Minutes from February meeting were approved with no changes. As the MOTA vendor, the Community Empowerment Center has established a relationship with another faith based organization. A tobacco grant was awarded by the state to educate retailers on selling to minors. Since the last meeting over 400 pamphlets on health disparities have been distributed. State Office of Rural Health, Roxanne Hale, DHMH Office of Primary Care Access Ms. Roxanne Hale, Director of the Office of Primary Care Access provided an overview of programs within the department, including: State Office of Rural Health: technical assistance, data support, recruitment assistance. SHIP: grant funds for small hospitals. Workforce Development Programs: Include Visa waiver programs, federal and state loan repayment program for practitioners. Full presentation is attached. Nicole Moore, Program Coordinator for Coastal Care Choices at Coastal Hospice Coastal Care Choices is a 5 year Medicare model study looking at long term effects of concurrent treatment and palliative care for patients diagnosed with cancer, HIV or AIDS, COPD or heart failure. Presentation attached.

2 The goal of the study it to determine whether the model effectively increases access to services, improves quality of life and reduces re hospitalization rates. Patients are referred by their primary physician, and once enrolled are served by their primary physician as well as registered nurses, social workers and chaplains who provide emotional support. Eligibility requirements include being enrolled in Medicare Parts A, B and D and have at least 2 hospitalizations in the last 12 months related to the diagnosis, among others. Coastal Hospice would like to have a census of approximately 27 patients to be able to support social work staff and chaplains. SUBCOMMITTEE UPDATES Tammy Griffin Chronic Disease, Diabetes and Obesity Subcommittee: Planning annual Walk at Lunch for May 18th from 11:30 2. Walk route will be altered due to construction at the Riverwalk. Walk will begin at the garden plaza on the downtown plaza. Planning is underway for a Chronic Disease and Oral Health Symposium in the fall of Working with Chris Leo at the state to develop an oral health and diabetes brochure. National Public Health Week is April 4 10, Presentation flyers were shared. All events are open to the public. 12 Heroin/Prescription Drug Overdose Epidemic 11:30 Dangers of Tobacco Lunch and Learn 3:00 Outstanding Public Health Leader ceremony 12:00 PreDiabetes/Obesity Informational YMCA 1:00 Zika Virus CFES 10 Comeback of Syphilis and PrEP Fritz health center April 9th: Dr. Lark Eshleman, childhood trauma expert is the keynote speaker at a seminar on healthy attachments. Breakfast and lunch are provided, and childcare is available. Flyer shared. Dr. James Cockey Sexually Transmitted Infections Subcommittee: Syphilis rates have increased 3 4 times locally, primarily due to prevalence of dating apps being used to find anonymous sexual partners. Recruitment has begun for PrEP program. Staff training is complete. Clients identified as high risk for HIV are evaluated and put on prophylactic treatment. First PrEP clinic is April 13, Adult Dental Task Force: Grant funding was not received to support dental clinic space not being used at the Holly Center. Currently working in partnership with HOPE, Inc. to help pay for dental services for enrolled adults. Asthma: Have worked with Recreation and Parks to revive Camp Huff N Puff, but will not take place this year.

3 HEALTH OFFICER UPDATES MEMBER UPDATES The health department was awarded PHAB accreditation on March 8th, and becomes one of only 5 local health departments in the state of Maryland that are PHAB accreditation. Annual report will be due in March Will report on a few areas as requested. CHRC grant funds were awarded to expand the School Based Wellness Center operations into one additional school. Services will begin at Wicomico Senior High in the fall. The methadone unit was smoothly relocated with cooperation from PRMC. The new facility has two dispensing windows, and wait times are greatly reduced. The facility has a counseling space available. A media competition for Wicomico county students to develop a video on heroin issues has just been announced. Funded by the State's Attorney s office and law enforcement. Scholarships will be awarded. One of the requirements to enter is that the student and their parent must watch a video of a young woman affected by prescription drug and heroin abuse. April 5th: Lori will be meeting with the County Council regarding a proposal for an outreach and education program to reach people that are using in areas known to be high overdose areas. Have been given potential candidates that are in recovery who would work alongside law enforcement to provide a bridge to treatment and prevent further overdoses. County Executive and the City are on board and funding is anticipated. Have been looking at overdose data. In calendar year 2015 there were 100 overdoses seen at PRMC ED. In January and February, 2016 there were 50, 21 in a 6 day span in January. Naloxone has prevented many, with only one fatal. As of July 1st, we become one of 8 jurisdictions moving the substance use disorder program to a fee for service program. Lori is serving on a state level subcommittee looking at the process for transitioning to fee for service. Working with Maryland Dept. of Agriculture and Mosquito Control to develop literature (door hangers and brochures) on mosquito control and abatement. County health rankings were recently released and will be reviewed in greater detail at a future meeting. Donna Clark, HOPE, Inc. In the last quarter, 98 adults have been reached with dental education. 7 dental providers are now on board, with TLC being the predominant provider. Biggest barrier is getting the word out to the public that funds are available for dental services. Funding limit is $500 per patient, with individual cases reviewed for higher payment levels. Amount covered for 28 patients in the last quarter totals $4,386 with approximately $10,000 in funds remaining. In May, will hear if Care First grant is awarded and whether CHRC funding is awarded.

4 George Kaloroumakis, Wicomico Detention Center Have received a preliminary funding commitment from the Governor s Office on Crime Control to establish a Vivitrol program. The program would fund a Vivitrol shot and education when inmates are released, and would include intensive outpatient treatment for the first few weeks, when they are most vulnerable. Should have official grant funding announcement in the next few weeks. Brian Polk from the health department is providing Narcan training at the detention center. He educates both male and female inmates. Steve Walas, HealthSouth Chesapeake Rehab Expansion is complete as of 4/8/16. Would like to host a future LHIC meeting and have members tour the facility in the next quarter once the employee cafeteria is put back together after housing equipment during the expansion. Alane Capen, Coastal Hospice Coastal Hospice and PRMC have partnered to provide end of life nurse education training. Flyer shared. Jennifer Berkman, Eastern Shore Area Health Education Center Office of Health Care Quality funding was received to provide aging simulation training for long term care and assisted living facilities. Training is for staff that work with the aging population. Participated in a HRSA webinar titles The Winds of Change and how change in priorities on the federal level and how it affects us locally. Have been working on HPV vaccination program as an oral cancer prevention program. Visiting pediatricians and dental providers to provide education and increase immunization rates. Chris Hall, Peninsula Regional Medical Center Working on completing a Community Needs Assessment. Have enlisted help from an outside agency to assist in completion. Will be completing key informant interviews on what perceived health needs are locally. If you would like to participate, please contact Chris or Stephanie. Lori asked that Chris present to the coalition once data is compiled. Moving to Epic electronic medical record. Karen Poisker, Peninsula Regional Medical Center Applied for an integrated grant along with McCready Memorial and Atlantic General to develop a regional partnership to improve population health. Submitted in December. Funding announcement has been delayed until May. Proceeding with assessment of ongoing access issues.

5 Stephanie Elliott, Peninsula Regional Medical Center Plan to start van outreach in May, beginning with volunteer nursing staff until funding is announced. Calendars will be published and shared soon. 4/2/16: Health Fest at James M. Bennett High School. PRMC partnered with the Health Department and Lower Shore Clinic to apply for a CHRC grant. The program, called Care Wrap targets dually diagnosed patients out of the ED that require intensive case coordination. Covers transportation to appointments, etc. Met with local EMS to address reducing admissions and ED recidivism. Bariatric program weight loss and wellness program options are available to people that don t qualify for bariatric surgery. Partnering with the MAC center to develop a garden, include physical activity, and support/education from a dietician. The program is a flat fee of $600, payable in installments. Donna Clark, HOPE, Inc. Cold weather shelter project ended at the end of March. Between HALO and church shelters,70 80 homeless men were housed nightly. Next year, the Division Street shelter space will not be available. Currently working with the city to find rent assistance funds for people that had used this shelter. Josh Boston, TLCCS Recently broke ground on the new ob/gyn building at the Woodbrooke campus. New OB and family medicine providers will be joining TLC this summer, replacing a few current providers that are leaving TLC. Dr. James Cockey New State lead testing regulations require children to be tested a 12 and 24 months. Have signed an MOU with a new dental provider for the Village Dental Clinic. Wicomico Health staff have been providing community Naloxone training, and will begin training other agencies to provide training within their own office. TB rates have increased nationally. Maryland s infection rate is lower than the national rate.
