Updates on budget Geoff Vercauteren, DHE Geoff shared some updates form David Cedrone on the budget situation (slide 3) o Although $400,000k was requested, N&AHI will be funded at $200k for this year o As stated before, this is barely enough to make salaries and will provide no funding for grants Updates on subcommittees Geoff Vercauteren, DHE Focus groups were terrific and we learned a lot o Thanks to the colleges and WIBs that helped me organize these o CC s should be proud employers universally said that CC graduates were much better prepared than other candidates, but it is difficult to hold onto them since most leave to go back to nursing or PA school o Geoff will publish finding soon and will share results with rest of AHAG o Also, Geoff will be publishing updated system inventory sheets for CNA/HHA programs and a new one for MA programs Scope and Role subcommittee o Unfortunately Patti-Ann couldn t make it to talk more about the work we are doing o We are still working toward the completion of a white paper addressing the myths and facts of the nursing care team essentially the RN/LPN/CNA team o The paper will take an informational tone first about clarifying the roles of LPNs and CNAs, especially in an acute care environment o Will also point out that CNAs are being asked to do more and more as patient populations get sicker and sicker, and hopefully promote a discussion on where is the appropriate limits to their job duties and capabilities New sub-committee: Nursing Assistant Certification o Purpose of group gather and document the challenges of the current education, testing, and licensing model for CNAs. After that not sure. Would like to bring to DPH New sub-committee: Scalable Projects o Purpose of group - Looking to document ideas that could be scaled up across the state. Second step putting into a frame work we could bring to funders. Third step find funders! Patricia Yu: o Patricia, Leanne and Geoff have all gotten involved with some work at the national level through MFP (money Follows the Person)

2 o It appears they are trying to get a snapshot of the landscape of efforts being made in many states to deal with the AH-DCW issue o We will be on monthly phone calls and doing pre-work for the next months Special guest presentation: o Tim Burgers, Associate Director, Home Care Alliance of Massachusetts : Medicare training requirements for home health aides o See slides for information o Specific comments Slide 17: Currently the guidelines say OR for parts (B) and (C), but language is changing to AND Curriculum should include all three of these Slide 18: This is a reorganizing of reporting of skin conditions. Should be satisfied with inservice trainings This guideline also encourages the training of HHAs with things that are NOT listed, but still critical to the job Slide 19: We are expecting that the guidelines will maintain current language regarding allowance to do training on a psedo-patient (mannequin), but it hasn t been explicitly staed yet Slide 20: Supervision has to be provided by an RN, but took out requirement that the RN needed 2 years of experience Slide 21: Instructor has to be an RN or supervised by an RN with 2+ years of experience, one of which has to be in home care Slide 23: If not already, might want to include in curriculum what does it mean to be part of an interdisciplinary team and what that looks like Slide 24: Important to note supervision of HHA expanded to other professionals besides RN Slide 25 & 26: Much more specific about what supervision of a HHA should include Defined what agency has to cover and what supervisor has to document Update on legislative action happening around HHA right now o Lisa Gurgone & Tim Burgers: There are a number of bills that could effect the home care industry up for debate right now One is to create a registry of HHA CNA: requires registration because of licensure PCA: optional to register HHA: currently no registry HCA and HCA Council support creating some sort of way to keep track of HHAs in the state but need to be very careful with personal info One bill proposed is asking for a lot of personal info (DOB, name, address, etc.) in a searchable website open to the public AHAG Planning: Looking Back Looking Ahead o Geoff talked about where we were at when we began the AHAG and reviewed the goals that were set forth in DHE s Allied Health Direct Care Workforce Plan in June of 2014 o The group was asked, Thinking back to June 2014, how would you characterize that time for the Allied Health Direct Care Workforce? Are things better, same, or worse now? Why or why not? o Responses: The group promotes more talking and sharing of views Great opportunity for sharing CNA curriculum when we began we were all doing different things. In the time we ve been meeting, I feel like we are working to get on the same page and teach the same things Times have changed things have gotten worse for this sector! When we began, unemployment was at ~5%. Now it s at ~3.5%. Even harder to find and retain talented staff.

3 Lack of good pay is still an issue; but also it s about having a good job and feeling valued and respected in your work environment The networking opportunities of the group is important get outside your region The sense of being involved in a collective effort is very helpful Helps those who are deep in this work make sense of the chaos we see around us Good to get views of people who see things from the 30 ft view and the 30,000 foot view The economic distress felt by these workers hasn t changed may have gotten worse Colleges feel like they are doing a better job of being predictive of changes in the sector, and may even be ahead of those changes in some cases Payment system is still so top down. All the money is being spent at the top of the care pyramid, with so little coming down to cover pay increases There are ethical challenges of training people for low-wage jobs that they might leave in 6-12 mos, yet it is still an important job to be done Retention is still an issue with workers going to other employers for $0.25/hr increases, or leaving the industry altogether for other low-wage work that pays better or is easier Political climate is a real challenge. Roughly half of the AH DCW in MA are immigrants. If there is any tightening of immigration policies or change to the status of Haitians like is being discussed it will be devastating for the industry There are no backups for these groups Those policy makers in state government don t know we exist. The make policies that effect the AH DCW often with incomplete information. Is there a way for this group to be a resource to them? Can we increase our voice so those decision makers know they can come to us with questions? Many students that we are training have barriers to their education, to the job search, etc. We should include more CBO s in our group. They are an important group to have at the table as well. One idea was to have IIE come to present on immigration issues We should spread the message of our work more broadly like tweeting, or having a newsletter/update sheet We have accomplished much and there are terrific best practices happening could we run a conference where we can have a larger dialogue about things? Share our successes? Showcase successful partnerships? Where are we now? o Geoff asked the group to write down as many projects, changes, collaborations or any other activity that happened, or is currently happening, that attendees are aware of or involved in, that are making an impact on the training, recruiting, hiring, or promoting of the Allied Health-DCW. o When completed, Geoff asked the group to transcribe those answers onto sticky notes and place them around the room under each of the 6 goals, or under a sign marked other. Geoff went on to instruct that if goals seemed to make sense in multiple goals, copy them and place them in each relevant goal. Results: 1. Create Career Preview Opportunities Offer or increase the length of clinical experiences Offer one day direct care previews Develop recruitment screening tools Bridges to healthcare career trainings with ABE/ESOL partners PCA info sessions CNA/HHA info sessions Researched on methodologies CC s use to orient interested students to healthcare careers

Increase Career Awareness Develop a direct care job website Invest in an information campaign for students (including financial aid awareness) Build direct care worker pathways Create recognized transferrable training Offer partial scholarships for students Created listings for all CNA/HHA/PCA trainings at community colleges posted online Created listings for all Med Assistant trainings at community colleges posted online 3. Change Policy Staff a cross-secretariat task force to identify barriers Launch an advocacy campaign to improve reimbursement and wages Create an innovation fund to support new, untested, promising ideas Building connections with community colleges that didn t exist before (industry/education) PHCAST HHA training Increased awareness of regional educational activities on CC level Regional WIOA meetings re: Healthcare Career Pathways like: ABE/ESOL credentials employment certificates degrees (x2) Collaboration on grants to increase access for workers to get into direct care training programs (Bridges grant; HHA grant) Building bridge to credit for our non-credit CNA programs Development of blended (non-credit/for-credit) CNA/HHA Plus programs Building internal partnerships (non-credit/for-credit); creating pathways and blended discussions CNA/HHA scholarships for private funders Bridge program: CNA/HHA RN BA in Health Economics (DHE grant) Collaboration from non-credit credit university (DHE grant) Provided access through our website to financial aid for typical students 4. Build the Business Case for Investing in the Direct Care Workforce Convene an education, employer and policy collaborative Articulate the business case for investing in the DCW Create regional data sets through the WIBs Launch a campaign to educate people on the value of the DCW in the healthcare system Government agencies collaborating on the AH DCW and LTC workforce (employer) CNA trainings; exploring online opportunities Legislation in progress DHE collaboration with EOEA Challenge: PCA contracts have drawn workers away from CNA/HHA jobs HCA trying to legislate HHA registry proposal 5. Invest in the Development of Core Competencies Align core competencies across jobs and build consensus for support Pilot core competencies curriculum and offer statewide core competency courses Inform DCW curriculum with the employer needs Incorporate life/soft skills into direct care curriculum Align job descriptions and standards of practice Hosting Allied Health Workforce Career Day Developed stackable programs and courses (from non-credit to credit, from certificate up)

6 Other Categories Not Listed in Major Goals HEART Consortium HHA Training with Boston Housing and JVS Lowell Initiative HHA training: International Institute; MCC; Ascentria Home Care Quincy initiative HHA training: Quincy College; Best of Care; Quincy Housing Further integration of institutions (college; state) with immigration services and CBOs College: Began a phlebotomy program in Allied Health division (based initially on feedback heard at AHAG) WIB and regional collaborations specifically on AH-DCW MeHi e-health curriculum designed specifically for CNA/HHA workers MCC; BCC developed curriculum to prepare MA s for national certification exam Berkshire Health Systems and Beth Israel-Milton: Acute care training for CNAs Focused goals of the AHAG o Geoff began a discussion on the focused goals of the AHAG which were identified by members in June of o Attendees were asked to look at their responses to the previous exercise, and think about how they would quantify progress on the goals that we said we would focus on in June 2015? Have we moved the needle? Small group discussion followed by report out. Some topics mentioned were: How would we quantify our progress? # of new programs and graduates Vacancy rate & turnover but not decreasing Enrollment trends crisis pending on # of inquiries for AH DCW programs Pathway progress has been made New goals? o Continue to overcome barriers to linear progression; not all DCW course stack into each other at all schools o Support workers in poverty o Increase retention o Increase focus on advocacy treat this workforce as educated and deserving of higher compensation o Continue to increase career awareness o Increase employer engagement in meetings not a lot of employers come regularly Other comments: Who is the next CNA? Continue to align CC with employer needs Increasing need for LPNs but some programs are closing Some regions can t spend out OJT funds Increase relationship of immigration issues <- -> workforce <- -> social services We should consider being more policy focused; we should start including more sessions on talking policy; need to get legislature more involved; need them to hear what is going on Some focus to be paid to HHAs - what they can and can t do; changing roles More information on benefits cliff; examining the gap between education benefits and job; how do folks weigh training vs. employment vs. benefits of working (for a low wage) vs. not working We should be asking employers, What can we do to mitigate the risk of thinking outside the box? Presentation on MeHi electronic health record curriculum for AH-DCWs
