442 Neurosciences Fund This fund supports programs and projects for the Neurosciences Institute. #440 Neurosurgery Education and Research Fund This fund will support the education and research projects for the department of Neurosurgery, including new #445 Next Generation Radiosurgery This fund supports continuing educational and research projects in the quickly evolving field of radiosurgery for the treatment of cancer. SandCastles Grief Support Program, established in 1997 by the Hospices of Henry Ford Health System, focuses on the belief that every child and teen deserves the opportunity to grieve in a supportive, nurturing environment. Grieving children and teens, ages 3 to 18, are offered a special place where they can interact with peers who have experienced similar losses, learn life-long coping skills and most importantly begin a healthy healing process. This program exists solely on donations given directly to the program. #555 Nancy Schlichting Endowed Leadership Lecture The fund will pay tribute to Nancy Schlichting's vision, innovation and determination, which ushered in a new era in Henry Ford's long and storied history. Every year, this lecture will invite great minds in leadership and health care to Henry Ford Health System to speak on topics that foster the same kind of illumination and transformation Nancy has inspired. #560 School-Based and Community Health Program The SBHI has successfully developed and now manages a network of school-based health centers in nine Detroit public schools and one Warren high school. The library receives a tremendous number of visits - both physically and on the internet. More than 125 people visit the library each day and receive more than 30,000 'hits' on the library website. By donating to this fund, you help support this service for the physician, nurses and patients served by Henry Ford Hospital. #571 Sleep Center Education and Research Fund The Sleep Center Education Fund will support the education and research projects for the division of sleep medicine, including new equipment, seed money for pilot projects, new technology and education tools. #580 Southern Region Patient Medical Needs The Southern Region Patient Medical Needs Fund was established for patients who need urgent help. The fund is available to fund distress screening, support groups, survivor programs, nutrition education, meditation, acupuncture, music and art therapy. #590 Surgery Education and Research Fund The Surgical Education and Research Fund supports innovative approaches to surgical resident education including computer based, multimedia instruction, and the use of simulators. The fund supports the development, purchase, and maintenance of the surgical education and simulation laboratory, as well as research projects which investigate the outcomes of these educational efforts. #599 Tomlanovich Endowed Fund The Department of Emergency Medicine is the largest gateway to Henry Ford Hospital. The Department has a distinguished history of both basic science and clinical research. This fund will continue to support the educational and research needs of the department. #482 Total Joint Research Program in Orthopedics Donations to this fund will help support research activities and projects related to bone and joint in the Department of Orthopedics. #600 Transplant Institute The Henry Ford Transplant Institute is one of the nation s busiest and highest regarded transplant centers. Patients and their families can feel confident knowing that Henry Ford's transplant survival rates are among the best in the state, as well as significantly above the national averages.

16 #623 Howard Neff Fund to Support Vision Restoration Vision restoration was a passion for Howard Neff, M.D. He believed that we will be able to implant a device in the eye that will help the blind to see again. Progress has been made in this field but more research is still needed. Money donated to this fund will support research in the development of neuro-prosthetic devices to restore vision. The Fund can be used toward: equipment, research methods, salary and other resources that will contribute to the development of vision restoration techniques, equipment and practices. #628 Warden Chair in Multi-Cultural Health This fund will support continuous support to the Institute on Multicultural Health for initiatives that serve underrepresented racial and ethnic populations through research, education and the delivery of culturally appropriate healthcare. #641 Women's Community Health Education and Research Fund This fund is for the development of research and education and will be used to further said program in the department of Women's Health at all locations.
