2 Psychosocial Oncology Specialization Roster of Practicum Education Agencies for MSSW Psychosocial Oncology Specialization Students Agencies and organizations utilized for Kent School practicum education are listed alphabetically. Due to unpredictable organizational and staff changes, there may be semesters in which agencies are temporarily unable to accept students in placement. Availability of individual agencies is subject to change without notice. Please note that most agencies now require a criminal records check before finalizing a commitment to accepting a student in practicum. Also, medical facilities will require a health screening and medical records for all students in practicum. Please note that medical settings require additional materials from you before they will finalize acceptance for a practicum placement. Please allow more time to satisfy all these extra requirements should you select a medical site. Generally, these screenings are at the cost of the student. SW 672/673 Advanced Practicum Concurrent with SW 640/677 These courses are offered only during the Fall and Spring semesters. This is a hour per week practicum only offered as a two consecutive semester course for a total of 500 hours and 6 academic credits. All Advanced students are required to take this practicum. The following organizations have been approved jointly by the Director of Field Education along with the Coordinator of the Psychosocial Oncology Specialization Program as satisfying the requirements for both the MSSW and the Oncology programs. *Other sites may also be acceptable, especially for remotely located students. Please work in conjunction with your field coordinator to ensure the appropriateness of a field placement agency. Local Placements (Louisville, Southern Indiana) 1. Baptist Health Cancer Care Center Oncology Social Worker serves as the Psychosocial Services Coordinator and works collaboratively with departments and/or community organizations to provide, improve, and expand the range of psychosocial services. The Oncology Social Worker provides psychosocial screenings and assessments to patients on the inpatient Oncology Unit, Outpatient Radiation Center, and Lung Care Clinic. The Oncology Social Worker evaluates and offers instruction in possible coping strategies and problem solving. Oncology Social Workers help patients and families make crucial decisions regarding their care particularly regarding quality of Life. Hours: Monday Friday 8:00 4:30pm.

Floyd Memorial Hospital Cancer Care Center (New Albany, IN) Baptist Health Cancer Care at Floyd provides state-of-the-art technology and an array of treatment options to fight cancer in adult patients. The comprehensive Center provides access to medical oncology as well as radiation oncology all in one convenient location. The facility sees approximately 160 patients per day for hematology, physician consultation, chemotherapy infusions and radiation therapy appointments. A social work intern placement at Baptist Health Cancer Care at Floyd will provide access to participation in quality reports and studies, support groups, implementation of the NCCN distress tool and psychosocial assessments, as well as helping meet the needs of patients throughout the treatment phases. 3. Friend for Life Cancer Support Network Students will assist staff in providing peer-matched support, disseminating information about the organization, recruiting volunteers, and supporting fundraising efforts. Students will complete intakes and eventually match support seekers with volunteers. 4. Gilda s Club Gilda s Club Louisville is a wellness community for people of all ages who are affected by cancer -- whether diagnosed themselves or connected to someone else who is. We are an open, diverse community of men, women and children. We are a peer-based model that offers peer support groups and networking groups, as well as lectures and workshops, and social events to help build a wider circle of support for members on their cancer journeys. We believe that social and emotional support is as important as the medical care our members receive. We also believe that every person affected by cancer is an expert, but that it is from sharing the collective wisdom of all members that each person s cancer journey is enhanced. We are not a clinical setting and do not offer mental health assessment or counseling, or traditional group counseling. 5. Hosparus American Cancer Society Grant The student will have a unique learning opportunity that includes the full trajectory of a cancer experience. The practicum is planned in partnership with Gilda s Club and the University of Louisville Palliative Care. The student will work with Gilda s and UL the first semester and Hosparus (active patient care

4 and grief counseling) the second semester. ***Only in Louisville*** 6. Hosparus Active Patient Care The student will serve as a social worker on an interdisciplinary team that assists terminally ill patients and their families at home, in nursing homes, or in the Hosparus inpatient unit. Social workers conduct psychosocial assessments and provide services that will include counseling, assisting patients/families in adapting to their situations and coordination of practical helping resources. Services are aimed at helping patients experience a safe and comfortable death, facilitating the patient s self-determined life closure and assisting the patients/families with effective grieving. May carry a caseload of up to 10 patients. Grief Counseling Center The student will provide bereavement services, including crisis intervention, assessments and referrals, grief counseling to individuals, families and groups. Opportunities are also available to work with bereaved children and adolescents. May carry a caseload of up to 10 clients. Limited evening and weekend hours may be available through the Grief. 7. Kentucky One Health--University of Louisville: Interdisciplinary Program for Palliative Care and Chronic Illness This is a macro placement focusing on health-related research on an interdisciplinary team of specialists in social work medicine chaplaincy nursing and psychology. We work to improve the lives of those affected by cancer and other advanced illnesses through research and education. Our research projects focus on quality of life supportive care symptom control special populations care-giving and systems of care. Educational projects involve learners from students to advanced practitioners test novel educational approaches and center on interdisciplinary training. Students in this placement will participate in implementation data collection and analysis and dissemination of the Interdisciplinary Curriculum in Oncology Palliative Care (icope) project funded by the National Cancer Institute. 8. Norton Healthcare Audubon: UK Transplant Specialty Clinic The UK Transplant Center specializes in the care of patients with advanced, endstage organ disease. The program performs more than 150 transplant procedures each year including adult and pediatric kidney, liver, heart and lung transplants. As an advanced center, we also perform combined transplants for kidneypancreas, heart-lung, and heart-kidney. The living kidney donor program provides an alternative to kidney transplants from a deceased donor for patients with end-stage renal disease. Site participates with the Paired Kidney Exchange Program through UNOS. This site does not directly perform transplants, but

5 assesses suitability of potential donors through psychosocial assessment and other techniques. 9. Norton Healthcare Cancer Institute Social workers serve patients with cancer in ALL Norton facilities; provides support group for care-givers. 10. Norton Healthcare (Downtown) Palliative Consult Team currently unavailable Provides comfort care to patients with chronic pain including cancer and other medical conditions. 9 am-6pm M-F 11. Veterans Administration Medical Center: Hospice and Palliative Care The VA provides services to former military personnel through a wide array of services. The Hospice and Palliative Care department at the VA provides pain management and end of life services to veterans. This placement interviews multiple students for a small number of positions and places early in the year. Previous Online Student Placement Sites 1. University of Kentucky Healthcare- Markey Cancer Center-Lexington, KY Markey is the only NCI-designated center in Kentucky and one of only a handful in the country. The clinical programs and services of the Markey Cancer Center are integrated with the UK Albert B. Chandler Hospital. The cancer specialty teams work together with UK Chandler Hospital departments and divisions to provide primary patient care and support services as well as advanced specialty care with applicable clinical trials. All diagnostic services, clinical and pathology laboratories, operating rooms, emergent and intensive care, and radiation therapy services are also provided to cancer patients through UK Chandler Hospital. The Markey Cancer Center is among the few institutions nationwide providing multidisciplinary care through Clinical Care and Research Teams (CCARTs). Major CCARTs include breast, gynecologic, gastrointestinal, head and neck, lung, brain, hematologic and genitourinary cancers. 2. Frankfort Regional Medical Center For nearly 40 years, Frankfort Regional has built a strong reputation for outstanding cardiology, emergency, orthopedic, surgery, and women s services. In addition, they have a strong network of quality primary care physicians in both family practice and

6 internal medicine. As it is a small facility, the student would be working with patients across departments. 3. Heartland Hospice Cincinnati, OH Students with BSW may take OH test for licensure and then may be allowed to practice independently in advanced practicum for master s degree. Provides advanced medical care with a compassionate touch where ever patients consider home: own home assisted living nursing home etc. Students assist with advance directives and health care decisions. Agency focuses on sustaining quality of life by encouraging personal choices supporting meaningful experiences and preserving patient dignity. Personnel build partnerships with patients and loved ones with Hospice interdisciplinary team members and encourages family members to be involved in all care decisions. 4. Lake Cumberland Regional Hospital-Cancer Treatment Center The Cancer Treatment Center provides radiation therapy treatments, but also includes services around the social-emotional well-being of patients and their families. Programs at the Center include massage therapy, cancer support groups, Love on a Leash visits, and one-on-one care. Students will be exposed to psychosocial assessments, drug assistance programs, financial assistance, home health care, and support groups. 5. Pikeville Medical Center-Leonard Lawson Cancer Center The Leonard Lawson Cancer Center (LLCC) is a full-service facility providing state-ofthe-art cancer treatment and follow-up care at a regional level. Staffed by expert oncologists, nurses, pharmacists, patient navigators and many other highly-trained healthcare professionals, LLCC provides quality care close to home. Services provided include: medical oncology, hematology, gynecological oncology, radiation therapy (7 days a week), infusion services (7 days a week), genetic counseling, palliative care, and patient navigation. 6. St. Jude Children s Research Hospital (Memphis, TN) 7. UCH-MHS (Memorial Health System) (Boulder, CO) Assist staff at UC Health Memorial in providing care coordination services to patients with a cancer diagnosis. Help provide extensive discharge planning for patients exiting the hospital and moving to appropriate facilities. Guide patients through financial assistance application and advocate for their needs.
